The "intra-abdominal version technique" for delivery of transverse lie by low-segment cesarean section.

Cesarean section is the delivery method of choice in all shoulder presentations of viable fetuses. The classic cesarean section is usually recommended when there is a transverse lie because of the potential technical difficulties associated with extraction of the fetus when a low-segment incision is used. This paper illustrates a technique that avoids the need of a classic cesarean section and permits the use of the more desirable low-segment cesarean section. The advantages of the procedure are discussed.

Ectopic pregnancy as an etiologic agent in appendicitis.

The association of ectopic pregnancy and acute appendicitis has seldom been discussed in literature. This report presents 2 patients who had simultaneous right tubal pregnancies and acute periappendicitis. The cases reported here suggest the possibility that an ectopic pregnancy can produce an inflammatory reaction of the appendix. Ectopic pregnancy as an etiological stimulus for acute appendicitis is discussed. The advisability of examining the appendix at the time of surgery for pelvic disease, and performing an appendectomy at the same time, if pathology of the appendix is suspected, is emphasized. The use of elective appendectomy in surgery for ectopic pregnancy is reviewed.

The effect of antibody against a purified sperm-coating antigen on human sperm.

A purified fraction of human seminal plasma containing a sperm-coating antigen and two minor contaminants was used to immunize rabbits by intravenous route. An antiserum containing only the antibody against the sperm-coating antigen as examined by immunoelectrophoresis was obtained from one rabbit. The effect of this antiserum on human sperm was examined by sperm-immobilization and sperm-agglutination tests. The results revealed that the antibody against the purified sperm-coating antigen was incapable of immobilzing or agglutinating human sperm. This indicates that the purified sperm-coating antigen is unlikely to be useful as an antifertility antigen for immunologic fertility control.

Galactorrhea associated with primary hypothyroidism. Report of two cases.

Two patients with primary hypothyroidism and galactorrhea are described. Elevated serum prolactin and thyroid-stimulating hormone levels were present. Following adequate thyroid hormone replacement restoration of clinical euthyroidism and cessation of galactorrhea occurred with a fall of both TSH and prolactin levels to normal.

Prenatal diagnosis of chromosome abnormalities.

It is possible to define a group of prospective parents who are at risk of producing offspring with disability due to chromosomal abnormalities. Such individuals should have evaluation of the 15 or 16 week fetus by karyotype of amniotic fluid cells. The techniques and problems of such diagnostic procedures are discussed.
