A comparison of two formulations of indomethacin ('Flexin Continus' tablets and 'Indocid' capsules) in the treatment of rheumatoid arthritis.

A multi-centre, double-blind, crossover study was carried out in 80 patients with rheumatoid arthritis to compare the efficacy and side-effect profiles of two formulations of indomethacin. Patients were allocated at random to receive 75 mg indomethacin per day either as 1 controlled-release tablet at night or as 1 immediate-release capsule given 3-times a day for a period of 4 weeks before being crossed over to receive the alternative treatment for a further 4 weeks. Pain scores, daily symptomatology and the requirement for escape analgesia recorded by both investigator and patient indicated that controlled-release indomethacin tablets, 75 mg given at night, was as efficacous as immediate-release indomethacin capsules given 3-times daily. However, the controlled-release formulation had a superior side-effect profile with a reduced incidence of abdominal/epigastric pain compared to the immediate-release preparation.

The efficacy and side-effect profiles of two formulations of indomethacin were compared in a multi-centre, double-blind, crossover study in 77 patients with osteoarthritis. Patients were allocated at random to receive 75 mg indomethacin per day either as 1 controlled-release tablet at night or as 1 immediate-release capsule given 3-times daily for a period of 4 weeks, after which patients were crossed over to receive the alternative treatment for a further 4 weeks. Pain scores, daily symptomatology and the requirement for escape analgesia recorded by the investigator and patient indicate that controlled-release indomethacin tablets, 75 mg given at night, were as efficacious as immediate-release indomethacin capsules, 25 mg given 3-times daily, in the treatment of osteoarthritis. The side-effect profiles of the two formulations were similar.

Ewing's sarcoma of maxilla--a case report.

This paper reports the case of an adolescent girl who was admitted in the ENT ward, IPGMR in February '88 with the complaints of nasal obstruction, nasal discharge associated with progressive swelling of the right side of her face for 1 year. She had an operation at Chittagong Medical College Hospital 1 year back for some problem in the right side of the nose. No history of pain, nasal bleeding or visual disturbance were present. The nasal mass was mobile, insensitive and did not bleed on touch, but the facial swelling was red, soft and cystic. Clinically diagnosed as an Antral growth with Ethmoidal involvement. Radiologically revealed soft tissue swelling, obliteration of nasal fossa and opacification of sinuses. After surgical resection the facial mass was diagnosed as Ewing's sarcoma histopathologically. The patient needed radiotherapy but did not follow it.

Percutaneous catheter closure of the ductus arteriosus in children and young adults.

The double-disk occluding device suitable for use in infants and children with patent ductus arteriosus, developed by Rashkind and Mullins, has been available to this institution since December 1987 on an investigational basis. Results of 43 nonsurgical ductal closure attempts using this device in children and young adults between December 1987 and October 1988 are presented. Successful implantation was achieved in 42 patients (98%). In 25 patients a residual shunt was seen on angiography immediately after implantation. In 12 patients a residual ductal shunt was detected by Doppler 24 hours after implantation; 6-week follow-up studies showed a small residual shunt in only 3 patients. After a second occluder device was implanted in 2 of these patients, complete closure of the patient ductus arteriosus was achieved. In 1 patient, early in the series, the occluder device embolized to the left pulmonary artery, necessitating surgical removal at the time of ductus division. This event was the only serious complication; there were no deaths.

Antibodies to rat sperm tail polypeptides recognize Sertoli cell secretory proteins.

We have previously reported that (a) polyclonal antisera raised against rat Sertoli cell secretory protein S70 and S45-S35 heterodimeric protein recognize outer dense fiber polypeptides from rat sperm tail, and (b) protein S70 is antigenically related to polypeptides S45 and S35, the disulfide-linked components of the heterodimeric protein. We now report that polyclonal antisera generated against three different outer dense fiber polypeptides recognize (a) the putative antigen of the sperm tail and (b) Sertoli cell secretory protein S70 and its antigenically-related polypeptides. Immunogold electron microscopy shows that outer dense fibers of epididymal sperm crossreact with anti-S70 serum as well as with an antiserum raised against the polypeptide D complex of extracted outer dense fibers. Electron microscopy demonstrates that outer dense fibers consist of filamentous, coil-coiled units aligned side-by-side with each other. Results of this study strengthen the antigenic homology between Sertoli cell secretory proteins and outer dense fiber polypeptides of the sperm tail.

Human Sertoli-spermatogenic cell cocultures prepared from biopsies of cryptorchid testes performed during orchidopexy.

A procedure is described for the preparation and maintenance of human Sertoli-spermatogenic cell cocultures using biopsies of normal and undescended testis. The evaluation of cell viability and differentiation potential of cultured spermatogenic cell was monitored by [3H]thymidine labeling combined with light microscopic autoradiography. Spermatogenic cells of the same progeny, connected by intercellular bridges, display synchronous DNA synthesis when labeled at the preleptotene stage of meiotic prophase. The pattern of [35S]methionine-labeled secretory proteins was determined by two-dimensional electrophoresis and autoradiography during testicular development and compared with these observed in human Sertoli-spermatogenic cell cocultures prepared from same specimens. Both testicular tissue and cocultured Sertoli and spermatogenic cells displayed comparable patterns of secretory proteins. A discrete group of acidic polypeptides of Sertoli cell origin enhanced their radiolabeling intensity during testicular development. Results of this paper indicate that human Sertoli-spermatogenic cell cocultures could be valuable for assessing the proliferation and differentiation potential of spermatogenic cells in children with cryptorchid testis.

Identification of rat testis galactosyl receptor using antibodies to liver asialoglycoprotein receptor: purification and localization on surfaces of spermatogenic cells and sperm.

We have found that the rat testis contains a cell surface galactosyl receptor that is antigenically related to the minor species of rat liver asialoglycoprotein receptor (ASGP-r) and has binding affinity for galactose coupled to agarose. In immunoblotting experiments, rat testis galactosyl receptor (RTG-r) is recognized by antiserum raised against the minor ASGP-r species of rat liver (designated rat hepatic lectin-2/3, RHL-2/3). Antiserum raised against the major species RHL-1 does not recognize an antigenic protein equivalent to RTG-r. Triton X-100-extracted rat liver and testes preparations fractionated by affinity chromatography on galactose-agarose and resolved by SDS-PAGE under reducing conditions, show that rat liver contains both the major (RHL-1) and minor (RHL-2/3) ASGP-r species whereas rat testis displays only a receptor species comigrating with RHL-2/3. RTG-r was present throughout testicular development. The receptor was found in seminiferous tubules, cultured Sertoli and spermatogenic cells, and epididymal sperm. Indirect immunofluorescent studies show RHL-2/3-like immunoreactivity on the surface of Sertoli cell, meiotic prophase spermatocytes, spermatids, and epididymal sperm. In spermatids and sperm, the immunoreactivity is restricted to the plasma membrane overlying the dorsal portion of the head. Because of RTG-r has galactose binding affinity, is present on surfaces of Sertoli and developing meiotic and postmeiotic spermatogenic cells, and overlies a region of the intact acrosome on epididymal sperm, RTG-r may have a role in spermatogenesis and in events leading to sperm-egg recognition.

Provocation and amplification of the transvalvular pressure gradient in rheumatic tricuspid stenosis.

A low cardiac output and high compliance of the systemic venous system may mask a resting tricuspid diastolic gradient in patients with significant rheumatic tricuspid stenosis. Thirty-three patients (mean age 28 +/- 10 years) with rheumatic tricuspid stenosis evidenced by 2-dimensional echocardiography (doming and restricted motion of all 3 tricuspid valve leaflets) were studied to expose occult and to amplify borderline and basal tricuspid valve gradients. At cardiac catheterization, the right atrium and right ventricular pressures were recorded simultaneously in the basal state, after intravenous infusion of 200, 400, 500, 700 or 1,000 ml of normal saline until a mean right atrial pressure of 12 mm Hg was achieved, and after 0.6 mg of intravenous atropine. Eleven patients (33%) had a mean tricuspid diastolic gradient of greater than 2 mm Hg at rest (group 1). After 483 +/- 240 ml of saline infusion, the mean tricuspid diastolic gradient increased from 5 +/- 2 to 9 +/- 3 mm Hg (p less than 0.001), secondary to a marked rise in right atrial pressure from 8 +/- 3 to 12 +/- 2 mm Hg (p less than 0.001). Concomitantly, there was no increase in right ventricular end-diastolic pressure, although the heart rate increased from 76 +/- 13 to 79 +/- 12 beats/min (p less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

Antigenic homology between rat sperm tail polypeptides and Sertoli cell secretory proteins.

A high performance liquid chromatographic procedure has been used for the purification of rat Sertoli cell secretory protein S70 and S45-S35 heterodimeric protein to determine their role during spermatogenesis. These two proteins display binding affinity for each other and appear antigenically related. We have observed that: 1. S70 and S45-S35 heterodimeric protein coelute during purification, 2. polyclonal antiserum raised against protein S70 recognizes common antigenic determinants in polypeptides S45 and S35, the disulfide-linked components of the heterodimeric protein, and 3. a monoclonal antibody that recognizes polypeptide S35 but does not crossreact with either protein S70 or polypeptide S45, immunoprecipitates the S70/S45-S35 heterodimeric protein complex. In immunofluorescent experiments, antisera raised against protein S70 and polypeptide components of S45-S35 heterodimeric protein immunoreact with two major sperm intracellular structures: the acrosome and periaxonemal outer dense fibers of sperm tail. Immunoreactivity was not detected on the sperm plasma membrane surface of unfixed, living sperm. Outer dense fibers extracted from sperm tails by a combined treatment with cetylthrimethylammonium bromide and 2-mercaptoethanol, yielded a characteristic polypeptide pattern. In immunoblotting experiments sperm tail polypeptides were recognized by polyclonal antisera raised against Sertoli cell secretory proteins. We conclude that Sertoli cell secretory proteins S70 and S45-S35 heterodimeric protein are antigenically related to each other and to keratin-like polypeptides from sperm tail.

Antibodies to rat Sertoli cell secretory proteins recognize antigenic sites in acrosome and tail of developing spermatids and sperm.

Polyclonal antisera raised against polypeptide components of antigenically related rat Sertoli cell secretory protein S70 and S45-S35 heterodimeric protein were used to identify antigenic sites in rat testis during spermatogenesis. Immunofluorescence, immunoperoxidase and immunogold electron-microscopic methods have shown that all antisera recognize immunoreactive sites in the acrosome of developing spermatids. Apical cytoplasmic regions of Sertoli cells in close association with bundles of developing spermatids displayed an immunoreactive product. The principal piece of the developing tail of maturing spermatids immunostained with antisera to Sertoli cell secretory S45-S35 heterodimeric protein, displayed characteristic apical-to-distal immunoreactive gradient patterns before acquiring uniform immunoreactivity at completion of maturation. Immunogold electron microscopy demonstrated that outer dense fibres were the predominant immunoreactive site. Results of this work, together with previous immunoblotting and immunofluorescent data, demonstrate that both Sertoli cell secretory proteins and components of the acrosome and tail of developing spermatids and epididymal sperm share antigenic homology.

Fetuin: a serum component associated with rat Sertoli and spermatogenic cells in coculture.

Cocultures of rat Sertoli-spermatogenic cells plated in a culture medium supplemented with 10% fetal bovine serum for 6-12 h and then maintained in serum free, hormone/growth factor-supplemented medium accumulated an acidic glycoprotein of molecular weight of 68,000 dalton (68 kD) and isoelectric point range of about 4.2-3.5. Anion exchange chromatography has allowed the partial purification of this protein, which consists of a major protein band of 68 kD and two minor, low molecular weight components. A rabbit antiserum raised against the 68 kD component also crossreacts with the two low molecular weight components, thus suggesting that these two minor components are antigenically related to the 68 kD protein. The 68 kD protein has been identified as fetuin, the major component of fetal bovine serum, based on similar molecular weight, isoelectric point, immunoreactivity and trypsin inhibitory activity. Labeling experiments with [14C]amino acid mixture show that 68 kD protein is not synthesized by cocultured rat Sertoli and spermatogenic cells. Immunocytochemistry and Western blot approaches carried out under various experimental conditions support the view that the fetuin-68 kD protein is taken up from serum by both Sertoli cells and pachytene spermatocytes. Because fetuin 1) behaves as a carrier protein for growth factors, 2) has protease inhibitory activity, 3) is preferentially internalized by Sertoli cells and pachytene spermatocytes and 4) fetal bovine serum-supplemented medium impairs spermatogenic cell viability, there is a need to further define appropriate conditions for optimizing long-term viability and differentiation of spermatogenic cells in vitro.

The application of DNA-cellulose chromatography in the isolation of immunoglobulin M and complement component C4b-binding protein from human serum.

By utilising the ability of some anionic serum proteins to bind to DNA-cellulose, at physiological pH and ionic strength, two such proteins have been purified and identified as immunoglobulin M (IgM) and complement component C4b-binding protein (C4BP). The method entails ammonium sulphate precipitation followed by QAE-Sephadex, DNA-cellulose and Sephacryl S-300 chromatographic steps. Fractionation of the anionic proteins eluting from QAE-Sephadex over a selected range of ionic strength has afforded IgM which was characterised electrophoretically and immunochemically. A mixture containing IgM complexed with C4BP has also been obtained by utilizing fractions eluted from QAE-Sephadex at higher ionic strength. C4BP can be isolated from this mixture by using immunoaffinity chromatography to remove IgM.

Seasonal variations, and the intra-household distribution of food in a Bangladeshi village.

Individual food intakes and body weights were measured in 53 rural Bangladeshi households at four seasons. Energy and protein intakes (kcal/kg/d) showed significant seasonal differences for adults and young children (1-4 yr). Energy intakes of women and children were expressed as a proportion of the male household heads', to give an indicator of food allocation. Women's and older children's proportional energy intakes remained constant through the year, and in line with expected values. Young children's proportional intakes were low, girls' being lower than boys'. The only seasonal variation was an increase in young girls' proportional energy intake when household food supplies were lowest. It is concluded that women's proportional intakes are not depressed when family food intake decreases. There is evidence that young girls receive particularly low intakes.
