[Long-term home parenteral nutrition using a totally implanted cannulation system].

A 29-year-old patient suffering from stenosis of the rectum and a periproctal fistula due to a severe form of Crohn's disease was completely fed by the parenteral route for 15 months, incl. 13 months at home, via a totally implanted cannula system Implantofix, Braun Co.. The patient felt throughout the period of parenteral nutrition very well, he worked and the secretion from the fistula stopped after four weeks. After 15 months of complete parenteral nutrition and elimination of oral food intake a marked improvement of the local finding in the rectum was observed. All laboratory findings (haemogram, liver tests, urea, creatinine, transferrin, albumin, cholinesterase and pre-albumin, serum levels of sodium, potassium, chlorides, calcium and phosphates) were throughout the observation period within a normal range. The body weight of the patient increased during the 15 months by 1 kg. The described case is the first one where domiciliary parenteral nutrition was used in Czechoslovakia under ambulatory conditions.

[Actinomycosis of the ovary as a cause of cachexia in a young patient].

The authors describe the case of a 40-year-old female patient treated for prolonged periods unsuccessfully with several antibiotics on account of a febrile condition of obscure aetiology and subsequently repeatedly subjected to laparotomy on account of intra-abdominal abscesses. The cause of the fever, abdominal pain and gradual cachectization was an abdominal form of actinomycosis. After establishment of the diagnosis the patient was successfully treated by long-term penicillin administration and recovered completely. The authors discuss experience reported in the literature and therapeutic possibilities in actinomycosis.

[Catabolism after direct myocardial revascularization].

The objective of the investigation was to assemble more knowledge of catabolic processes after cardiosurgery. In 14 patients after 6-hour intervals following an aortocoronary bypass the total production of urea nitrogen, Bistrian's catabolic index and the sodium:potassium ratio in urine was assessed. The urea nitrogen production varied within a range of 5.4-9.5 g per 12 hours. These values when calculated per 24 hours are 94-108 g of protein. The catabolic index was in the range of 5.8-11.2, i.e. within the zone of intense stress. Also the values of the Na/K ratio (1.0-1.9) suggested intense stress. The authors discuss the causes and possibilities to relieve the stress response.

Acute phase proteins and prognostic inflammatory and nutritional index (PINI) in moderately burned children aged up to 3 years.

In 27 children (12 boys and 15 girls) aged 3 months to 3 years, the plasma levels of albumin, prealbumin, orosomucoid and C-reactive protein (CRP) were determined after burn trauma. The results of these measurements were used to calculate the prognostic inflammatory and nutritional index (PINI). The children were assigned to two groups: a group with burns covering less than 10 per cent of the body surface area (BSA) (minor burns), and those with burns covering 11-20 per cent BSA. The lesions involved were either dermal burns (Class IIB) or full skin thickness burns (Class III). While the group with minor burns showed only a small decrease in albumin and prealbumin levels, the decrease in children with extensive burns was well below values on the first 7 days. On days 2, 5 and 7 after burning, the differences between both groups were statistically significant. Orosomucoid levels were somewhat elevated in the group with extensive burns. This difference, however, was not statistically significant compared with the group with minor burns whose values were slightly below the normal range. The levels of C-reactive protein were statistically significantly increased in children with extensive burns compared with those with burns. The PINI value correlated positively with the burn area given in per cent (for day 5 of follow-up, the regression line equation was y = 9.346 + 1.65 x). These data indicate that there are no pronounced changes in plasma protein levels in the group with minor burns.(ABSTRACT TRUNCATED AT 250 WORDS)

Analysis of rapid oscillations of glucose and free fatty acids in plasma.

The authors analyzed rapid oscillations of blood sugar (GL) and free fatty acid levels (FFA) in serum of healthy subjects. They investigated a series of blood samples taken under conditions of absolute rest from the cubital vein at 15-s intervals for a period of 6 min. In addition to common statistical parameters, they calculated the course of autocorrelation and cross-correlation functions and periodograms. The magnitude of oscillations is significantly higher than the error of the biochemical methods. In some sequences periodicities were detected which were statistically significant in 23.8% of GL and in 38.1% of FFA. 24-point series of GL collected in parallel from both arms correlate in 36.3% positively, in 27.3% negatively, and in 36.4% they do not correlate. Series of FFA and GL collected simultaneously from one site correlate mutaually in almost all instances either positively or negatively, frequently with a time shift. The oscillations may be due to (a) feedback regulations of the levels of the two metabolites, (b) permanent mutual interaction between the FFA and glucose level and (c) an uneven concentration of the two metabolites in different parts of the circulation. The above factors may combine, and the list of possible factors may not be complete.
