Screening of kidney function by plasma creatinine and single-sample 51Cr-EDTA clearance determination--a comparison.

Nomograms or simple formulas have been suggested for estimating the glomerular filtration rate (GFR) from either plasma creatinine (ClK) or from a single plasma sample drawn after an intravenous injection of 51Cr-EDTA (ClA/B). This study compares the precision of estimating GFR as ClK and ClA/B. A multiple plasma-sample standard method (ClSM) served as reference. The comparison was performed in 136 individuals. The variance of ClA/B on ClSM was 14.4 ml/min/1.73 m2 while the variance of ClK on ClSM was 204.5 ml/min/1.73 m2. It is concluded that the ClA/B values are more precise than the ClK values (p less than 0.01). The single-sample method for determination of 51Cr-EDTA clearance seems well-suited for evaluation and monitoring of patients with known or suspected kidney diseases. It may even be applied for screening purposes in cases where plasma creatinine would otherwise have been chosen instead of the usual resource-demanding method for determination of ClSM.

Determination of 51Cr-EDTA clearance between 15 and 40 ml/min/1.73 m2 by a single plasma sample.

A single-sample method for calculation of 51Cr-EDTA clearance (Cl) between 15 and 40 ml/min/1.73 m2 (ClB) is introduced. It was tested in 109 consecutive patients and compared with a previously published single-sample method for calculation of Cl for Cl greater than or equal to 30 ml/min/1.73 m2 (ClA). When Cl less than 40 ml/min/1.73 m2 the results were more precise when calculated as ClB than when calculated as ClA (p greater than or equal to 0.05). The ClA values were more precise than the ClB values when Cl was greater than or equal to 40 ml/min/1.73 m2 (p less than 0.01). Used in combination (ClA/B) there was a close correlation with Cl (r = 0.98) for the whole range of tested clearance values. It is concluded that Cl can be precisely determined by combined use of the two methods, provided Cl greater than 15 ml/min/1.73 m2 and the single blood-sample is drawn between 240 and 300 min after the injection. If ClA/B less than or equal to 15 ml/min/1.73 m2 it is recommended that a second blood-sample be drawn after 24 h and that Cl is calculated by a conventional slope method.

Fine needle aspiration and Tru-Cut biopsy in the diagnosis of soft tissue metastases in breast cancer.

Soft tissue is a common site of the first recurrence in patients with breast cancer, and soft tissue lesions are often used to assess the efficacy of systemic therapy. It is therefore desirable to obtain histological/cytological verification of such lesions. It is our experience that FNA and TCB in psycho-social respects are superior to a surgical biopsy. Using a surgical biopsy as the key-diagnosis we have compared the diagnostic value of Fine-Needle Aspiration (FNA), Tru-Cut Biopsy (TCB) and clinical evaluation. The FNA was found to be significantly better than TCB in establishing the diagnosis. The diagnostic specificity and sensitivity for FNA was 1.0 and 0.65, while the corresponding figures for TCB and the clinical diagnosis were 1.0 and 0.36 and 0.76 and 0.33, respectively. Since no false positive results were obtained by use of FNA, this procedure seems to be a sufficient diagnostic procedure. However, in case of a negative outcome of the FNA, the diagnosis must be obtained by a surgical biopsy.

51Cr-EDTA clearance determined by one plasma sample in children.

51Cr-EDTA clearance was determined in 50 children from the activity in one plasma sample by means of two methods. The calculation of 51Cr-EDTA clearance by method I (ClI) was based on a transformation of the biexponential time-activity curve into a monoexponential with identical area below the time-activity curve. The calculation of 51Cr-EDTA clearance by method II (ClII) was based on a close relation between 51Cr-EDTA clearance and the reciprocal apparent volume of distribution normalized with respect to body surface area. It was demonstrated that the two methods were closely related. To test the accuracy of the two methods, the results of ClI and ClII were compared to 51Cr-EDTA clearance calculated by means of a standard, multiple plasma sample method (ClSM). The correlations between ClSM and ClI and between ClSM and ClII were almost identical (r = 0.993 and r = 0.992). The accuracy of 51Cr-EDTA clearance calculated as ClI and ClII did not differ significantly from each other.

Lymphatic albumin clearance from psoriatic skin.

In nine patients with untreated psoriasis vulgaris, human serum albumin labelled with 125I or 131I was injected intradermally in symmetrically located involved and uninvolved skin. The activity of the depots was followed by external detection, and the arrival of labelled albumin in plasma was monitored. In involved psoriatic skin the local mean half-time (T1/2) for tracer disappearance was 20.8 +/- 8.2 (S.D.) hr and in clinically normal skin, 29.1 +/- 9.6 (S.D.) hr. The difference was significant (p less than 0.002). Accordingly, the tracer from involved skin reached higher plasma levels than the tracer from uninvolved skin. However, under slight lymphatic stasis the appearance rate of radiolabelled albumin in plasma from both tissues was minimal during 1 to 2 hours after the injection, indicating that a local direct transvascular drainage of plasma albumin from the interstitium of diseased and normal skin was negligible. We conclude that the previously demonstrated increased extravasation of plasma proteins in involved psoriatic skin is compensated by an increased lymphatic drainage of plasma proteins, and not by an increased local transvascular return.

51Cr-EDTA clearance determined by one plasma sample.

51Cr-EDTA clearance was measured in 99 consecutive patients. Based on the individual plasma activity at 180, 200, 220, and 240 min after injection (normalized with respect to injected dose/body surface area), a nomogram and a formula were derived for the relation between clearance and the plasma activity in one sample drawn between 180 and 240 min after injection. The nomogram was tested by the activity in 141 plasma nomogram were compared to the corresponding clearance value calculated by a conventional standard method. The correlation coefficient for this comparison was r = 0.982. It is recommended that the nomogram should only be used if clearance values above 30 ml/min are expected.
