[Forum: dermopigmentation or medical tattooing. Dermography in cancerology. Apropos of 220 clinical cases].

Based on a series of 220 cases of reconstruction (63 cases) or correction (67 cases) of the nipple-areolar complex and various extramammary procedures (tattoo scar revision, lips, eyebrows, eyelashes) (90 cases), the authors present their 2-year experience of medical dermography in a cancer centre. Dermography allows breast reconstruction to be completed in a large number of patients who had initially refused to complete their reconstruction. This minimally aggressive outpatient technique which reconstitutes the areolo-nipple complex with sufficient quality, is now part of reconstruction protocols and is gradually replacing surgical techniques.

[Conservative and esthetic surgery in cancer of the breast].

After a review of the studies justifying conservative surgery in the treatment of breast cancer, the author describes the possibilities of improving the results of this surgery either by the use of flaps or prostheses or (in the case of large breasts) by performing mammoplasties. The review of a series of 116 cases illustrates the range of possibilities of cosmetic surgery.

[Trend towards conservative surgery for intraductal carcinomas of the breast. (Retrospective study of 165 cases)].

One hundred and sixty-five intraductal, non invasive breast carcinomas in 162 patients were reviewed. The mean follow-up was 75 months. Recurrence rates seem to be influenced by the therapeutic approach: 4.8% following mastectomy (4 cases out of 83); 16% following breast conserving surgery associated with radiotherapy (4 cases of 25); 22.8% following subcutaneous mastectomy (8 cases out of 35); 50% following breast conserving surgery alone (11 cases out of 35). When considering the clinical and radiological sizes of the tumor in the group of tumor excision and breast irradiation, additional information is gained. When the size is less than 25 mm the recurrence rate drops to 6.6% (1 case out of 15). When the lesions are extensive a subcutaneous mastectomy followed by irradiation (with or without a breast implant) might be an interesting alternative to modified radical mastectomy.

[Imaging of non-functional retroperitoneal paragangliomas. Apropos of a case].

The non functional extraperitoneal paragangliomas are rare tumors (less than 50 cases reported). We report a case with ultrasound and CT scan imaging. The tumor is usually large with important necrotic areas leading to a cystic aspect. The malignant tumors more frequent than the benign tumors present with a regional extension and delayed metastasis.

Chest wall reconstruction after full thickness resection: an experience with 22 patients.

The authors present their experience of chest wall reconstruction after full thickness resection in 22 patients. The patients are from a series of 80 patients treated by chest wall resection from 1967 to 1989. Whether performed on breasts still in place, for recurrent disease, or for radiation-induced lesions (which are often associated), the defect created by complete resection of the chest wall layers causes difficulties. Large cutaneous flaps, often including the opposite breast were used at the beginning of the series. Then came omentum associated to Mersilene mesh and myocutaneous flaps. The results, in terms of comfort and local control are acceptable, even though surgery is only palliative for cancer patients. We feel that full thickness chest wall resection is the only effective treatment for some primary and recurrent malignant tumors and for extensive thoracic radionecroses. Such procedures are designed to improve the patient's quality of life even if they do not actually prolong survival. The goals guiding the reconstruction programme are: (1) the restoration of a stable parietal rigidity; and (2) the reconstruction of long lasting superficial layers.

[Primary mammary focus of malignant non-Hodgkin's lymphoma. Apropos of 10 cases].

The authors describe 10 cases of malignant non-Hodgkin's lymphoma in a mammary site in women aged 38 to 82 years. The clinical examination and the sometimes suggest an adenocarcinomatous lesion, thus leading to a useless surgical exeresis. An initial histological study provides the diagnosis. A complete assessment of extension allows an accurate staging and the refinement of the therapeutic schedule. Exclusive radiation therapy seems to be justified in low-malignancy forms remaining strictly confined to the breast. For the high- and medium-malignancy forms, the essential weapon is chemotherapy, which seems to improve the duration and rate of remission.

[Our experience in the use of epidermal cell cultures in the reconstructive surgery].

The plastic surgery team of the Antoine-Lacassagne cancer center (Nice, France) presents the results of 18 grafts (13 patients) of epidermal cells grown in culture. Cell culture techniques and histologic features are analyzed. The possibilities of this method are discussed on the basis of study results and comparison with other publications on the repair of cutaneous wounds other than burns. The favorable rate of successful grafts (50%) and the indications for the technique are discussed. Used as a complementary procedure, epidermal cell grafts permit better quality scarring, comparable to that obtained with thin skin grafts. In addition, disfiguration of donor sites is eliminated.

Gracilis blood supply: anatomical and clinical re-evaluation.

In a review of 20 gracilis flaps in 18 patients, variations in flap viability have been found. In response to these findings, a detailed anatomical study was performed in 15 fresh cadavers. Selective dye injections evaluated the limits of the skin paddle depending on the dominant pedicle. Most of the musculocutaneous perforators were in the upper two-thirds of the gracilis. In one selected application the main blood supply to the skin was recognised to be proximal. Despite the variations in viability, the flap proved to be a versatile and totally adequate tool.

[Autograft procedures in reparative cancer surgery of the loss of substance of the nose. Critical study].

Using several examples, the authors analyse the choice of reconstructive methods available after excision of cutaneous tumours of the nose. Although a variety of procedures exist, one or two specific flap procedures may be preferred in terms of their safety and cosmetic result. Before describing these procedures, the authors briefly review the basis of surgical treatment in the particular case of the nasal region.

[Repair of chest wall defects after full thickness surgical resection].

The authors discuss the development of their approach to the repair of full-thickness chest wall resection based on a series of twenty-two patients operated between 1967 and 1989. The defect resulting from resection of all tissue planes, either for breast in situ, recurrences, radiation lesions or primary tumours, raises difficult problems. Mammary or cutaneous autoplasties, performed during the early years, have become less frequent, essentially because of the development of musculo-cutaneous flaps (pectoralis major, rectus abdominis, latissimus dorsi) which provide global anatomical and functional repair. The greater omentum island flap, used since 1974, still occupies an important place because of its plastic and trophic qualities in irradiated regions. When chest wall rigidity is compromised, Mersilene patch remains the material of choice. The quality of the results obtained in terms of comfort justifies the use of this major surgery which is now well defined.

[Treatment of cancer of the anal canal with combination radiochemotherapy: apropos of 12 cases].

We present 12 patients (3 male, 9 female) aged from 53-88 years with squamous cell cancer of the anal canal treated with combined radiotherapy-chemotherapy (5 FU-mitomycin C); the distribution of the tumours was 2 T4, 6 T3, 3 T2, 1 T1). In 11 patients evaluated, complete remission was obtained in 8 with partial remission in 2 others. In spite of this good result with combined treatment, 4 patients had a recurrence between 9 and 26 months. Five patients survived and 2 died without disease (16-38 month), 3 patients died and one survived with disease. The small series confirms the efficacy of this combination which treated bulky tumours (T3-T4). It is necessary however to compare these results with radiation alone in a randomised trial.

Factors affecting relapse in node-negative breast cancer. A multivariate analysis including the labeling index.

Between 1975 and 1982, 167 patients with carcinoma of the breast without axillary lymph node metastases were studied. The thymidine labeling index (LI), representing the percentage of cells in the DNA synthesis phase, was measured in all these patients. High LI values were more frequently encountered in young patients (P = 0.05), in low estrogen receptor (ER) tumor content (P = 0.007) and in high grade tumors (P = 0.0002). The overall 8-year relapse-free survival (RFS) was 68%. Univariate analysis demonstrated that RFS was influenced by histological grading (P = 0.03), ER (P = 0.03), PR (P = 0.02) and LI (P = 0.01). Multivariate analysis using the Cox regression model selected the LI as the single significant prognostic factor with regard to RFS (P = 0.037). These results emphasize the important role of cell proliferation kinetics in defining node-negative breast cancer patients with a high risk of relapse.

Practical importance of breast microcalcifications: report on 112 cases.

The authors discuss the results of a diagnostic and therapeutic study of 112 cases of breast microcalcifications with no palpable lesions. The criteria in favor of obtaining a surgical specimen are discussed as a function of population characteristics and a review of the literature. Techniques for the localization of these lesions are defined. Decisions as to the necessity for ulterior treatment of the breast are based on certain criteria, and above all on the risk of evolution of invasive lesions with nodal involvement. The high quality of current mammograms has considerably increased the number of patients referred for biopsy. In the case of microcalcifications masking malignant lesions, the advances made in conservative treatment of breast cancer are not always applicable owing to the risks of failure with such conservative approaches.
