[Treatment of 11 cases of pulmonary arteriovenous fistula].

We have experienced a total of 11 patients operated for pulmonary arteriovernous fistula (PAVF) from 1976 to 1993 consisting of three males and eight females with the average age of 35 years. To reserve the pulmonary function, limited resections were applied for all cases (partial resection in 8, fistula excision in 2, and segmentectomy in one case). The most recent case, a 36-year-old female who had two abnormal shadows on her routine chest X-ray, was diagnosed as PAVF by pulmonary arteriogram. She was treated by fistula excision in the right S8 lesion and embolization in the left S1 + 2 lesion. Because of its unsatisfactory natural course, PAVF should be treated aggressively by surgery and/or by embolization according to the size, shape and localization.

[Clinical analysis of lung cancer in patients with chondromatous hamartoma].

Between 1978 and 1993, 776 patients with lung cancer and 18 patients with chondromatous hamartoma were operated on in the department of Surgery II in Okayama University Hospital. 3 patients had both lung cancers and chondromatous hamartomas. They were all men. Two patients were 65 years old and the other was 82 years old. They all pointed out them by a medical examination of chest X-ray. 2 patients had lung cancers and chondromatous hamartomas in the same lobe, the other had them in other lobe of the same side. Lung cancer and chondromatous hamartoma localized in near area. It was difficult to discriminate chondromatous hamartoma between pulmonary metastasis of lung cancer before an operation.

Administration of prostaglandin E1 after lung transplantation improves early graft function.

Early graft dysfunction continues to be a major clinical problem after lung transplantation. The objective of this experiment was to determine whether continuous administration of prostaglandin E1 (PGE1) after lung transplantation has a beneficial effect on early graft function. Left lung allotransplantation was performed in 10 size-matched mongrel dogs (weight, 24.4 to 31.4 kg). Lung preservation consisted of a bolus injection of PGE1 (250 micrograms) into the pulmonary artery, followed by a pulmonary artery flush with 50 mL/kg of 4 degrees C modified Euro-Collins solution. The lungs were then stored at 1 degree C for 12 hours. Left lung transplantation was performed using standard technique. The right pulmonary artery and right bronchus were ligated prior to chest closure. Animals were placed in the supine position and ventilated for 6 hours with 100% oxygen at a rate of 20 breaths/min, a tidal volume of 550 mL, and a positive end-expiratory pressure of 5 cm H2O. Animals were randomly allocated to one of two groups. Group I animals (n = 6) received continuous PGE1 infusion from the onset of implantation. The dose was gradually increased and fixed when mean systemic pressure showed a 10% decrease (mean PGE1 dose, 31.7 +/- 6.9 ng.kg-1.min-1). Group II animals (n = 4) received no PGE1. After the 6-hour assessment period, arterial oxygen tension and alveolar-arterial oxygen pressure difference were preserved in group I compared with group II (group I versus group II: arterial oxygen tension, 255.8 +/- 37.6 mm Hg versus 64.7 +/- 7.9 mm Hg [p < 0.05]; alveolar-arterial oxygen pressure difference, 411.1 +/- 70.5 mm Hg versus 597.5 +/- 1.3 mm Hg [p < 0.05]).(ABSTRACT TRUNCATED AT 250 WORDS)

Pentoxifylline reduces lung allograft reperfusion injury.

Early graft dysfunction remains a significant problem in clinical lung transplantation. Pentoxifylline, a methylxanthine derivative, has been shown to have various beneficial effects on neutrophil-induced lung injury. We investigated effects of pentoxifylline on early posttransplantation lung function in a canine allograft model. Ten dogs underwent left lung allotransplantation. Donor lungs were flushed with modified Euro-Collins solution (50 mL/kg) and stored in an inflated state for 18 hours at 1 degrees C. In five experiments (group I), pentoxifylline was added to the flush and storage solutions (200 mg/L) at the time of harvest. The recipient animals received pentoxifylline (20 mg/kg intravenously) before reperfusion followed by pentoxifylline (0.1 mg.kg-1.min-1 intravenously) during the 6-hour posttransplantation assessment period. In group II, donors and recipients received no pentoxifylline. To evaluate only allograft function, the right main pulmonary artery and bronchus were ligated immediately after implantation. For 6 hours thereafter hemodynamics and gas exchange were assessed at 15-minute intervals while the animal was ventilated at an inspired oxygen fraction of 1.0. After 1 hour of assessment there was a significant difference in gas exchange between the groups, which persisted until the end of the study. By the end of the 6-hour assessment, the mean arterial oxygen tension was 236.7 mm Hg for group I versus 101.1 mm Hg for group II (p < 0.01), and the alveolar-arterial oxygen difference was 443.1 mm Hg versus 562.2 mm Hg (p < 0.015). Hemodynamics were not different between groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Donor lung procurement: assessment and operative technique.

Given the relative scarcity of suitable donors and the widespread application of cardiac and pulmonary transplantation, it is imperative that a heart and two lungs be extracted from each donor. From increasing clinical experience and laboratory investigation in lung preservation, more flexible criteria for the assessment of potential lung donors are emerging. In this communication, we present our current criteria of donor lung suitability, and a simple and reliable technique of combined cardiopulmonary extraction that has provided suitable heart and lung grafts with excellent preservation, used in our last 150 donor organ procurements.

Lung preservation with low-potassium dextran flush in a primate bilateral transplant model.

We used a bilateral lung transplant model to confirm, in primates, the results of lung preservation studies previously obtained in a canine single-lung transplant model. The donor lungs were flushed with low-potassium dextran solution and maintained semiinflated with 100% oxygen at 10 degrees C for a planned ischemic time of 12 hours for the lung implanted first. Of eight experiments performed, results in the 6 operative survivors form the basis of this report. After bilateral lung transplantation, animals were maintained on a ventilator for 6 hours; arterial oxygen tension, pulmonary artery pressure, and pulmonary vascular resistance were determined in the recipients at 2, 4, and 6 hours after transplantation and compared with donor values, which served as controls. Arterial oxygen tension in the recipients did not differ from the controls (p = not significant), whereas the pulmonary artery pressure and pulmonary vascular resistance showed significant elevation (p < 0.05 versus control values). After the 6 hours of assessment, the animals were extubated and 3 survived for 48 to 72 hours with a mean arterial oxygen tension of 69 mm Hg on room air. These results demonstrate excellent lung function after a minimum of 12 hours of preservation in a primate model in which the animal is totally dependent on the function of transplanted lung tissue, and confirm the potential for prolonged clinical lung preservation.

[Extended resection for lung cancer].

Of the 740 patients with primary lung cancer operated on at our department between January, 1982 and December, 1991, 108 (14.6%) underwent extended operation involving combined resection of adjacent organs. The 5-year survival rates in patients who underwent resection of the parietal pleura (n = 62), thoracic wall (n = 25), diaphragm (n = 11), left atrium (n = 7), superior vena cava (n = 6), and aorta (n = 5) were 13.8%, 20.5%, 14.9%, 0%, 51.4%, 0%, and 20.0%, respectively. Surgical treatment for stage III advanced lung cancer is controversial, but the 5-year survival rates in those who underwent combined resection of the parietal pleura, thoracic wall including the ribs, left atrium, and aorta were not lower than that in all patients with stage III lung cancer. Especially, long-term survivals of 5 years and 11 months and 10 years and 6 months were obtained in patients who underwent resection of the left atrium, and 6-year survival was observed in a patient who underwent resection of the aorta, suggesting that extended resection is effective in some cases. The outcome of combined resection of the diaphragm and the superior vena cava was poor, with no patients surviving for 5 years.

[Thirty-eight operative cases of mediastinal cyst: with particular reference to bronchogenic cyst].

Thirty-eight patients (16 males and 22 females) with mediastinal cysts have been treated at the Second Department of Surgery at Okayama University from 1978 to 1989. There were thirteen cases of thymic cyst, nine cases of pericardial cyst one case of pericardial diverticulum, eight cases of bronchogenic cyst, two cases of dermoid cyst, two cases of cystic lymphangioma, one case of esophageal cyst, one case of gastroenteric cyst, and one case of aneurysmal bone cyst. MRI seemed to be useful for the qualitative diagnosis of mediastinal tumors as cystic or solid. Based on Maier's classification and considering the result of this study, bronchogenic cysts may be divided into the following groups: 1) paratracheal, 2) carinal, 3) hilar, 4) paraesophageal [a) upper (Iu), b) lower (Im, Ei)], 5) cephalad recess of the azygos vein (CRAzV) and 6) miscellaneous. According to this new classification, these bronchogenic cysts could be allocated to (3) hilar: 1 cyst, (4-a) paraesophageal-upper: 1 cyst, (4-b) paraesophageal-lower: 1 cyst, (5) CRAzV: 4 cysts, and (6) miscellaneous: 2 cysts.

The effects of radical scavengers and leukocyte-depleted blood on reperfusion injury of extirpated rabbit lung.

Oxygen radicals produced by polymorphonuclear leukocytes were considered primarily responsible for reperfusion injury in lung transplantation. Using the extirpated rabbit lungs as a transplant model, we measured lung water volume, the oxygen radicals of lung tissue using a direct method (electron spin resonance) and an indirect method (measurement of peroxide lipids). The effects of free radical scavengers, human superoxide dismutase (h-SOD) and catalase (CAT), and leukocyte-depleted blood on reperfusion injury were evaluated in three experimental groups. Group I (n = 8, control): Lung reperfusion was performed with blood from other rabbits. Group II (n = 7): Immediately before reperfusion, h-SOD (1,500 u/ml) and CAT (3,000 u/ml) were added to the blood. Group III (n = 7): Reperfusion was performed with the leukocyte-depleted blood. Severe pulmonary edema and an elevation of malondialdehyde (MDA) occurred in Group I. In Group II, addition of radical scavengers to the reperfusion blood produced only mild pulmonary edema, but an elevation of MDA occurred as in Group I. In Group III, pulmonary edema and MDA elevation were almost completely suppressed.

[Clinical study of synchronous multiple primary cancer involving the lung].

Ten cases of synchronous multiple primary cancer involving the lung were experienced in our hospital during the period from January 1962 to May 1987. The incidence rate was 1.3% of all hospitalized cases of primary lung cancer. The counterpart organs were thyroid gland (4 cases), stomach (2 cases), colorectum (2 cases) and lung (2 cases). Three of four thyroid cancer cases were resected at the same time of pulmonary resection, whereas the stomach and colorectal cancer cases underwent two stage operation. Lung perfusion scintigraphy was very useful in determining the operative method for synchronous multiple primary lung cancer. The 5-year survival rate of all cases of synchronous multiple primary cancer involving the lung was 26.3%, with no hospital death. Surgical treatment is recommended for such cases when both cancers are expected to be resected completely.

The surgical treatment of invasive thymoma. Resection with vascular reconstruction.

We have treated 96 patients with thymoma during the past 27 years; 57 of them had invasive thymoma. Surgical resection was undertaken in 45 of the 57 patients with invasive thymoma, including 14 patients in whom tumors were extirpated with vascular reconstruction. In addition, six patients with other thymic malignancies were treated in the same way. In six patients only angioplasty was performed, whereas artificial grafts were used in the other 14 patients. Ten of the 20 patients are still alive and well. One has probably been cured, since no signs of recurrence have been noted during the postoperative follow-up period of 8 years. No complications or occlusive symptoms were observed in these patients postoperatively, except in one who died after gastrointestinal hemorrhage. In one patient long-term patency of the vascular graft was confirmed by angiography and magnetic resonance imaging 5 years after operation. This intensive surgical procedure improved the quality of life for patients with the superior vena caval syndrome and may also have improved the prognosis of the patients with invasive thymoma.

Changes in levels of arachidonic acid metabolites in blood and bronchoalveolar lavage fluid after warm ischemia-reperfusion of lung.

The purpose of this study was to evaluate the role of arachidonic acid metabolites in the reimplantation response after lung transplantation in mongrel dogs. The left lung was used and two groups were studied. Group I underwent hilar stripping, while Group II underwent hilar stripping plus warm ischemia for 60 min., achieved by clamping the left pulmonary artery and veins. We measured the lung wet to dry weight ratio (W/D ratio), total pulmonary vascular resistance (TPVR), and blood and bronchoalveolar lavage fluid (BALF) levels of leukotriene B4 and C4 (LTB4,C4) and thromboxane B2 (TXB2). These parameters were measured periodically for 7 days after reperfusion. In group II, the W/D ratio and TPVR were significantly increased in comparison with Group I. The blood LTC4 level was elevated immediately after reperfusion, and BALF level of LTC4 also rose subsequently. These levels changed concomitantly with the W/D ratio. The above results suggest that arachidonic acid metabolism plays an important role in the reimplantation response, especially in pulmonary edema.

[A case of malignant lymphoma arising from chest wall in chronic empyema].

A 70-year-old male was admitted with complaints of anterior chest pain and tumor formation. He has a history of pulmonary tuberculosis and was performed artificial pneumothorax therapy 37 years ago. The diagnosis on admission was perforation of empyema cavity into the chest wall. But thoracotomy revealed that the chest tumor arising from chronic empyema wall was a malignant lymphoma. Decortication and extirpation of the tumor were performed with combined partial resection of chest wall. Immunohistological examination showed that the histology was diffuse large T cell type of Non-Hodgkin's malignant lymphoma. The patient has been alive and well for 24 months postoperatively.

[Experimental studies on changes of SOD activity levels and reperfusion injury after lung transplantation].

The purpose of this study is to evaluate the role of superoxide dismutase (SOD) in reperfusion injury after lung transplantation in mongrel dogs. Canine left lungs were used and three groups were studied. Group I underwent complete hilar stripping (n = 6). Group II underwent complete hilar stripping and was kept in warm ischemia for 60 min. by clamping left pulmonary artery and veins (n = 6). Group III underwent the same surgery as Group II and kept in warm ischemia for 120 min (n = 6). To evaluate the function of the lung, arterial blood gas, left total pulmonary resistance (ITPR) and lung wet to dry weight ratio (W/D ratio) were measured in transient contralateral pulmonary arterial occlusion periodically for 7 days after reperfusion. Also, plasma and bronchoalveolar lavage fluid (BALF) levels of SOD like activity, angiotensin converting enzyme (ACE) activity and ceruloplasmin were measured before operation and periodically after reventilation and reperfusion. Additionally, using dialyzer and electron spin resonance (ESR) spectrometry, plasma levels of extracellular SOD (EC-SOD) activity were measured. The results obtained were as follows. 1) In Group II and III, W/D ratio, ITPR and arterial blood gas were significantly increased in comparison with Group I. 2) Though there were no significant changes in the BALF levels of SOD like activity, ACE and ceruloplasmin and in the plasma levels of ACE and ceruloplasmin, the plasma level of SOD like activity rose 3 hours after reperfusion. 3) The plasma level of EC-SOD activity rose along with that of SOD like activity without any change in intra-cellular SOD levels. The above results suggest that EC-SOD plays an important role in cyto-protection against reperfusion injury after lung transplantation.

[A case report of widely recurrent carcinoid tumor of the thymus].

We performed wide resection of recurrent carcinoid tumor of thymus. A 62-year-old male patient admitted to our department because of swelling of anterior chest wall. He had received extirpation of thymic carcinoid tumor 5 years ago and he was pointed out large mass formation in anterior mediastinum before admission. Wide resection of tumor including pericardium, chest wall, left lung and left brachiocephalic vein was performed. Left brachiocephalic vein was reconstructed and pericardium was replaced with Gore-Tex patch. The carcinoid tumor of thymus sometimes invade to adjacent organs and is apt to recur long time after first operation.

[Three cases of endobronchial hamartoma].

We reported three cases of endobronchial hamartoma. There were some difficulties in diagnosis of the disease by bronchoscopic examination, because the tumors were covered with normal bronchial epithelium. But X ray film and CT film contributed to the diagnosis when calcification of intra-bronchial tumor was observed. Three different modes of operation were performed as extirpation of tumor by wedge resection of upper lobe bronchus, sleeve resection of left S6 segmental bronchus, and right middle lobectomy.

[A case of mediastinal cystic lymphangioma--case reports and review of the literature in Japan].

A 15-year-old woman was admitted to our hospital with complaints of left anterior chest pain and an abnormal shadow on her chest X-ray film. At the left thoracotomy, a large cyst (20.5 x 15 x 3 cm) was located in the left anterior mediastinum and contained translucent yellow fluid. Histological diagnosis was cystic lymphangioma. Mediastinal cystic lymphangioma is very rare among mediastinal tumors. Altogether 32 cases of mediastinal cystic lymphangioma including our case were collected from the Japanese literature and reviewed.
