Effect of aqueous extract of Cyamopsis tetragonoloba Linn. beans on blood glucose level in normal and alloxan-induced diabetic rats.

Effect of feeding orally the aqueous extract of beans of Cyamopsis tetragonoloba was investigated on fasting blood glucose levels in glucose loaded, normal and alloxan-induced diabetic rats and compared with gliclazide, a reference drug. The aqueous extract of beans at 250 mg/kg body wt significantly lowered blood glucose levels in alloxan-induced diabetic rats within 3 hr of administration. Continued administration of the extract at the same dose daily for 10 days produced statistically significant reduction in the blood glucose levels while marginal activity was seen in normal and glucose-loaded rats.

Homologous transplantation of the mitral valve: a review.

Numerous experimental studies were conducted on homologous transplantation of the mitral valve either for mitral or tricuspid valve replacement in the early '60s. The first mitral homograft in humans was performed in 1965 by Senning. Since that time, there has been a limited number of implants mainly because of technical difficulties related to the insertion of the papillary muscles. Based on principles established for mitral valve repair, a reproducible method of homograft replacement of the mitral valve was described. Following validation in animals, the technique was applied in a series of 104 patients undergoing partial or complete mitral valve replacement. Significant improvements concerned: selection of indications and contraindications, staged approach according to the extent of the lesions leading either to partial or total replacement, systematic use of prosthetic ring annuloplasty, understanding of papillary muscle anatomy allowing a rationale for a reliable attachment method. In hospital mortality was 3.8%. At 8 years, the incidences of patients free from cardiac death and from all death were 90.6% and 82% respectively. Freedom from any cardiac event (death or reoperation) was 72% at 7 years. Similarly to the aortic homograft, mitral homograft durability was decreased in younger patients. Partial homograft replacementoffered satisfactory results particularly in the case of endocarditis and enhanced the possibilities of valve repair. The limitations of the technique are: the technical difficulty which does not permit a completely standardized operation, the risk of early valve dysfunction related to valve mismatch and the risk of late deterioration mainly leading to stenosis. Homologous transplantation of the mitral valve was also applied for tricuspid valve replacement in the case of infective endocarditis or for replacement of a degenerated bioprosthesis. Satisfactory results have been reported. However due to the lack of anatomical landmarks, the implantation procedure has remained technically challenging. Thus until further progress demonstrates a clear superiority of the mitral homograft, bioprosthesis remains the gold standard for replacing the mitral or the tricuspid valve with a biological substitute.

Argyroside from Argyreia nervosa seeds.

The phytochemical investigation of the seeds of Argyreia nervosa has resulted in the isolation of a new steroidal glycoside, (24R)-ergost-5-en-11-oxo-3beta-ol-alpha-D-glucopyranoside, designated as argyroside. The structure has been elucidated by spectroscopic and chemical methods.

Mechanism of garlic (Allium sativum) induced reduction of hypertension in 2K-1C rats: a possible mediation of Na/H exchanger isoform-1.

Garlic causes reduction in blood pressure (BP), however the role of Na/H exchanger (NHE) which mediates hypertension and related tissue-damage is poorly understood. In this study the effect of an established dose of raw garlic extract was investigated on the expression of NHE-1 and -3 and sodium pump activity in a 2K-1C model of hypertension in rats. 2K-1C animals showed high BP, increased serum concentration of PGE2 and TxB2, hypertrophy of the unclipped kidneys, but not in the clipped kidneys In addition, NHE-1 and NHE-3 isoforms were increased in both the 2K-1C kidneys, whereas alpha-actin was increased in the clipped but not in unclipped kidneys. Sodium pump activity was decreased in the clipped kidneys, but remained unchanged in the unclipped kidneys. Garlic treatment reduced the induction of NHE-1 only in the unclipped 2K-1C kidneys, whereas garlic treatment increased the sodium pump activity in both the 2K-1C kidneys. These findings demonstrate that the antihypertensive action of garlic is associated with a reversal of NHE-1 induction in the unclipped kidneys. Induction of NHE isoforms together with a reduced sodium pump activity might cause necrosis in the 2K-1C clipped kidneys due to cellular retention of Na+. On the other hand, activation of sodium pump by garlic extract in the kidneys should reduce intracellular Na+ concentration and normalize BP. These findings signify the use of garlic in the treatment of hypertension.

[Percutaneous dilatative tracheostomy versus conventional surgical tracheostomy: a retrospective trial].

BACKGROUND: In this retrospective study we compared endoscopically controlled percutaneous dilatative tracheostomies (PDT) with conventional surgical tracheostomies as a bedside procedure and in the operating theatre. PATIENTS: Between 1998 and 2000 we performed 360 tracheostomies electively, 152 in PDT-technique (42 %) and 208 (58 %) with the conventional procedure. Referring to the PDT-technique 74 % (n = 112) were performed at the bedside and 26 % in the operating theatre. The conventional tracheostomies took place at bedside in 53 % (n = 110) and in the operating theatre in 47 % (n = 98) of the cases. The complications were divided in 5 groups with special interest if the operation took place in the operating theatre or as a bedside procedure. RESULTS: In general the rate of complications in the PDT group was 33 % (50/152) versus 22 % (46/208) referring to the conventional group. Referring to the PDT group the rate of complications were 35 % (39/112) at the bed site procedure and 28 % (11/40) in the operating theatre. The complication rate for the conventional group was 27 % (30/110) as a bedside procedure versus 16 % (16/98) in the operating theatre. Significant differences were found for the PDT with an increase of tubal obstructions (p = 0.007). For the conventional tracheostomy we found a significant increase of wound infections (p = 0.006). There was significantly higher postoperative hemorrhage if the procedure was done at bed site. CONCLUSION: PDT and conventional tracheostomies have different complications. The higher risk of postoperative hemorrhage for bed site procedure has to be considered.

Yield of dual endoscopy for positive fecal occult blood test.

OBJECTIVES: Dual endoscopy is frequently performed on the same day in patients whose stools are found to be positive on fecal occult blood testing (FOBT). This is often done to localize the potential sources of GI bleed. The diagnostic yield of same day dual upper endoscopy (EGD) and lower endoscopy (colonoscopy) for the detection of positive FOBT is uncertain. In the era of cost-efficient medical practice, we investigated whether a more evidence-based and structured approach could be used to guide physicians to the workup of patients who present with positive FOBT. METHODS: We performed a retrospective analysis of 309 patients, and 260 patients from this population met our inclusion criteria. Inclusion criteria included FOBT without acute GI hemorrhage, hematochezia, or melena. Patients were required to have had EGD and colonoscopy within the same day (<24 h). RESULTS: Of 260 patients, a total of 135 (52%) patients had positive findings on colonoscopy and a total of 42 (16.1%) patients had positive findings on EGD. Sixteen (6.1%) had positive EGD and negative colonoscopy; 109 (42%) had positive colonoscopy and negative EGD; and 26 (10%) had positive findings on both EGD and colonoscopy. CONCLUSIONS: Colonoscopy should be chosen as the initial procedure of choice in the evaluation of patients who present with positive FOBT. Same day dual endoscopy does not seem to be cost-effective.

Overexpression of transcripts containing LINE-1 in the synovia of patients with rheumatoid arthritis.

OBJECTIVE: To identify novel diagnostic markers by comparing gene expression in rheumatoid (RA) and reactive arthritis (ReA) synovium. METHODS: Synovial biopsy specimens were obtained by needle arthroscopy from the knees of 10 patients with either RA or ReA. RNA was isolated from the biopsy specimens and cDNA synthesised for analysis using a customised cDNA macroarray. Confirmatory analysis was performed using in situ hybridisation on a second set of synovial samples. RESULTS: Two unique transcripts (ReXS1 and fibronectin) were consistently more abundant in ReA and three homologous transcripts were more abundant in RA. The latter all mapped within long interspersed nucleotide elements (LINE-1), that form one of the families of repetitive sequences in the human genome. CONCLUSIONS: The abundance of transcripts containing LINE-1 in the RA synovium may be an epiphenomenon or may have pathogenic significance. Further work is required to determine the identity of the full length transcript(s) before its use as a diagnostic marker in RA can be assessed.

Cree encephalitis is allelic with Aicardi-Goutiéres syndrome: implications for the pathogenesis of disorders of interferon alpha metabolism.

Aicardi-Goutiéres syndrome (AGS) is an early onset, progressive encephalopathy characterised by calcification of the basal ganglia, white matter abnormalities, and a chronic cerebrospinal fluid (CSF) lymphocytosis. Cree encephalitis shows phenotypic overlap with AGS although the conditions have been considered distinct because of immunological abnormalities observed in Cree encephalitis. We report that levels of interferon alpha (IFN-alpha), a marker of AGS, are raised in Cree encephalitis. Moreover, linkage analysis indicates that the disorders are allelic and refines the AGS1 locus to a 3.47 cM critical interval. Our data show that a CSF lymphocytosis is not necessary for the diagnosis of AGS and strongly suggest that AGS and pseudo-TORCH syndrome are the same disorder. Recognition of immunological dysfunction as part of the AGS phenotype provides further evidence of a primary pathogenic role for abnormal IFN-alpha production in AGS.

Aqueous extract of walnut (Juglans regia L.) protects mice against cyclophosphamide-induced biochemical toxicity.

Walnut (Juglans regia L.) is extensively used in traditional systems of medicine for treatment of various ailments. It is described as an anticancer, tonic, blood purifier and detoxifier agent. The present study was undertaken to investigate modulatory effects of walnut extract on the toxicity of an anticancer drug, cyclophosphamide (CP) with special reference to protection against disruption of drug metabolizing and antioxidant enzymes. Plant extract+CP group animals showed restoration in the level of cytochrome P450 (CYP) content and in the activities of glutathione S-transferase (GST), glutathione peroxidase (GP) and catalase (CAT) in both liver and kidneys. But plant extract restored the activity of superoxide dismutase (SOD) and the level of reduced glutathione (GSH) in the kidneys only when compared with CP-treated animals. Plant extract treatment alone caused significant reduction in the content of CYP in the kidneys mainly. The extract showed a significant increase in the level of GSH and in the activities of GP in both the tissues and CAT in liver only, whereas no significant change was observed in the activities of GST and SOD. CP treatment resulted in a significant (P < 0.01) increase in the lipid peroxidation (LPO) in the liver and kidneys compared with controls, while the extract+CP treated group showed a significant decrease in the LPO in liver and kidneys when compared with the CP-treated group. The study shows that the use of J. regia extract might be helpful in abrogation of CP toxicity during the chemotherapy.

Two new anthraquinones from the roots of Rubia cordifolia Linn.

Two new anthraquinones, named cordifoliol and cordifodiol, have been isolated from the roots of Rubia cordifolia. Their structures have been established as 1-hydroxy-3-ethyl-9,10-anthraquinone (1) and 1,8-dihydroxy-11, 20 (15-pentyl-naphthaquinonyl) phenanthrene (2) on the basis of spectral data analyses and chemical reactions.

New oleanene and fernane-type triterpenes from the stem bark of Betula pendula roth.

A new oleanene-type triterpene, named betuloleanolic acid acetate and fernane-type triterpenes, namely betufernanediol A and betufernanediol B (isomers) have been isolated from the stem bark of Betula pendula. Their structures have been established as olean-12-en-3beta-ylacetate-29-oic acid, fern-22(29)-en-3beta,12beta-diol and fem-22(29)-en-3alpha,12beta-diol, respectively, on the basis of spectral data analysis and chemical reactions.

A novel naphthanol glycoside from Terminalia arjuna with antioxidant and nitric oxide inhibitory activities.

A novel naphthanol glycoside, arjunaphthanoloside (1), was isolated from the stem bark of Terminalia arjuna and its structure was established as 2,3,6,7,8,9-hexahydroxynaphthalene-2-O-alpha-L(-)-rhamnoside by means of spectroscopic and chemical methods. Compound 1 showed potent antioxidant activity and inhibited nitric oxide (NO) production in lipopolysaccharide (LPS)-stimulated rat peritoneal macrophages.

Exchange bias in spin-engineered double superlattices.

Exchange bias has been observed in sputtered magnetic double superlattices which consist of a ferromagnetically coupled superlattice grown on an antiferromagnetically (AF) coupled superlattice. This system exhibits a parallel domain wall, a spin flop transition, and exchange bias when the anisotropy is large in the AF block. This work shows that neither the domain wall nor the spin flop are directly related to exchange bias but that the anisotropy is essential.

Hypertonic saline nasal provocation and acoustic rhinometry.

BACKGROUND: Hypertonic saline (HTS) acts as an airway irritant in human nasal mucosa by stimulating nociceptive nerves and glandular secretion. HTS does not change vascular permeability. In asthma, HTS causes airflow obstruction. OBJECTIVE: To determine the effect of HTS on mucosal swelling using acoustic rhinometry (AcRh). Potential vasodilator effects were controlled by maximally constricting mucosal vessels with oxymetazoline (Oxy). METHOD: Normal subjects had AcRh before and 30 min after either 0.05% Oxy or saline (0.9% NaCl) nasal treatments. Nasal provocations followed immediately with five step-wise incremental escalating doses of HTS administered at 6-min intervals. AcRh was performed 1, 3 and 5 min after each HTS administration, and then after blowing the nose at 5 min. The minimum cross-sectional area (Amin), volume of the anterior 6 cm of nasal cavity (V6) and incremental changes from pre-drug treatment baseline levels (delta, mean +/- SEM) were calculated. RESULTS: Oxy increased Amin by 46% (delta = 0.48 +/- 0.07 cm2, P = 0.0001) and V6 by 53% (Delta = 9.9 +/- 1.5 mL, P < 1 x 10-7) during the first 30 min. Saline (vehicle) treatment had no effect. The maximum HTS dose had no effect after 1 or 3 min. However, in the 4th and 5th minutes there were reductions in Amin (delta = 0.07 +/- 0.03 cm2, P = 0.035) and V6 (delta = 1.57 +/- 0.42 mL, P = 0.004) with an increase in the weight of secretions (delta = 700 +/- 100 mg, P < 0.05). Blowing the nose returned Amin and V6 towards baseline. Oxy had no effect on HTS-induced changes in Amin, V6, pain, rhinorrhea or weight of secretions. CONCLUSION: HTS induced nociceptive nerve stimulation and mucus secretion, and reduced V6 and Amin. Oxy caused vasoconstriction but did not alter HTS-induced effects. HTS may stimulate neurogenic axon response-mediated glandular secretion that contributes to perceptions of nasal obstruction in normal subjects.
