[Stress analysis of bridge abutment teeth with cemented dowels].

Stress analysis of bridge abutment teeth having cemented dowels was conducted using a two-dimensional finite element method. The results obtained are as follows: 1. In fixed-fixed bridges, compressive stress was found concentrated within the dowel, on the mesial side of the dowel apex, and in the distocervical part of the crown. 2. In cantilever bridges, tensile stress was found concentrated in the disto-occlusal and mesiocervical parts of the crown, and along the mesial sides of the root and the dowel. Compressive stress was concentrated in the distocervical part of the crown, along the distal sides of the root and dowel, and on the mesial side of the dowel apex. 3. In comparison with fixed-fixed bridges, stresses were significantly greater in cantilever bridges in the abutment teeth, the dowel, and crown margins. 4. In both fixed and cantilever bridges, stresses increased in relation to the length of the span. The rate of increase was less, however, in fixed-fixed than cantilever bridges.

[McArdle's disease without typical symptoms].

A 25-year-old female with McArdle's disease was reported. She had no characteristic symptoms for McArdle's disease such as muscle cramp and brown urine, but had general fatiguability from childhood. On examination, she showed no neurological abnormalities including muscle atrophy and weakness. On laboratory examination, serum creatinine kinase (CK) level was elevated, though serum lactic acid level remained unchanged after the ischemic forearm exercise test. Muscle biopsy from the biceps brachii showed almost completely absent phosphorylase activity both histochemically and biochemically. Thus, she was diagnosed as having McArdle's disease. The skinned fiber test of the muscle showed no enhanced Ca induced Ca release (CICR), and serum VLDL level was normal. Her 27-year-old elder brother had similar clinical symptoms and serological abnormalities and may also have McArdle's disease, although muscle biopsy was not performed. A possibility of McArdle's disease should be considered when we encounter a patient who has only general fatigue and high serum CK level.

[Statistical observation of crowns and bridges in Matsumoto Dental College].

A study was made of 11,425 crowns and 2,385 bridges which had been fabricated for 5,588 patients at the Prosthetic Clinic of Matsumoto Dental College during the fourteen-year period, from January 1973 to December 1986. Some of the results were as follows: 1. 753 patients, 1,585 crowns and 372 bridges in 1983 were the most numerous in all the survey years. 2. The rate of vital teeth of crowns has increased from 1984. 3. Of crowns, 22.1% and of bridge retainers, 22.3% were fabricated as porcelain fused to metal crown in 1985. 4. The rate of resin facing crowns of crowns and bridge retainers increased sharply in 1986. 5. Of crowns, 19.0% and of bridge retainers, 30.5% were fabricated as partial coverage crowns in 1986. 6. The rate of more than 5-unit bridges increased from 1982 to 1985.

[The significance of measurement of contents of serum serine protease inhibitors in senile dementia of the Alzheimer type].

We have evaluated serum alpha 1-antichymotrypsin content in dementia of the Alzheimer type (DAT). The subjects consisted of 26 patients with DAT, 15 with cerebrovascular dementia, 10 with mixed type dementia, 2 with Down syndrome, 17 with Parkinson disease, 14 with spinocerebellar degeneration, 14 with cerebrovascular disease without dementia, 12 with Duchenne muscular dystrophy and 77 normal controls. DAT group showed statistically significant increase of serum alpha 1-antichymotrypsin content, as compared with normal control group (p less than 0.001), mixed type dementia group (p less than 0.05) and other 6 groups (p less than 0.001). However, the levels of both alpha 1-antitrypsin content and inter-alpha-trypsin inhibitor content, included in serine protease inhibitors, were not significantly different between DAT and normal control groups. These findings indicated that measurement of serum alpha 1-antichymotrypsin is useful as the diagnostic marker of DAT.

Serum concentration of alpha 1-antichymotrypsin is elevated in patients with senile dementia of the Alzheimer type.

Levels of the serum protease inhibitors (alpha 1-antichymotrypsin, alpha 1-antitrypsin, and inter-alpha-trypsin inhibitor) were measured in patients with senile dementia of the Alzheimer type (SDAT) to determine whether these levels might be useful as diagnostic markers of SDAT. The levels of alpha 1-antichymotrypsin were found to have no relationship to either age or sex. The SDAT group showed levels of alpha 1-antichymotrypsin significantly higher than those of any of the following groups: normal control group (p less than 0.001), vascular dementia group (p less than 0.01), mixed type dementia group (p less than 0.05), and 3 other groups (Parkinson's disease, spinocerebellar degeneration, cerebrovascular disease) (p less than 0.001). The serum concentration of alpha 1-antichymotrypsin seemed to increase slightly in advanced stages of the disease. The serum levels of both alpha 1-antitrypsin and inter-alpha-trypsin inhibitor differed insignificantly in comparison of the SDAT group and normal control group. Of the 3 serum protease inhibitors measured, only alpha 1-antichymotrypsin was found to increase significantly in SDAT. These findings suggest that the measurement of serum concentration of alpha 1-antichymotrypsin may prove useful as a diagnostic marker of SDAT.

[An autopsy case of a so-called carcinosarcoma of the pancreas].

A case of a "so-called carcinosarcoma" of the pancreas that occurred in a 48-year-old female is reported. At autopsy, the pancreatic tumor was seen to have deeply invaded the gastric wall, causing ulceration. The pancreatic tumor consisted of two different histological components: a squamous cell carcinoma and spindle-cell sarcomatous tumor cells. Transitional areas between the two different tumor cells were also present. Since there were transitional areas and a lack of definitive electron microscopic evidence favoring a sarcoma in the sarcomatous tumor cells, the pancreatic tumor was considered to represent a "so-called carcinosarcoma".

Homicide in childhood. Distinguishable patterns of risk related to developmental levels of victims.

To study the characteristics of homicide in childhood, a review was done of the 29 medical examiner-autopsied children under 10 years of age who died under suspicious circumstances in Cook County in the first half of 1981 (17 by homicide, 12 by manner undetermined). Eleven cases occurred in January. The city victims came from neighborhoods characterized by poverty and female-headed households, consistent with the ecological model of child abuse. Eleven victims were infants: seven female, five white. The older victims were increasingly male and black with increasing age. Mean ages differed markedly for different types of injuries, with CNS and miscellaneous injuries typical in infancy, arson and beatings in toddler/preschool ages, and gunshot and pedestrian injuries in the school years. The data suggest that there are three clinically distinguishable types of homicide in childhood--infanticide, fatal child abuse and neglect after infancy, and murder in the community--related to developmental characteristics of the victims.

LOH analyses of premalignant and malignant lesions of human breast: frequent LOH in 8p, 16q, and 17q in atypical ductal hyperplasia.

The number of breast cancer patients is increasing yearly, and it is important to establish effective methods for clinical management. We investigated loss of heterozygosity (LOH) in tumors derived from 23 patients that harbored synchronous lesions of atypical ductal hyperplasia (ADH), ductal carcinoma in situ (DCIS), and invasive ductal carcinoma (IDC). Fourteen selected microsatellite markers that were mapped to and/or very close to the tumor suppressor genes or regions with frequent LOH in breast cancer. Our results suggested that i) losses of chromosomal regions 8p, 16q, and 17q are early genetic changes, and ii) ADH is a premalignant lesion for the development of breast cancer.
