Habituation of auditory event-related potentials in patients with Behcet's disease.

Behcet's disease is a progressive and/or relapsing-remitting multisystem inflammatory disorder. Involvement of the central nervous system occurs in 10%-29% of cases with Behcet's disease. The aim of the present study was to assess the effect of repetitive auditory stimulations on event-related potentials (ERP) in patients with Behcet's without neurologic manifestations. The study was performed in 14 patients with Behcet's disease and 14 healthy volunteers. ERPs were recorded from Fz, Cz and Pz. The auditory ERPs test was continued until 20 artefact-free rare tones were averaged, which was accepted as one trial block. After six blocks were obtained in a sequential manner, the test was completed. According to the results of the present study, P300 latency changed in different manners over trial blocks in the two groups. P300 latency values showed a progressive increase in the controls; however this was not determined in the Behcet's group. Thus, habituation was not observed in the patient group. Because of the lack of habituation related to P300 latency, it may be thought that there is an insidious pathologic process in Behcet's disease without neurologic manifestation.

Granuloma annulare associated with prostate carcinoma.

Granuloma annulare occurs as an idiopathic and benign condition, although it may be associated with some neoplasms. We describe a 60-year-old man with granuloma annulare on dorsal hands who developed prostate carcinoma. To the best of our knowledge, this is the first case report of granuloma annulare with prostate carcinoma. Granuloma annulare, especially in older age groups, may be classified as a sign of various carcinomas.

Electrodermal activities and autonomic nervous system in Behçet's patients.

Behçet's disease is often a progressive disorder. In some cases, there is a possibility of subclinical involvement without neurologic signs and symptoms. The autonomic nervous system is affected in Behçt's disease. The aim of the present study was to investigate autonomic nervous system functions in Behçt's patients by analyzing electrodermal activities. A total of 16 patients and 16 healthy volunteers were accepted for the study. Skin potential recordings were taken at room temperature in a quiet place within a Faraday's cage. The mean values of basal skin potentials and skin potential responses in the patients' group were reduced when compared with those of the control group ( p=0.0001). There was no difference between the groups regarding the mean values of latency ( p=0.09). The findings suggest that electrodermal activities may reflect autonomic nervous system dysfunction in Behçet's patients, and the measurement of electrodermal activities may be useful for the assessment of autonomic nervous system involvement in Behçet's disease.

P300 in Behcet's patients without neurological manifestations.

OBJECTIVE: Behcet's disease is the association of recurrent aphthous stomatitis with genital ulceration and eye disease. Neurologic involvement patterns include meningomyelitis, a brain stem syndrome, pyramidal and extrapyramidal abnormality and stroke. In the present study, subclinical involvement was investigated by using P300 in Behcet's patients without neurological manifestation. METHODS: Fifteen patients and 15 healthy volunteers were accepted for the study. P300 from vertex (Cz) electrode sites of the 10-20 system using electrodes and motor response time were recorded. RESULTS: Patients had significantly prolonged latencies of P300 as compared to normal controls (p=0.013) but no significant differences in amplitude (p=1.000). Patients showed a significantly delayed motor response time than controls (p=0.006). Nine patients (60%) had P300 latency and eight patients (53.3%) had motor response time values exceeding the mean of controls by two standard deviations. CONCLUSION: The findings suggest that the P300 measures and motor response time may reflect subclinical neurologic involvement in Behcet's disease.

Pedunculated lipofibroma.

Pedunculated lipofibroma is a relatively rare form of nevus lipomatosus superficialis. The lesions are large, slow growing, pedunculated tumors. Histologically, groups and strands of fat cells are found embedded among the collagen bundles of the dermis. We presented a woman diagnosed with a pedunculated lipofibroma clinically and histopathologically.

Kangal hot spring with fish and psoriasis treatment.

The effect of "Kangal Hot Spring with Fish" in the treatment of psoriasis is investigated. The study was carried out on 87 patients with psoriasis vulgaris and the patients were evaluated by a dermatologist for 21 days. The evaluation of the disease was performed using PASI (Psoriasis Area and Severity Index) scores. Recurrences were investigated in the patient population, who had been previously treated in the same hot spring. The first examination scores were significantly higher than the scores of the 3, 6, 9, 12, 15 and 21 days after treatment (p < 0.01). Longer remission periods, when compared to the topical corticosteroid treatment, expressed by the 35 patients previously treated in hot spring (p < 0.01). Spa therapy under observation of a dermatologist may be effective and useful for psoriasis vulgaris patients.

A case of acrodermatitis enteropathica.

Acrodermatitis enteropathica is a rare hereditary disorder affecting zinc metabolism that is characterized by dermatitis, alopecia, gastrointestinal disturbances, eye infections, and growth failure. We report a 17-month-old girl with acrodermatitis enteropathica. Physical examination showed a cutaneous eruption consisting of vesiculobullous and psoriasiform skin lesions symmetrically distributed in the perioral, acral, and perineal areas. Her plasma zinc level was decreased (75 micrograms/dl), but within the normal range (60.00-135.00 micrograms/dl). The patient was given zinc sulfate 50 mg/day. At the end of two months, she had significantly improved.

Percutaneous fibrin sleeve stripping of failing haemodialysis catheters.

BACKGROUND: One of the most frequent reasons for failure of haemodialysis lines is catheter blockage caused by fibrin sheath formation. We report our experience of percutaneous fibrin sheath stripping in treating this problem. METHODS: A consecutive series of failing haemodialysis catheters underwent percutaneous fibrin sheath stripping in an attempt to retrieve and prolong the life of the catheter. Immediate technical success, clinical success, and primary and secondary patency were measured based on clinical follow-up. RESULTS: Sixteen non-functional permanent haemodialysis lines in 15 patients underwent percutaneous fibrin sheath stripping on 21 occasions. Technical success rate was 100%. Catheter flow rates sufficient for initial dialysis were achieved in 12 (75%) lines. Successful percutaneous fibrin sheath stripping produced a mean catheter patency of 126 days (range 6-299 days). CONCLUSIONS: Percutaneous fibrin sheath stripping is a simple, repeatable procedure that can usefully extend the life of a failing dialysis line. However, a randomized trial will be needed to evaluate its role compared with catheter replacement.

Sneddon's syndrome and antithrombin III.

Sneddon's syndrome, an uncommon disorder, is characterized by multiple cerebrovascular accidents along with ideopathic livedo reticularis. In this article, two cases who had increased amounts of antithrombin III and were diagnosed with Sneddon's syndrome are presented.

Elephantiasis neuromatosa and Becker's melanosis.

The most characteristic lesions of neurofibromatosis are the extremely large plexiform neurofibromas involving an entire extremity, which give rise to the condition known as elephantiasis neuromatosa. In this article, we present a patient who was diagnosed as elephantiasis neuromatosa with Becker's melanosis clinically and review the literature briefly.

Neuroendocrine cell hyperplasia in colonic tissue used for long term augmentation cystoplasty.

A case is described of neuroendocrine cell hyperplasia in intravesical colonic mucosa, implanted previously during augmentation cystoplasty. The patient was a 28 year old man born with posterior urethral valves, a non-functioning right kidney, and a poorly functioning dilated left kidney. The hyperplasia consisted of pure neuroendocrine acini and tubules within the lamina propria, separate from the normal intestinal glands. Adjacent intraepithelial colonic neuroendocrine cells were increased diffusely. Rectal biopsy and previous biopsies of intravesical colonic tissue contained normal neuroendocrine cell populations. Implantation of gut segments into the urinary tract predisposes to late neoplasia, but there is only one report of carcinoid tumour in uroenteric tissue. Intestinal neuroendocrine cell hyperplasia usually occurs diffusely rather than as aggregates, except when associated with adjacent carcinoid tumour. Both diffuse and nodular hyperplasia were present in this case, with an unusual and striking morphology. This is the first report of neuroendocrine cell hyperplasia in gastrointestinal tissue implanted into the urinary tract; this raises the possibility of a risk of late carcinoid tumour in uroenteric segments.

Effects of continuous ambulatory peritoneal dialysis and kidney transplantation on advanced glycation endproducts in the skin and peritoneum.

Advanced glycation endproducts (AGE) form as a result of non-enzymatic reaction of reducing sugars with proteins. Patients with chronic renal failure (CRF) have elevated AGE in plasma, skin and peritoneum. We measured AGE in the skin and peritoneum of individuals with CRF, patients undergoing continuous ambulatory peritoneal dialysis (CAPD) and in renal transplant recipients (TR). Pentosidine concentration and collagen-linked fluorescence (CLF) were measured. Pentosidine and CLF correlated in all patient groups (CRF r=0.688, p<0.01; CAPD r=0.674, p<0.05; TR r=0.811, p<0.01). Successful kidney transplant reduced AGE levels in the skin (CRF 11.7 +/- 4.51 U/mg; TR 5.02 +/- 3.13 U/mg, p<0.00001) and peritoneum (CRF 17.5 +/- 6.16 U/mg, TR 9.4 +/- 4.97 U/mg, p<0.0001). However in contrast to the TR group, CLF in peritoneum increased following CAPD (CRF 17.5 +/- 6.16 U/mg; CAPD 24.2 +/- 10.4 U/mg; p=0.06). Our results suggest that AGE might be formed in the peritoneum during CAPD treatment.

Kidney transplantation decreases the tissue level of advanced glycosylation end-products.

We have studied the effect of chronic renal failure (CRF) and kidney transplantation on advanced glycosylation end-products (AGE) measured as collagen-linked fluorescence (CLF) in the skin and peritoneum of non-diabetic patients. Of 34 patients with CRF, 18 were studied before the commencement of peritoneal dialysis (CRF group), and 16 were studied 5-31 weeks after kidney transplantation (transplant group). The control group consisted of 24 patients with normal renal function. Skin CLF in the CRF group (20.9 +/- 2.02 U/mg) was higher than in the control (8.52 +/- 1.08 U/mg, P = 0.0001) and transplant groups (10.7 +/- 2.43 U/mg, P = 0.003). Peritoneal CLF in the CRF group (30.5 +/- 5.64 U/mg) was higher than in the control group (16.1 +/- 2.25 U/mg, P = 0.031) but was not different from the transplant group (19.4 +/- 3.66 U/mg, P = 0.11). Peritoneal CLF of control and transplant groups were not different (P = 0.45). The results of this study suggest that restoration of renal function affects tissue AGE levels.

Acute torsion of the gallbladder.

Acute torsion of the gallbladder is an acute surgical emergency which may be encountered more frequently in the presence of an ageing population. We report a 98-year-old patient in whom the diagnosis was made at exploratory laparotomy performed for unexplained peritonism. Emergency cholecystectomy was followed by an uncomplicated post-operative recovery.
