Feedback to nurse managers about staff nurses' perceptions of their jobs.

This article describes a survey feedback intervention in which staff nurses were surveyed about various job characteristics, job satisfaction, and intent to remain in the organization. Nurse managers received the feedback through graphs and a workshop. A year later the same survey was conducted, and the results were compared with preintervention data. Of the 13 units surveyed, six showed significant improvement in one area and one showed significant improvement in 11. Nurse managers considered the survey feedback helpful, but the feedback alone was not sufficient for achieving broad changes in 1 year. However, the feedback is a useful component of continuous quality improvement efforts.

Health promotion across the continuum: challenges for the future of cardiovascular nursing.

Issues surrounding health care spending, reform, and future directions have generated much concern and debate among health care providers. Increasingly, cardiovascular nurses are challenged to respond to the health care needs of clients in the 21st century. This article discusses trends in the health care system, including the current emphasis on illness and emerging efforts supporting health promotion. The discussion includes an overview of health care restructuring and the use of care management as a comprehensive strategy for cardiovascular nursing practice to support health promotion across a trajectory of illness and wellness.

Making the hidden obvious. Management education through survey feedback.

Staff nurse perceptions of the work setting are powerful indicators of job satisfaction and often guide intent to remain or leave the job. In this study, staff nurse perceptions of the unit work environment were disclosed to nurse managers in an innovative management education strategy. Equipped with unit-specific information from their staff members, nurse managers found that they were better able to address the factors identified as most important. Use of survey-based information promoted better management decisions and a higher level of confidence.

Regulation of insulin-like growth factor-I (IGF-I) and IGF-binding proteins by growth hormone in rat white adipose tissue.

Insulin-like growth factor-I (IGF-I) mRNA levels in rat white adipose tissue (WAT) are in the same range as those in liver, the major source of serum IGF-I, and are far above the levels in other tissues. IGF-I mRNA and IGF-I peptide levels in WAT decrease drastically after hypophysectomy and are restored to near normal by GH treatment in vivo. IGF-I gene expression in WAT from hypophysectomized rats is also stimulated by GH in vitro; half-maximal stimulation of IGF-I mRNA occurs between 0.25-0.5 nM GH, and maximal stimulation (3.5- to 5.5-fold) at 15 nM after 2 h. However, maximally stimulated IGF-I mRNA levels in vitro lie far below those measured in vivo in normal or GH-treated hypophysectomized rats. T3 does not enhance the GH effect in vitro. Rat WAT expresses the messages for IGF-binding protein (IGFBP)-2, -3, -4, -5, and -6. IGFBP-2, -3, -5, and -6 mRNAs are all regulated by GH. IGF-I mRNA and IGFBP-5 mRNA are localized in both adipocytes and stromal-vascular cells, whereas IGFBP-2 and -3 expression is restricted to stromal-vascular tissue. As physiological concentrations of IGF-I are able to induce adipocyte differentiation in vitro, we suggest that locally produced IGF-I and IGFBPs play a crucial role in the in vivo differentiation of adipose cells from stromal-vascular cells.

Mechanism of monoclonal antibody inhibition/stimulation of reactions catalyzed by cytochrome P450IIB1.

We describe two monoclonal antibodies (MAbs) against rat cytochrome P450IIB1 and investigate the mechanisms by which they influence P450IIB1-mediated catalysis. MAb ce9 partially inhibits the activities toward p-nitroanisole, 7-ethoxycoumarin, and benzphetamine as well as NADPH oxidation. These findings can be explained by the observation that ce9 cross-links P450 to form large aggregates resulting in the inhibition of the functional interaction with NADPH cytochrome P450 reductase. Binding of ce9 to P450IIB1 does not affect the spin state of the P450 heme, as revealed by comparing the magnetic circular dichroism (MCD) spectra of free and antibody-bound P450IIB1. On the other hand, the second antibody tested, MAb 14E10, induces a remarkable low to high spin transition upon binding to P450IIB1, as shown by MCD difference spectroscopy. This MAb stimulates activities toward p-nitroanisole and 7-ethoxycoumarin without affecting the rate of NADPH oxidation. This observation indicates that MAb 14E10 may increase the efficiency of electron utilization by P450IIB1. Benzphetamine metabolism remains unchanged in the presence of MAb 14E10.

Managing complexity in health care settings.

Health care settings are becoming ever more complex. With this increased complexity, however, people often become less clear about their responsibilities and accountability. In this article, the authors discuss the process of responsibility charting and illustrate how it can be used to clarify the roles of the many different people whose work may be affected by the implementation of new programs and procedures. Responsibility charting is a structured process for establishing sound patterns of communications and collaboration over time.
