The natural history of Sudanese post-kala-azar dermal leishmaniasis: clinical, immunological and prognostic features.

In an exploration of the natural history of post-kala-azar dermal leishmaniasis (PKDL), 134 residents of Sudan who had recently been diagnosed as cases of the disease were investigated. In each case, diagnosis had been based on clinical criteria, the temporal relationship between the rash and the treatment of visceral leishmaniasis (VL), positive results in direct agglutination tests (DAT) and/or leishmanin skin tests (LST), and the exclusion of other skin conditions. The mean (S.D.) age of the subjects was 6.4 (3.0) years. Although PKDL appeared commonest among those aged 4-8 years (P < 0.05), it was most severe in children aged <or= 3 years. Males and females were equally affected. The rash developed 0.5-13 months following the apparently successful treatment of VL, a relatively rapid onset being associated with severe disease (P = 0.01). The distribution of the rash on the body often reflected the clothing habits of the case, being most frequent on skin that was regularly exposed to the sun. Spontaneous healing of the lesions occurred within 12 months in the majority of cases, the PKDL lasting for a mean (S.D.) of 9.7 (4.7) months. Persistence of lesions beyond 1 year was significantly associated with the combination of a high titre in the DAT and a negative result in the LST (P < 0.05). In conclusion, spontaneous healing is commonly seen in Sudanese patients with PKDL. Persistence of the lesions is frequently associated with non-reactivity in the LST and high levels of anti-leishmanial antibodies.

Second-line chemotherapy for small-cell lung cancer: a review.

Small-cell lung cancer (SCLC) is an aggressive malignancy, and only a minority of patients survive 2 years. Although this cancer is sensitive to both chemotherapy and radiotherapy, the majority of patients relapse, and second-line treatment is an option for many. Currently in the United States, the combination of cisplatin/etoposide is the standard first-line therapy in SCLC. At this time, topotecan is the only Food and Drug Administration-approved chemotherapeutic agent for second-line treatment of SCLC. In this paper, we review studies of second-line chemotherapy for SCLC.

Study on the effect of Bacillus thuringiensis (B.t. serotype H-14) and its toxins against Cephalopina titillator (Clark), the Egyptian camel myiasis producing dipterous fly.

Bacillus t. israelensis (B.t. serotype H-14) and its toxins have a marked lethal effect on both the eggs and the newly moulted 3rd stage larvae of Cephalopina titillator. The Bacillus was less effective than Bacillus and its toxins on both the eggs and larvae. On the other hand, the larvae were more affected by both Bacillus and Bacillus and its toxins than the eggs. Prolonged exposure time showed more lethal effect.

Rectosigmoid bilharzioma causing intestinal obstruction.

Schistosomiasis is an infection caused by flukes (trematodes) of any species of the genus Schistosoma whereas bilharzioma implies a localized mass of fibrous and inflammatory tissue, which usually contains many eggs, frequently involving serosa and mesentery. Bilharziomas are probably caused by a reaction to numerous eggs produced by one or more pair of worms in a single site, although altered host reactivity may also be a factor. Bilharziomas, even in endemic areas are quite uncommon lesions in adults. So far they have not been reported to cause intestinal obstruction in children. Herein we present a case masquerading as large bowel malignancy in a child. Management and follow up are also mentioned.

On the condition and activities of Jeddah Endoscopy Center.

Present condition and activities of Jeddah Endoscopy Center managed in cooperation with Saudian and Japanese doctors are reported. Total number of patients examined are 1,183 cases; 73% of which were subjected to endoscopy and 27% were to fluoroscopy. With respect to the stomach diseases, gastric cancer and ulcer were found in 16% and 14.7%, respectively. On the other hand, high incidences of severe atrophic gastritis and haemorrhagic or erosive gastritis accounted totally 64.7% of all stomach diseases were noted. Out of 264 cases of duodenal diseases, duodenal ulcer was diagnosed in number of 202 cases (76%). The ratio between gastric and duodenal ulcer was, therefore, approximately 1:6. It was further noted that most of patients with esophagus cancer were found in males having the local habit of taking green tobacco. From the results obtained, discussions were made on the statistical differences of gastrointestinal diseases between Saudi Arabia and Japan.

Malignant neoplasms in Saudi Arabia.

In a sample of 1000 consecutive malignant neoplasms in Saudis resident in the Western Region of Saudi Arabia, malignant lymphoma was the commonest of the life-threatening malignancies. The differences between malignant lymphoma in this sample and Western series include the greater frequency of lymphoma; the tendency for reticulum cell and poorly differentiated lymphomas to present as abdominal lesions; the earlier peak of prevalence of Hodgkin's disease, and the dissimilar proportions of its subtypes. The distribution of cancers in the gastrointestinal tract in our sample is almost the reverse of that encountered in the West in that cancer of the mouth and esophagus were more common than cancer of the lower intestinal tract. Lung cancer was relatively uncommon. The smoking habit is not so prevalent in Saudi Arabia as in the West and there is a need to maintain this situation by discouraging smoking. Cancer of the breast was by far the commonest major malignancy in the female, although most Saudi women have their first child early in their reproductive life. Skin cancers proved to be the most prevalent malignancy, and of these squamous cell carcinoma was the most common. The biases that affect studies such as ours in Saudi Arabia are stressed.
