Malignant tumours presenting after dental extraction: a case series.

OBJECTIVE: To present the clinical spectrum of malignant neoplasms detected after extraction of teeth. DESIGN: Retrospective study. SETTING: Department of Oral and Maxillofacial Surgery, University of Benin Teaching Hospital, Nigeria. MAIN OUTCOME MEASURES: Demographic and clinical data of patients diagnosed with malignant neoplasms related to a history of tooth extraction. RESULTS: Twelve patients with histopathologically confirmed tumours detected after extraction of teeth were studied. There were 11 males and one female giving a male to female ratio of 11:1. They ranged in age from 15-85 years with a mean age of 53 years. Pain and swelling were the most common presenting complaints. The mandible was more often involved seven (58.3%) cases while five (41.7%) cases occurred in the maxilla. Squamous cell carcinoma (in 9 cases) was the most common malignant neoplasm among these patients. CONCLUSION: Malignant tumours can mimic periapical infections necessitating a high index of suspicion when faced with toothache of uncertain cause.

Extramedullary presentation of multiple myeloma in the parotid gland as first evidence of the disease -- a review with case report.

Myeloma is a neoplasm of plasma cells that causes painful, bone-destructive lesions. It may present as a solitary lesion (plasmacytoma), or involve multiple sites (multiple myeloma). Myeloma may occur in an extraosseous location as a solitary lesion, or less commonly, in combination with multiple myeloma. Oral manifestations may be the first sign or symptom of the disease. Soft tissue plasmacytomas of the head and neck tend to involve the nasal cavity or nasopharynx, rather than the oral cavity. Its occurrence in the parotid gland is rare. We present an unusual case of multiple myeloma with extramedullary involvement involving the parotid gland as first evidence of the disease. Multiple myeloma should be considered in the differential diagnosis of salivary gland swellings.

Midline lethal granuloma complicating pregnancy: case report.

A case of midline lethal granuloma in a 28-year- old female Nigerian patient is reported. Oral, ocular and nasal lesions were present and these preceded a spontaneous abortion of a three month old pregnancy. The clinical course of the disease and its similarity to other granulomatous diseases, which are generally classified as midline granuloma syndrome, are highlighted. The prognosis is poor but early diagnosis and treatment appears to improve a patient's condition

Indications for the extraction of impacted third molars in a semi-urban Nigerian Teaching Hospital.

Medical records and radiographs of 450 consecutive patients that had extractions of impacted third molars at a semi urban Nigerian Teaching Hospital were studied to highlight common indications for such extraction. Infections accounted for majority (69.6%) of third molars extracted at the lower jaw, followed by periodontal pocket formation distal to second molars (12.5%). Caries, cysts, jaw fractures and neoplasm were respectively responsible for 5.7%, 0.6%,0.5% and 0.3% of impacted lower third molars extracted. Obscure jaw pain (48.4%) was the most common reason for removing impacted upper third molars. Only 2 (3.2%) upper third molars were extracted because of infections. Since pericoronitis was the most common indication for the extraction of impacted lower third molars, it was suggested that where resources for third molar surgery are limited, it may seem reasonable to prioritise removal of asymptomatic third molar covered by soft tissue alone.

Oral malignancies in Ile-Ife, Nigeria.

Fifty nine patients with histologically confirmed cases of oral malignant neoplasia were included in this study. There was a male preponderance with the male to female ratio being 2.5 to 1.0. The average waiting period before presentation was 8.9 +/- 6.5 months (range: two to 24 months). Squamous cell carcinoma was the predominant variety, 50.8%, of all malignancies recorded. The alveolar ridge and the gingivae were the commonest sites accounting for 40.7% of all cases. Majority of the patients with squamous cell carcinoma, (80.8%), presented with stage IV of this disease. The pattern of oral malignancies in the population studied appears slightly different from earlier observations in Western Europe and Asia. The necessity to strengthen research on the epidemiology of the disease in Nigeria and other African countries is highlighted.

The frequency of tooth transposition in Nigerians, its possible aetiologic factors and clinical implications.

This paper reports the frequency of tooth transposition in Nigerians in an attempt to determine the treatment needs of the population under study. The frequency of tooth transposition in this study is 0.135 per cent consisting of 4 males (36.36 per cent) and 7 females (63.64 per cent), which is higher than those reported by some authors in Caucasian. The higher figure may be due to small sample size or due to greater penetrance of this anomaly in blacks. There were 17 transpositions of which 7 were of mandibular lateral incisors and canines (41.18 per cent) 5 maxillary lateral incisors and canines (29.4 per cent) 4 maxillary canines and premolars (23.5 per cent) and one mandibular canine and central incisors (5.88 per cent). The possible aetiologic factors and challenges to management are discussed in view of the dearth of qualified orthodontists for a population of about 88.5 million people.

Clarification on the differential diagnosis of giant cell containing lesions of the oral and maxillofacial regions.

A broader overview of the essential differences and overlap in the histopathology and clinical presentation of giant cell containing lesions are discussed. This is an attempt to eliminate some confusions attendant to their diagnosis and possible prognosis. These lesions occur both intraosseously and extraosseously. Langhans' type giant cells have been shown not to be an exclusive preserve of tuberculoid granulomas but also of giant cell lesions. The osteoclast like lesions with the exception of giant cell tumour (GCT) are treated by local curettage while the latter is treated by radical resection.

Case report of dentigerous cyst of lower incisor.

This paper reviews the pathogenesis, clinical features and the behaviour of dentigerous cyst. A case of mandibular dentigerous cyst involving the incisor tooth (an unusual site) is presented to illustrate this pattern of behaviour. A recurrent pyogenic granuloma in the region of the lower incisors prevented the eruption of the lower left incisor with subsequent cystic formation around the tooth. The histological and radiological presentation is consistent with that of a dentigerous cyst. The significance of the potential of the cyst lining is stressed. The cyst was enucleated with the associated tooth as the latter was not in a favourable position to erupt. There was no recurrence after one year of treatment.

Histological identification of mitosis in oral mucosal lesions--problems and solutions.

Additional criteria for identification of mitosis in conventional histological sections of the oral mucosa were devised. The application of these criteria and the use of Harris's haematoxylin have helped to resolve some inconsistencies in recognizing mitotic figures which are slightly different from those seen in classical cell culture studies.

Mitotic phase distribution in oral epithelial hyperplasia and neoplasia.

Assessment of mitotic phase distribution in oral epithelial lesions has shown closely similar features in two categories of hyperplastic condition. By contrast, squamous cell carcinomas showed a significantly different mitotic phase distribution with an increased proportion of metaphases suggesting delay or blockage at this phase. Squamous cell papillomas showed an increased proportion of prometaphase mitoses.

A scoring system for sick children.

A scoring chart for sick children was designed using the vital signs of temperature, respiratory and pulse rates, and the central nervous system. The scoring chart was used on 68 sick children who had been randomly selected from a child welfare clinic in Lagos. Analyses of the charts showed that although values overlapped when related to clinical impression, there was an inverse correlation with the severity of the illness. The correlation coefficient between the doctors' and nurses' scores was high. Despite the need for further modification and investigation the scoring chart was promising as a screening tool for use by people who were not doctors and, possibly in some situations, by those who were not nurses.
