Treatment of melanotic spots in the gingiva by argon laser.

Dark colored areas, ranging from light blue to black, often occur in the gingiva as a result of the abnormal deposition of melanin. These can develop at any age, creating an unesthetic appearance. The effective elimination of these melanotic areas can be accomplished by treatment with monoline 514 nm green light (1.5 W, 300 milliseconds, 0.5-mm spot size) produced by an argon laser. Restoration of the mucosa is optimal, giving excellent esthetic results.

Monoline argon laser (514 nm) treatment of benign pigmented lesions with long pulse lengths.

A dichroic filter was adjusted in order to make use of the green line of an argon laser for the treatment of pigmented lesions (actinic and senile lentigo, ephelis, café-au-lait marks and spilus nevus). Using a power of 1.5 W, a spot size of 0.5 mm and pulse lengths of 200 or 300 ms, satisfactory elimination of 620 pigmented cutaneous lesions was achieved. Although the pulse lengths used were much longer than the thermal relaxation time of the melanosomes, the histology after treatment showed little damage of adjacent structures. Good restoration of the epidermis and a return of normal pigmentation led to excellent cosmetic results, with few complications.

Bone fracture consolidates faster with low-power laser.

Low-power laser radiation is currently used in the treatment of pain and osteoarticular inflammation. However, the mechanisms of the laser biostimulating effects on tissue are still not completely understood. With laser treatment, we have achieved activation of osseous regeneration in human bone fractures. After 7 years of positive clinical control in human beings, we decided to start an experimental study of fractures in the tibia of mice, histologically controlling its reparation after exposure to 632 nm. He/Ne laser in doses of 2.4 Joules in one point was used. The radiation was directly applied to the area of fracture in a series of 12 treatments (one treatment every second day). By optic microscope we observed, in the treated animals, an important increase in vascularization and faster formation of osseous tissue with a dense trabecular net compared to the control group, which presented only chondroid tissue and poor vascularization corresponding to an earlier stage of bone consolidation (controls were also analyzed by electron microscopy). Potentially, the laser effect might modulate the function of osteocytes, promoting faster metabolism and reaction of bone callus.

He/Ne laser treatment of hemorrhoids.

In the treatment of first and second degree hemorrhoids, the He/Ne laser is an effective, simple and harmless clinical procedure, as an alternative to medical therapy or surgery. The authors describe the treatment carried out, indicating method and results of the application of red light 632.8 nm He/Ne laser. On the basis of therapy carried out at intervals of 5 days for a total of 6 treatments, a statistic control was reached. Results lead to the conclusion that greater or lesser laser radiation intensity lacks apparent importance. Treatment is effective in pain alleviation from the first session and final results are excellent as is shown by the table of statistics. The effects produced on rectum mucous are discussed offering a hypothetical explanation for its therapeutical action.

Periocular skin reshaping by CO2 laser coagulation.

When blepharoplasty is performed by transconjunctival route, sometimes the excess of skin created by overlaxness of the lower lid does not retract to achieve the desired cosmetic result. We propose a method for controlled retraction of the eyelid by coagulating the skin using the CO2 laser. Thanks to this approach, the lower eyelid's tone recovers and the desired cosmetic appearance is achieved. Results of 37 patients that were treated by 10 W CO2 laser, 5-mm defocused beam diameter, and pulses of 100 ms, are presented. Follow-up shows that periocular skin reshaping by CO2 laser for laxened eyelid is a safe method and it can be considered a good alternative of treatment for blepharochalasis without skin excision.
