The natural history of lung cancer estimated from the results of a randomized trial of screening.

The results from a randomized controlled trial of screening for lung cancer in Czechoslovakia have been used to estimate parameters of the natural history, using a model to simulate the disease process and the effects of screening. The results suggest that the period before clinical presentation during which lesions can be detected by screening is very short (seven to eight months). This implies that to detect three-quarters of all lung cancers by screening, two examinations per year are necessary, and that such a program would advance diagnosis by six months if there were complete participation. The results of the trial itself suggest that the benefit, in terms of a reduction in mortality from lung cancer, is likely to be very small.

[Evaluation of epidemiologic studies using the EPI INFO program].

The authors describe the EPI INFO system suitable for introducing on a personal computer, compatible with the standard IBM XT/AT. The system was elaborated by a group of workers from Centres of Disease Control in Alabama in 1988. The advantage of the system is in addition to ready availability (it is distributed on a non-commercial basis), in particular because it combines the advantages of the data base and statistical programme system. It makes it possible storage of data, their easy processing by classification and tabulation, evaluation by some statistical (parametric and non-parametric) tests and simple graphical presentation of results, e.g. in histograms or figures using columns. Moreover, it is possible to make in the EPI INFO system further modifications of the stored data and to combine groups of similar or different format. The system does not only create its own external data groups but makes also possible their conversion into other systems and conversely is able to create from external groups of data its own groups of data.

Epidemiologic Methods

Lack of benefit from semi-annual screening for cancer of the lung: follow-up report of a randomized controlled trial on a population of high-risk males in Czechoslovakia.

Cigarette-smoking males (6,364), aged 40-64, were randomized into an intervention group which received 6-monthly screening by chest X-ray and sputum cytology, and a control group which received no asymptomatic investigation. After 3 years, both groups entered a follow-up period during which they received annual chest X-rays. Lung cancer cases detected by screening were identified at an earlier stage, more often resectable, and had a significantly better survival than "interval" cases diagnosed mainly because of symptoms. Comparison of the 2 groups showed a higher incidence of lung cancer in the intervention group, despite the follow-up period when both groups received annual examinations. There was no significant difference in mortality between the 2 groups.

[Supravalvular aortic stenosis].

The authors describe the case of an inborn supravalvular aortal stenosis in an adult patient who was successfully treated by a plastic operation of the stenotic portion of the ascendent aorta by means of a synthetic patch. At the same time also a plastic operation of an incompetent mitral valve was made.

[The occurrence of circadian rhythm in respiration in healthy persons and in patients with bronchial asthma].

The authors examined in 69 healthy young men at times 0, 3, 6, 9, 12, 15, 18 and 21 hours values of FVC, FEV1, FEF50 a PEF. In 149 patients with bronchial asthma after the same time intervals FEV1 was assessed, whereby the interval at 3 a.m. was used as the mean of values at times 0 and 6 a.m. By analysis of variance the minima of the majority of investigated indicators were assessed in healthy subjects at 3 a.m. and 9 a.m. By cosinor analysis a statistically insignificant circadian rhythm for values FEC and FEV1 was proved in the entire group of healthy subjects and in subgroups of smokers and non-smokers. No rhythm was found in values of FEF50 and PEF. Rhythms in the ventilation values were detected in 40-60% of the subjects, in the remainder no rhythms were detected. In patients with bronchial asthma a significant circadian rhythm of FEV1 changes was found in groups with a morning or variable type of respiration. In groups with a stabilized ventilation a rhythm was detected but was not significant. In the group with an apparently irreversible obstruction no circadian rhythm was found.

[Inflammatory aneurysm of the pelvic artery].

The authors describe an uncommon case of an inflammatory aneurysm of the common abdominal aorta which was successfully treated by resection and prosthesis. In the discussion the authors deal with the specific features of this type of aneurysm, as compared with common sclerotic ones. They recommend the best diagnostic procedure, surgical approach and postoperative treatment.
