Supraventricular tachyarrhythmia as a cause of sudden cardiac arrest.

INTRODUCTION: Supraventricular tachyarrhythmias (SVTA) are an accepted cause of cardiac arrest in patients with Wolff-Parkinson-White syndrome (WPW) and hypertrophic cardiomyopathy but their participation in other conditions is less well understood. The purpose of the study was to examine the role of SVTA in sudden cardiac arrest (SCA) by comprehensive evaluation of patients successfully resuscitated from SCA. METHODS: A total of 169 survivors of SCA in the absence of acute myocardial infarction underwent systematic evaluation that included echocardiography, Holter monitoring, coronary angiography and electrophysiological study (EPS) with additional testing in selected cases using provocative drug testing with isoproterenol, ajmaline or ergonovine. RESULTS: SVTA was found as the only possible cause or as the cause facilitating SCA in 29 patients: (1) 3 had a WPW syndrome related to accessory pathway with short refractory period; (2) for 12 patients, SVTA was the cause of cardiovascular collapse; heart disease (HD) was present in 11 cases, but disappeared in two of four with dilated cardiomyopathy after the restoration of sinus rhythm; (3) in 14 patients, SVTA degenerated either in a VF or ventricular tachycardia (VT); HD was present in 12 cases, but disappeared in one; two had no HD and recurrent similar arrhythmia was documented by cardiac defibrillator in one of them. SVTA induced coronary ischemia was the main cause of SCA. CONCLUSION: Rapid SVTA was a cause of SCA, either by cardiovascular collapse or by the degeneration in VT or VF. The complication generally occurred in patients with advanced HD or with rapid SVTA-induced cardiomyopathy and rarely in patients without HD. The incidence of SVTA as the only cause or the facilitating cause of SCAs is probably underestimated, because it is difficult to prove.

Management of type C intercondylar fractures of lower end humerus in adults: A clinical study.

Thirty-six patients of type C intercondylar fractures of lower end of humerus who visited JN Medical College, Aligarh between January, 2001 and January, 2003 were included in the study. All patients were treated surgically by open reduction and internal fixation with 4mm cancellous screws, reconstruction plates, one-third tubular plates. Early physiotherapy was started and the results graded using Krishnamoorthy criteria.

A review of the current treatment of renal artery stenosis.

PURPOSE: Atherosclerotic renal artery stenosis (ARAS) is associated with morbidity and mortality consequent to progressive ischemic renal failure and the cardiovascular consequences of hypertension. There is considerable uncertainty concerning the optimal management of patients with this condition. This review considers the aetiological factors and the physiologic consequences of ARAS and compares the results of clinical studies of medical and endovascular therapies on blood pressure control and preservation of renal function. RESULTS: Although, in patients with fibromuscular disease the results of percutaneous transluminal angioplasty (PTA) are clearly superior to medical therapy and surgery, in asymptomatic patients with ARAS the antihypertensive benefits and preservation of renal function of endovascular, surgical and medical therapies appear similar. In selected symptomatic patients interventions may, however, be life-saving. Surgery is generally reserved for arterial occlusions with preserved renal parenchyma and function. CONCLUSIONS: The results of larger, multicentre, randomised, controlled trials are required to clearly clarify the role of interventional therapy in asymptomatic patients.

Von Willebrand disease associated with superficial temporal artery pseudoaneurysm.

We report on two patients with von Willebrand disease (vWD) that presented with superficial temporal artery pseudoaneurysms following minor blunt trauma. We discuss the possible pathophysiological link between vWD and blood vessel abnormalities. The cases highlight the importance of considering the diagnosis of vWD in patients presenting with pseudoaneurysm.

Glycaemic and insulin responses of six traditional Sudanese carbohydrate-rich meals in subjects with Type 2 diabetes mellitus.

AIMS: The objective of this study was to investigate the influence of six Sudanese traditional carbohydrate-rich meals on glucose and insulin responses and to formulate appropriate dietary guidelines based on glycaemic excursions of traditional foods. METHODS: On six occasions with 1-week intervals, 10 Type 2 diabetic subjects consumed six Sudanese traditional carbohydrate-rich meals. The following meals were tested and also analysed for their composition: wheat gorasa (pancakes), sorghum kisra (flat bread) and sorghum acida (porridge), millet kisra and millet acida and maize acida. Blood samples were collected before and after meal ingestion at 0, 30, 60, 120 and 240 min and analysed for plasma glucose and plasma insulin and incremental areas under the curves (AUC) were calculated. RESULTS: A significant variation in AUC for glucose and insulin responses were found between meals, the over all differences in incremental AUCs between the six meals were significant for both plasma glucose (P = 0.0092) and insulin (P = 0.0001). The 2-h glucose values were 10.5 +/- 2.7 for sorghum flatbread, 9.5 +/- 3.1 for sorghum porridge, 10.3 +/- 3.4 for millet flatbread, 10.6 +/- 3.6 for millet porridge, 11.4 +/- 2.7 for maize porridge and 8.7 +/- 2.4 for the wheat pancakes. The comparison between the AUCs of the meals showed that millet acida (porridge) followed by wheat gorasa (pancakes) displayed significantly lower post-prandial glucose and insulin responses, whereas maize acida induced a higher post-prandial glucose and insulin response. CONCLUSIONS: The comparison of glycaemic and insulin responses to six traditional Sudanese meals show differences of clinical importance, and could form a basis for dietary advice to diabetic subjects in Sudan and countries sharing similar food traditions.

Importance of screening household members of acute brucellosis cases in endemic areas.

Isolated reports of brucellosis among family members have been documented. The aim of this study is to determine if active serological screening of the households' members of acute brucellosis cases will detect additional unrecognized cases. From May 2000 to October 2001, patients with acute brucellosis were enrolled and their household members were serologically screened for brucellosis using the Standard Agglutination Test (SAT). Fifty-five index cases with acute brucellosis and 404 household members were enrolled. The majority of index cases (48%) were young adults, and 79% were illiterate. Ownership of animals and ingestion of unpasteurized raw milk were reported by 45 and 75% of the index cases respectively. Of the 55 families screened, 23 (42%) had two family members or more with serological evidence of brucellosis and 32 (58%) had only the index case. Households of > or = 5 members and a history of raw-milk ingestion by family members were risk factors associated with the seropositives (P < 0.05). Of the 404 household members screened, 53 (13%) were seropositive; of these 39 (74%) were symptomatic, and 9 (35%) had brucella bacteraemia. Symptomatic seropositives tended to have bacteraemia and higher brucella antibody titres compared to asymptomatic seropositives (P < or = 0.05). Screening family members of an index case of acute brucellosis will detect additional cases.

Primary splenic flexure volvulus--a case report with review of literature.

Splenic flexure is a rare site for volvulus. Very few cases have been reported in the literature. Cases mostly present with recurrent attacks of obstruction with distension of the abdomen. A case of a young female who had primary splenic plexure volvulus due to absence of ligamental attachment has been presented here. Resection of the volvulus segment with end to end anastomosis is the curative surgical treatment.

Superior mesenteric artery aneurysms: is presence an indication for intervention?

INTRODUCTION: Although rare, superior mesenteric artery (SMA) aneurysms have a definite rupture risk. Past reports have suggested that this risk is low, yet most investigators recommend repair in selected patients. We reviewed our experience with 21 patients to try to determine when intervention was indicated. METHODS: A retrospective review of the medical records of all patients with SMA aneurysms at our institutions from January 1980 through December 1998 was undertaken. Only patients with true aneurysms of the SMA were included. RESULTS: Twenty-one patients with true SMA aneurysms were identified and included 14 males (67%) and seven females (33%). This represents a 6.9% incidence rate of all visceral aneurysms seen at our institutions. Eight patients (38%) had rupture at presentation, including seven of the 14 males (50%). In contrast to previous reports, only one patient (4.7%) had an infectious etiology. Five patients were on beta-blocker therapy, but none were seen with rupture. However, eight of the remaining 16 patients (50%) without beta-blockade had rupture. Thirteen patients (62%) had calcified aneurysms, but all ruptures were seen in noncalcified aneurysms. Operative intervention occurred in 11 of the 21 patients (52%). All eight patients with rupture underwent operation, including six ligations and one successful embolization, and one patient died before completion of repair. The operative mortality rate was 37.5% for ruptured aneurysms. Elective repair included one prosthetic graft, one excision and patch angioplasty, and one embolization, with no mortality. Ten of the 21 patients (48%) with SMA aneurysms were observed, and all were alive and well at a mean of 67 months' follow-up (range, 2 to 148 months). CONCLUSION: SMA aneurysms are rare but appear to have a higher risk of rupture than previously reported. Male patients and patients with noncalcified aneurysms appear to have a greater risk of rupture. beta-Blockade may have some protective effect against aneurysm rupture. Intervention is reasonable in all patients at good operative risk with SMA aneurysms, considering the high rupture rate in our series.

Non-invasive assessment of cardiovascular autonomic activity induced by brief exposure to 50% nitrous oxide in children.

BACKGROUND: The use of the equimolecular mixture of oxygen and nitrous oxide is widely recommended for relief of pain in children undergoing minor procedures. Although the benefits and adverse effects of the clinical use of nitrous oxide seem well known, its effects on the autonomic nervous system have never been studied in children. The aim of this study was to evaluate changes in autonomic cardiovascular activity induced by brief exposure to 50% nitrous oxide in children. This study was based on non-invasive continuous recordings of RR-interval and non-invasive arterial pressure. Vascular and cardiac sympathetic activity and cardiac parasympathetic activity were investigated using spectral analysis of systolic arterial pressure variability (SAPV) and RR-interval variability (RRIV). In addition, the sensitivity of the spontaneous baroreflex (SBR) was assessed using the sequences and the cross-spectral analysis methods. METHODS: Sixteen non-pre-medicated pre-pubertal children undergoing middle-ear surgery, were studied. Data analysis was performed at three points: baseline, when the end-tidal concentration of nitrous oxide was stabilized at 50%, and after withdrawing nitrous oxide. Low (0.04-0.14 Hz) and high frequency (0.2-0.6 Hz) components of the spectral power of RRIV and SAPV, and SBR sensitivity were calculated using these 2-min data epochs. RESULTS: Our results show that brief exposure to 50% nitrous oxide in children results in: (1) absence of effect on mean AP and SAPV; (2) attenuation of the low frequency component of heart rate variability with a shift of the sympathetic-parasympathetic cardiac balance toward a parasympathetic predominance; and (3) absence of alteration of spontaneous baroreflex sensibility. CONCLUSIONS: Unlike the results demonstrated in adults, our findings show very few cardiovascular effects of nitrous oxide in children. Furthermore, whereas in adults nitrous oxide is associated with an excitatory cardiovascular profile, in children this agent seems to be associated with a depressant cardiovascular profile. The rapid return to baseline after discontinuation of administration and the absence of baroreflex changes are positive attributes for the use of nitrous oxide in children.

Bucharioside and buchariol from Salvia bucharica.

A new monoterpene-glycoside (2-exo-beta-D-glucopyranosyl-1,8-cineol) named bucharioside from the methanol-soluble part and a new sesquiterpenoid (4,10-epoxy-6alpha-hydroxyguaiane) named buchariol from the hexane-soluble part of Salvia bucharica were obtained. Their structures were elucidated with the help of NMR spectroscopy including 1D and 2D experiments.

Study of efficacy and tolerance of ketoprofen and diclofenac sodium in the treatment of acute rheumatic and traumatic conditions.

A comparative, multi-centre study, was conducted during June to December, 1996 to evaluate the efficacy and tolerance of Ketoprofen 100 mg Enteric Coated (EC) tablet and 100 mg intra-muscular injection; with that of Diclofenac Sodium 50 mg tablet and 75 mg intra-muscular injection in acute rheumatic and traumatic disorders. Total of 180 patients (90 per drug), were studied, 82 men and 98 women, between the ages of 18 and 75 years. The symptoms and the number of patients were backache 50, arthritis 64, frozen shoulder 32 and sprains 34. Pain was qualitatively assessed by visual analogue scale (VAS), XY pain index, pain at mobilization and the level of pain handicap. For pain (VAS 75-100) the treatment was initiated with an injectable bid, followed by tablets bid or tid. If the pain score on VAS was less than 75, tablets were given in a bid dosage. The duration of treatment was 15 days in each case. The overall complete relief of symptoms occurred in 25% (23/90) patients with Ketoprofen and in 10% (9/90) diclofenac sodium. Moderate to mild relief was found in 75% (67/90) cases with Ketoprofen and 87% (78/90) with diclofenac sodium. No pain relief was seen in 3% (3/90) with diclofenac sodium, as against no failure in pain relief in the ketoprofen group. Tolerance was found as excellent-good for ketoprofen in 72% (65/90) with diclofenac sodium in 50%, moderate to poor for ketoprofen in 28% (35/90) and with diclofenac sodium in 50% (45/90). Our results indicate that ketoprofen compared to diclofenac sodium is efficacious in acute rheumatic and traumatic injuries. Ketoprofen injection, compared to diclofenac sodium was found to be more effective in providing analgesia.

Supportive care of cancer patients in Dubai.

Supportive care of cancer patients starts from the first day of diagnosis. It is a comprehensive approach involving psychosocial, physical and spiritual supports. Maximum efforts are put in to keep the patients as comfortable as possible during diagnosis, during treatment and during the terminal stages of their disease.

Prevalence of dental caries in Omani 6-year-old children.

OBJECTIVE: To determine the prevalence of dental caries in Omani 6-year-old children. DESIGN: Clinical caries cross-sectional survey, conducted by 16 trained and calibrated dentists. SETTING: Omani primary schools in December 1994. SUBJECTS: 3,114 subjects, randomly selected to achieve an overall 6.6 per cent sample of Omani 6-year-old children. OUTCOME MEASURES: Caries diagnosis based solely on clinical examination in accordance with criteria of British Association for the Study of Community Dentistry. RESULTS: Only 484 subjects (15.5 per cent) were caries-free; regional variations ranging from 4.4 per cent to 31 per cent. Overall, the national dmft averaged 4.61; the majority of caries experienced being in the form of untreated decay, with occlusal surfaces of first primary molars being the most commonly involved site. CONCLUSIONS: Compared with results from a survey of Omani 12-year-old children in 1993, a much smaller proportion of this 6-year-old sample were caries free, emphasising the need for continuance of existing, and the development of further, preventive programmes.

Intravenous iron sucrose complex in the treatment of iron deficiency anemia during pregnancy.

OBJECTIVE: To evaluate the safety and efficacy of intravenous iron sucrose complex (ISC) as compared with oral ferrous sulfate in the treatment of iron deficiency anemia during pregnancy. STUDY DESIGN: prospective, open, controlled study in which pregnant women with iron deficiency anemia were sequentially selected from the antenatal clinic and assigned either to ISC (study group) or to ferrous sulfate (control group). METHODS: Each study patient was given the total calculated amount of ICS (Hb deficit (g/l) x body weight (kg) x 0.3) in divided doses (200 mg (elemental iron) in 100 ml normal saline intravenously over 1 h daily) followed by 10 mg/kg to replenish iron stores. Each patient of the control group was given ferrous sulfate 300 mg (60 mg elemental iron) orally three times a day. All patients were monitored for adverse effects, clinical and laboratory response. RESULTS: There were 52 patients and 59 controls. ISC group achieved a significantly higher Hb level (128.5 +/- 6.6 g/l vs. 111.4 +/- 12.4 g/l in the control group P < or = 0.001) in a shorter period (6.9 +/- 1.8 weeks vs. 14.9 +/- 3.1 weeks in the control group, P < or = 0.001). ISC complex group showed no major side effects while 4 (6%) of the control group could not tolerate ferrous sulfate, 18 (30%) complained of disturbing gastrointestinal symptoms and 18 (30%) had poor compliance. CONCLUSION: We conclude that ISC is safe and effective in the treatment of iron deficiency anemia during pregnancy.

The oral health status of Omani 12-year-olds--a national survey.

This study reports the first ever national oral health survey of Omani 12-year-olds. Conducted in October 1993, of the 3,435 children examined, 1,438 (41.9%) were caries-free, although regional variations ranged from 24.8% to 61.9%. Overall, the national DMFT averaged 1.53, the majority of caries experienced being in the form of untreated decay, with occlusal surfaces of first permanent molars being the most commonly involved site. Oral hygiene was poor, only 11% of those examined being scored as plaque-free.
