Value of video-assisted thoracic surgery in traumatic extrapleural hematoma.

The current article reports on a rare case recently experienced, in which a medially displaced extrapleural fat layer with parietal pleura, revealed by CT scan of the chest, was a sign of traumatic extrapleural hematoma. Video-assisted thoracic surgery was not suited to approaching and managing the extrapleural hematoma, so that a limited thoracotomy was mandatory. Extrapleural hematoma should be considered a relative major contraindication to video-assisted thoracic surgery.

Coadministration of basic fibroblast growth factor and sucrose octasulfate (sucralfate) facilitates the rat dorsal flap survival and viability.

The effective use of local growth factors and cytokines may replace the lengthy staged surgical delay process. We tested the efficacy of basic fibroblast growth factor (bFGF) coadministered with sucralfate (sucrose octasulfate) on the rat dorsal flap model. A total of 76 male Wistar rats were used in this experiment. Four groups of the animals were divided. Group 1 (n = 5) was the vehicle control (saline soaked), group 2 (n = 5) was sucrose octasulfate soaked (100 microg/ml, 1 ml), group 3 (n = 5) was bFGF soaked (1 microg/ml, 1 ml), and group 4 (n = 5) was both bFGF and sucrose octasulfate soaked. All agents were soaked equally in Gelfoam. The flap survival measured by the quantitative computer-assisted morphologic analysis was significantly improved by day 5 postoperatively in the combined administration group compared with the vehicle control (81 and 53 percent, respectively; p < 0.05). In lead oxide-gelatin microangiography, there was enhanced pedicle vessel formation observed as well as the extended vessel sprouting up to very close to the distal end in combined group on day 5. The endogenous bFGF mRNA expressions shown by reverse transcriptase-polymerase chain reaction were detected in all four groups. The angiogenesis indicated by alpha-smooth muscle actin immunopositivity was significantly more enhanced in the combined group than the vehicle control (37.3 and 19.4, respectively; p < 0.01). In the combined group, there was stronger immunopositivity for bFGF in epidermis and hair follicles observed, and more notably bFGF-immunopositive dermal fibroblasts were evident. Thus, coadministration of bFGF and sucralfate markedly facilitates the rat dorsal flap survivability by enhancing the bFGF expression and angiogenesis.

A furanoid labdane diterpene from Potamogeton nodosus.

A new furanoid diterpene, 15,16-epoxy-12-oxo-8(17), 13(16), 14-labdatrien-20,19-olide (1) was isolated from an ethanolic extract of Potamogeton nodosus. Its structure was elucidated by the usual spectroscopic methods, including 2D NMR techniques. Compound 1 was found to exhibit moderate inhibitory activity against a number of both Gram-positive and Gram-negative bacteria.

Mediastinal perforation and contralateral hemothorax by a chest tube.

Tube thoracostomy is an invasive and common procedure that is often life-saving, but by no means innocuous. We describe herein a case of chest trauma in which the chest tube crossed through the mediastinum between aorta and esophagus and penetrated the contralateral pleural cavity causing mild hemothorax. A literature search has failed to identify a similar case: the misplacement was detected in a control radiograph which led to early adjustment of the tube and no sequalae.

A simple technique for anchoring chest tubes.

Although some techniques have been described for securing chest tubes, many complications are still reported. A simple method is described for securing chest tubes and closure of the wound, resulting in good fixation of the tube and an airtight closure of the chest wall with a cosmetically satisfactory result.

Acquired pure red cell aplasia--a case report.

A married female patient of 36 years with chronic anaemia, because of pure erythroid aplasia with a haemolytic component and hypothyroidism due to antithyroid auto-antibodies, was subsequently discovered as a case of systemic lupus erythematosus (SLE). She was treated with corticosteroid and immunosuppressive therapy and her anaemia was corrected. The response of erythroid aplasia to corticosteroid and other immunosuppressive agents suggests that immunological factors play a role in erythroid aplasia in SLE. The occurrence of red cell aplasia in association with a variety of immune phenomenon supports the concept that in SLE, erythroid aplasia may be of immune aetiology.

Isolation and characterization of new anti-HIV and cytotoxic leads from plants, marine, and microbial organisms.

New cytotoxic isomalabaricane triterpenes have been isolated from a sponge Stelletta sp. (1-7); anti-HIV pterocarpans (8 and 9) and isoflavanoids (12-16 and 18) were elucidated from two tropical plants in the genus Erythrina; and anti-HIV enniatins (20 and 22-23) were characterized from fungi in the genera Fusarium and Alternaria. The enniatins were evaluated for in vivo anti-HIV activity in the hollow fiber assay.

Chemotherapy of acute myeloblastic leukaemia with DAM regimen.

32 patients of denovo-ANLL were treated with Doxorubicin, Ara-C and 6-Mercaptopurine (DAM) regimen. Remission induction was instituted with 1-3 cycles of DAM regimen and maintenance was given by 6-MP continuously with intermittent DA (1,5) regimen. In the remission induction, Doxorubicin 30 mg/m2 for 3 days, Ara-C 150 mg/m2 for 5 days and 6-Mp 100 mg/m2 daily was given. Complete remission (CR) was observed in 60% cases. The probability of 2 years disease-free survival of patients with complete remission is 56.73%.

A benzoic acid glycoside from Geniostoma antherotrichum.

Fractionation of an HIV-inhibitory organic extract of Geniostoma antherotrichum afforded a glycoside derivative, which has been characterized as 2-hydroxy-3-O-beta-D-glucopyranosyl-benzoic acid (1) on the basis of spectral analyses. The HIV-inhibitory activity of the extract was traced to polymeric tannins, while 1 was found to be inactive in the National Cancer Institute's primary anti-HIV screen.

Patellamide F, A new cytotoxic cyclic peptide from the colonial ascidian Lissoclinum patella.

Cytotoxicity-directed fractionation of an organic extract of the tunicate Lissoclinum patella afforded a new cyclic octapeptide, patellamide F [1]. The structure and absolute stereochemistry of 1 were established by a combination of spectroscopic and chemical methods. Three known cyclic peptides, patellamide B [2], ulithiacyclamide, and lissoclinamide 3, were also isolated and identified.

Mycalolides D and E, new cytotoxic macrolides from a collection of the stony coral Tubastrea faulkneri.

Fractionation of a cytotoxic extract of the stony coral Tubastrea faulkneri yielded a series of cytotoxic polyoxazole macrolides and several noncytotoxic indole derivatives. Two new macrolides, mycalolides D [1] and E [2], were isolated and identified, in addition to the known compound mycalolide C [3]. The macrolide structures were elucidated by detailed analysis of their spectroscopic data and by comparison with related compounds.

Classification of leprosy cases under field conditions in Bangladesh. I. Usefulness of skin-smear examinations.

In 2 non-governmental organization projects in Bangladesh 244 new leprosy patients were classified in the field according to clinical criteria. Skin smears were taken at 4 standardized sites and at the most active peripheral lesion, where a biopsy was also taken. Comparison of the clinical field classification with the results of the skin smears and biopsies gives a sensitivity of 92.1% for the clinical criteria, but a specificity of only 41.3%. The skin-smear results, on the other hand, have a sensitivity of 88.4% and a specificity of 98.1%. Thus, skin smears may contribute considerably to the operational classification of leprosy patients under field conditions. Quality control of the peripheral laboratory is essential. Appropriate site selection for the smear taking will also contribute to increased performance. Analysis of the skin-smear results suggests that the policy of taking smears at standardized sites should be abandoned in favour of the earlobes and active peripheral lesions.

Classification of leprosy cases under field conditions in Bangladesh. II. Reliability of clinical criteria.

In 2 non-governmental organization projects 244 new leprosy patients in Bangladesh were classified in the field according to clinical criteria i.e. number of skin lesions and number of enlarged nerves. Comparison of these classification results with the results of skin smears and biopsies yielded a sensitivity (for detection of a MB case) of 92.1%, but the 'unconfirmed MB rate' amounted to 52.6%. In order to improve the reliability of the operational classification, several additional clinical criteria were investigated. It was found that neither the presence of anaesthesia in the skin lesions nor the presence of grade 2 disabilities or peripheral anaesthesia or voluntary muscle testing (VMT) impairment contributed to an improved classification. Counting the number of body areas showing signs of leprosy, which had proven very useful in other programmes, did not result in a more reliable classification in the 2 projects in Bangladesh. The presence of clinical signs of lepromatous leprosy, more specifically nodules or diffuse infiltration, could be a useful addition to the classification criteria. If the sensitivity must remain higher than 90%, the lowest 'unconfirmed MB rate' obtainable in Bangladesh, using clinical criteria only, is 46.4%, for a sensitivity of 91.0%. However, the inclusion of skin-smear results in the classification criteria could improve the sensitivity to 96.6% and lower the 'unconfirmed MB rate' to 40.3%. A reduction in MB overclassification will result in more efficient and more cost-effective leprosy control programmes.
