[Effectiveness of combined treatment of infective endocarditis].

The purpose of the study was to develop and perfect complex therapy of modern infective endocarditis (IE). Ten-year experience in clinical diagnostics of 240 patients with acute, subacute, and lingering IE was summarized; the effectiveness of their treatment was evaluated. The features of modern IE were acute and subacute course of the disease with the involvement of aortal (37.8% of cases), tricuspid (16.8%), mitral (15.2%) valve, and septicemia, caused by highly virulent microflora (staphylococci--51% of cases; gram-negative bacteria--13%; fungi--5%). The clinical manifestations of modern IE included toxic infectious syndrome (24% of cases), heart failure (21%), symptoms of complications (16%), central hemodynamics disturbances (12%), and splenomegaly (8%). The treatment included antibacterial, pathogenetic, and symptomatic therapy, as well as extracorporal hemocorrection and cardiosurgery. Analysis of the results of surgical treatment of 140 IE patients allowed development of indications for valve prosthesis.

[The role of endothelial dysfunction in the pathogenesis of combined cardiopulmonary diseases].

The aim of the study was to evaluate the endothelial function in patients with coronary heart disease and an exacerbation of chronic obstructive pulmonary disease (COPD), and the role of endothelial dysfunction (ED) in the pathogenesis of reciprocal exacerbation syndrome in cases of combined cardiovascular pathology. The subjects were 366 patients with single or combined cardiovascular diseases, in whom non-invasive evaluation of the endothelial regulation of the vascular tone was performed, and serum levels of NO were measured. The study revealed the biggest number of cases with an altered endothelium-dependent vasodilatation, and the most prominent decrease in NO level in patients with CHD and COPD. All the patients were followed up during two years in order to compare clinical manifestations of single and combined cardiovascular pathology. The results show that ED may be considered a predictor of complicated clinical course of COPD in patients with CHD.

[Immunology-related aspects of gastroesophageal reflux disease and Barrett's esophagus formation].

The purpose of the research was to study particular features of the cellular immune status and serum cytokines in patients with the gastroesophageal reflux disease (GERD) and Barrett's esophagus. We have examined 70 patients with erosive GERD and 42 patients with Barrett's esophagus. Clinical, endoscopic, morphological and immunological study methods have been used. Distinctions in parameters of the cellular immune status and serum level of Interleukin-4, Interleukin-8, Interleukin-10, tumor necrosis factor-alpha and interferon-gamma in patients with GERD in the dynamics of treatment and patients with Barrett's esophagus were determined.

[Features of gastroesophageal reflux disease in young people].

The article contains ethiopathogenic, clinical and diagnostic criteria of gastroesophageal reflux disease (GERD) in young people. GERD verification was based upon the results of the examination of 137 men aged 18 to 23 years. These results included clinical findings as well as the data from the following tests: intraesophageal and intragastric pH-metry with evaluation of parietal cell response to atropine, and endoscopic and morphological examination of the upper division of the gastrointestinal tract including detection of Helicobacter pylori (HP) expansion, evaluation of inflammatory and regenerative processes in the mucosa of the lower third of the esophagus and the antral part of the stomach. The results show that the feature of GERD in the given age group is a certain morphological picture in the esophagus characterized by a various degree of squamous epithelium dystrophy with almost no metaplasia, as well as a high degree of HP adhesion in the esophagus with atropine-resistent hyperchlorhydria in the stomach. It shoud be noted that eradicational and antisecretory therapy in young people with GERD leads to the reestablishment of the esophagial mucosa structure.

[Melanin-producing and NO-synthase gastric cells and the processes of cell regeneration in gastric and duodenal ulcers].

The subjects of the study were 50 patients with gastric ulcer, 110 patients with duodenal ulcer, 70 patients with chronic Helicobacter-associated gastritis, and 60 practically healthy individuals. Diagnosis was based upon the results of clinical, instrumental, and morphological examination. The results of the study show thatfunctional activity of gastric epitheliocytes and apudocytes, manifesting in decrease or increase of certain hormone activity, correlates with the process of apoptosis, which leads either to activation of ulcerogenesis, or to healing of a peptic ulcer. The study established the role of NO-positive melatonin epitheliocytes in peptic ulcer development.

[The role of stomach epithelial cells being immunopositive for NO synthase in the formation of chronic cholecystitis].

The article provides information on 145 patients with chronic acalculous cholecystitis and 62 patients with chronic calculous cholecystitis subjected to a study of the quantitative characteristic of gastric antral epithelial cells immunopositive for NO synthase. The increased number of NO synthase immunopositive gastric epithelial cells at chronic cholecystitis was discovered. Most of them were discovered at the calculous process. The study discovered the relation between the gallbladder inflammatory activity, bile lithogenicity and the number of NO synthase immunopositive gastric epithelial cells. The clinical, biochemical and morphologic analyses showed that the application of Hepabene for treating patients with chronic acalculous cholecystitis produces a clinical and biochemical remission in a shorter term. Bile lithogenicity decreases; the quantitative characteristic of NO synthase immunopositive gastric epithelial cells normalizes. The study results make it possible to recommend Hepabene as a highly efficient drug for treating patients with chronic acalculous cholecystitis.

[Regeneration of the esophagus epitheliocytes. Evaluation of pariet efficacy in the treatment of patients with reflux esophagitis].

In this paper the information of treatment by Pariet (Rabeprasol) in 20 patients with erosive reflux-esophagitis are represented. On the basis of clinico-endoscopic, morphological and immunohistochemical investigations it was shown, that using Pariet in treatment of the patients with erosive esophagitis the clinical-endoscopic remission, the regression of inflammation and apoptosis processes and proliferative epithelial activity is achieved in more short terms. The results of observation allow to recommend Pariet as a high effective preparation for treatment of the patients with gastroesophagities reflux.

[Esophageal epithelial cells renewal in evaluation of the pariet efficacy in the treatment of patients with reflux esophagitis].

The article provides the data on 20 patients with erosive reflux esophagitis, who were treated with Pariet (rabeprazole). It was shown on the basis of conducted clinical endoscopic, morphological and immunohistochemical studies that the application of Pariet in treatment of patients with erosive esophagitis contributes to clinical endoscopic remission, regression of inflammatory changes, recovery of apoptosis processes and proliferative activity of esophagus epithelial cells within a shorter period of time. The results of the observation allow recommending Pariet as a highly efficient preparation for treatment of patients with gastroesophageal reflux disease.

[Content of calretinin-immunoreactive epitheliocytes of the stomach in patients with chronic gastritis and peptic ulcer].

The article covers the information about 50 patients with gastric ulcer (GU) and 110 patients with duodenal ulcer (DU). 60 healthy persons and 70 patients with chronic helicobacter gastritis (CHG) formed comparative groups. Calretinin-immunoreactive epitheliocytes in antral region's gastric mucous membrane were reaved by clinical-endoscopic morphological and immunohistochemical investigations. It is shown that hyperplasia of calretinin-immunoreactive cells is in CHG cases. On the contrary, in DU and GU there is hypoplasia of calretinin-immunoreactive cells. Diametrically opposed data on quantitative description of focused endocrine cells in patients with CHG and peptic ulcer disease permitted to make a conclusion about the importance of calretinin-immunoreactive cells in peptic ulcer. The results of the investigation made it possible to give the prognostic appreciation of CHG course and its transformation into peptic ulcer disease.

[Morphofunctional characteristics of endocrine gastric cells in chronic biliary pancreatitis].

Clinicoendoscopic morphological and morphofunctional studies in 50 patients with chronic cholecystitis and 50 patients with chronic biliary pancreatitis have demonstrated that morphometric analysis of the antral stomach endocrine cells secreting melatonin, neurotensin and somatostatin can be used in addition to standard laboratory and device methods for differentiation chronic biliary pancreatitis with biliary pathology.

[Apudocytes and mast cells in chronic inflammation of colon: clinicomorphological correlations].

AIM: To study functional morphology of a total population of endocrine cells of colon mucosa, mast and enterochromaffine cells. MATERIALS AND METHODS: Light and electrone microscopy, immunohistochemical methods, morphometry were used to study endocrine and mast cells of the sigmoid colon in inflammation. RESULTS: Changes of functional morphology and size of endocrine and mast cell population as well as apudocytes producing serotonin, melatonin, vasointestinal peptides were stated. Apudocyte and mass cell functional morphology, clinical symptoms and mucosal structural changes correlated. Specificity of some parameters in Chron's disease is shown. CONCLUSION: The results may provide additional criteria in diagnosis of different variants of chronic colitis.

[Melatonin and serotonin in inflammatory colon diseases and colorectal neoplasms].

Patients with inflammatory intestinal diseases and colorectal adenocarcinoma in sigmoid mucosa were examined using light and electron microscopy, immunohistochemical tests, radioimmunoassay and morphometry. Functional morphology of the general population of the endocrine cells and apudocytes producing melatonin and serotonin as well as urine excretion of 6-sulfatoxymelatonin changed in quality and quantity. The parameters showed specificity depending on clinicomorphological variant of colorectal pathology. The findings provide additional criteria in diagnosis and prediction of the course of different types of inflammatory and tumor affections of the colon.

[Changes in the APUD system of the large intestine as a risk factor for colorectal cancer].

A common population of endocrine cells, apudocytes producing serotonin, melatonin, vaso-intestinal peptides and mast cells were examined on unaffected mucous membranes of 27 colorectal cancer patients. Light and electron microscopy, immunohistochemical and morphometric methods were used. A marked decrease in total number of apudocytes, hypoplasia of cells producing serotonin, melatonin, vaso-intestinal peptides were revealed at early stages of cancer irrespective of the tumor structure. Mast cells in premetastatic period outnumbered those after emergence of metastases. The results obtained may serve as additional criteria of early colorectal cancer diagnosis.

[Chronic cholecystitis--some lithogenic aspects].

A total of 100 patients were examined: 35 patients with biliary dyskinesia, 44 patients with chronic acalculous cholecystitis, 21 patients with chronic calculous cholecystitis. The patients were evaluated clinically with investigation of motor-evacuation function of the gall-bladder, activity of inflammation in the gall-bladder, bile lithogenicity, morphofunctional characteristics of the stomach APUD-cells and degree of Helicobacter pylori contamination. Chronic calculous cholecystitis is characterized by clearcut symptoms, hypomotor dyskinesia, high cholesterol, signs of diffuse atrophic gastritis. Chronic acalculous cholecystitis with hypomotor dyskinesia was accompanied by mild intoxication, presence of C-reactive protein, high cholesterol, diffuse atrophic gastritis.

[Quantitative characteristics of various endocrine cells of the duodenal bulb in the pre-ulcer conditions and duodenal ulcer].

Endocrine cells of the duodenal bulb producing adrenalin, noradrenalin, serotonin, histamine and beta-endorphine were assayed in 12 patients with preulcer and 39 patients with duodenal ulcer. It is shown that preulcer was mainly characterized by hyperplasia of endocrine cells in the duodenal bulb mucosa, secreting adrenalin and noradrenalin in the presence of the deficiency of beta-endorphine-producing cells. The recurrent duodenal ulcer was associated with a dramatic fall in the number of catecholamine-containing endocrine cells in the duodenal bulb.
