Influence of strain difference on the karyotypic changes in N-nitroso-N-butylurea--induced mouse lymphomas.

Karyotypes were analyzed in 44 cases of mouse lymphomas induced by continuous oral administration of N-nitroso-N-butylurea (NBU): 14 cases of C57BL/6JKok, 6 from LT/sv, 5 from MT/Hok, 2 from (AKR/MsHok x T1Ct)F1, 2 from (AKR/MsHok x CBA/H-T6Hok)F1, and 15 from the first backcross generation of (CBA/Hok x CBA/H-T6Hok)F1 to CBA/Hok. Thirty-six were thymic lymphomas (TLs) and the remaining 8 nonthymic lymphomas (NTLs). No chromosome abnormality was found in 8 NTLs, whereas modal cells were karyotypically changed in 13 TLs. Unlike the situation in NTLs, chromosomally abnormal cells were always present in the remaining 23 TL cases in which modal cells showed no apparent deviation from the constitutional host karyotype. A total of 31 independent karyotypic changes were identified in 26 cytogenetically abnormal major clones from 19 TLs. Although trisomy 15 was the most frequent karyotypic change, being present in 9 TLs, its incidence was substantially low compared with experimentally induced murine TLs so far reported. Furthermore, 6 of the 9 trisomy 15 clones clustered to (CBA x T6)F1 x CBA. Thus, it seems probable that the genetic background of the host animals has a profound influence upon the frequency of trisomy 15 in TLs.

Nurse practitioners in primary care. 3. The southern Ontario randomized trial.

A group of nurses who formerly had performed office functions received a special university-based educational program designed to prepare them to assume much of primary care management as nurse practitioners. The associated family physicians would shift their role to general supervision and attention to difficult clinical problems. To test this new form of practice, two complementary randomized trials have been conducted in south-central Ontario. The particular trial reported here was intended to assess the influence of the educational program on the changing roles of the professional personnel. The nurses of 14 family medical practices, with the physicians' support and commitment to participation, applied for the new training. Seven applicants were randomly selected to receive the training and their corresponding practices became the experimental group, while the remaining nurses and practices were retained as controls. During the subsequent year of investigation important changes occurred in professional roles of the experimental group. Nurse practitioners spent more time in clinical activities than conventional office nurses. The shift was not at the expense of time devoted to clinical work by physicians. Doctors delegated more professional activities to nurse practitioners than to conventional nurses. Except for remuneration (affected by legal constraints) job satisfaction among experimental physicians and nurses remained high after one year of experience with the new method.

A 14-year-old patient with Ewing's sarcoma presenting at autopsy with multiple neurofibrillary tangles and Lewy bodies in addition to hemiatrophy of the central nervous system.

The autopsy findings of a 14-year-old Japanese girl with Ewing's sarcoma, who had multiple neurofibrillary tangles and Lewy bodies and hemiatrophy of the central nervous system (CNS), are reported. She had retinoblastoma of her right eye 8 months after birth, which was treated with chemotherapy and irradiation (40 Gy), twice, seizures 1 year and 2 months after birth, and thereafter severe mental retardation. She showed left hemiparesis after a febrile seizure at the age of 7 years and CT disclosed the right cerebral hemiatrophy. For the last 2 years of life she suffered from Ewing's sarcoma. Extrapyramidal signs were absent. Neuropathologically, tangles consisting of paired helical filaments were distributed symmetrically in virtually all the grey matter. They were particularly numerous in the frontal cortex and substantia nigra, but sparse in the nucleus of Meynert, hippocampus, and brainstem. Several Lewy bodies, which were ultrastructurally identical to those seen in Parkinson's disease, were present in the substantia nigra (more on the left than right) and locus coeruleus. Morphometrically, the number and size of substantia nigral neurons were reduced, the reduction in the latter being more marked than the former, but the melanin pigment contents and shapes of the remaining neurons appeared normal. The right cerebral hemiatrophy with contralateral cerebellar hemiatrophy may have been attributable to irradiation. Although our patient did not have parkinsonism, her features resembled those of a 28-year-old autopsy case reported by Popovich et al. [1987].
