British women's choice between sentinel node biopsy and axillary node clearance for breast cancer.

BACKGROUND: Recent studies have demonstrated that the sentinel node biopsy (SNB), which utilises a simple principle, is a reliable and minimally invasive method for determining the status of the regional lymph nodes in patients with clinically node-negative breast cancer. The technique is increasingly being used in the management of patients with early breast cancer despite the lack of long-term data from randomised controlled trials which are currently in progress. However, increasing numbers of breast cancer patients are specifically requesting SNB in order to avoid axillary nodal clearance (ANC) if the sentinel node is negative for malignancy. The aim of this study is to determine the proportion of female patients attending a breast unit who would choose the SNB in preference to ANC. METHODS: A detailed questionnaire, based on a utility analysis and containing clear and precise information on arm morbidity associated with ANC (1 in 10) versus a potential reduction in the 5-year-survival rate with SNB (1 in 1000) was distributed to 200 female patients attending the Princess Grace Hospital Breast Unit (London, UK) during January 2003. The questionnaire consisted of two possible treatment scenarios (ANC or SNB) for early-stage breast cancer with their associated risks and benefits, and participants were asked to choose one of the two options. RESULTS: A total of 200 completed questionnaires were received. Out of the 200 women, 99 (49.5%) chose the SNB option and 101 (50.5%) chose the ANC option. There is no significant relationship between the option made with a woman's age (mean age: 53.4 vs 54), family history of breast cancer (p = 0.118), personal history of breast cancer (p = 0.85), or employment status (p = 0.96). CONCLUSION: This study demonstrates that a high level of acceptance exists among women for SNB as an alternative to ANC as the preferred treatment of early breast cancer, despite the risk involved. As this exciting technique continues to evolve and improve, it is hoped that more women will be given the choice to opt for this treatment and that eventually SNB may completely replace ANC as the standard treatment for early breast cancer.

Infrequent occurrence of vancomycin-resistant enterococci in poultry from Malaysian wet markets.

Fifty samples of chicken, duck and geese faeces were obtained from 13 wet markets in Kuala Lumpur to study the prevalence of vancomycin-resistant enterococci (VRE) among local market poultry. Biotyping of colonies grown on azide agar incubated at 45 degrees C yielded E. pseudoavium, E. faecalis, E. faecium and E. gallinarum from chicken faeces and E. malodoratus, E. faecalis, E. faecium, E. gallinarum, E. hirae/dispar, and E. durans from goose and duck faeces. On agar containing 6 mg/ l of vancomycin, one strain of E. flavescens was identified, giving a VRE detection rate of 2.0%. This isolate had a vancomycin M.I.C. of 8 mg/l as determined by the Etest, and the van C-3 gene that was identified by PCR followed by sequence analysis. The prevalence of VRE among poultry sold in local markets appears to be low, and may reflect the infrequent use of antimicrobials in our poultry farms. Nevertheless, the possibility of human acquisition of microbes via the food chain cautions against the use of antimicrobials in animal husbandry that may encourage the emergence and spread of multi-drug resistant organisms like the VRE among animal microbial flora.

Twin pregnancy following transmyometrial-subendometrial embryo transfer for repeated implantation failure.

Most in-vitro fertilization (IVF) failures are due to failure of implantation. We present a case of a successful attempt at transvaginal/transmyometrial/subendometrial embryo transfer in a patient with seven previous failures in assisted conception cycles. We demonstrate achievement of a twin pregnancy, which resulted in the delivery of healthy twin girls in January 1997, following a novel technique of embryo transfer.

Potentiation of the renal responses to bradykinin by inhibition of neutral endopeptidase 3.4.24.11 and angiotensin-converting enzyme in anesthetized dogs.

The renal vascular and excretory responses to intrarenal bradykinin were obtained in anesthetized dogs receiving the angiotensin-converting enzyme inhibitor, captopril, and the neutral endopeptidase inhibitor SQ 28,603 (N-[2-(mercaptomethyl)-1-oxo-3-phenylpropyl]-beta-alanine) given individually and together. Intrarenal bradykinin injections of 0.1, 1 and 10 ng/kg, respectively, increased the area over the curve of the renal blood flow response (RBF) by 6 +/- 2, 15 +/- 2 and 101 +/- 8 ml, respectively, sodium excretion by 0.6 +/- 0.4, 2.9 +/- 0.5 and 21.8 +/- 1.3 microEq/min, respectively and urinary cyclic GMP excretion by 23 +/- 12, 56 +/- 22 and 210 +/- 35 pmol/min, respectively. After 10 mumol/kg i.v. of captopril, area over the RBF curve increased by 5 +/- 4, 30 +/- 6 (P < .05) and 142 +/- 14 ml (P < .05), respectively. Captopril also significantly prolonged the sodium response to 10 ng/kg of bradykinin although the peak natriuretic activity (32 +/- 5 microEq/min) was not significantly different from the natriuresis obtained under control conditions (22 +/- 1 microEq/min). In contrast, 30 mumol/kg i.v. of SQ 28,603 increased only the peak natriuretic responses to 1.1 +/- 3.9 (P < .05), 12.1 +/- 8.4 and 40.4 +/- 12.4 (P < .05) microEq/min without prolonging the sodium response or affecting the vascular activity of bradykinin.(ABSTRACT TRUNCATED AT 250 WORDS)

The effect of clomiphene citrate treatment on cervical mucus and plasma estradiol and progesterone levels.

OBJECTIVES: To study the relationship between cervical mucus (CM) quality, postcoital test (PCT) results and plasma estradiol (E2) in clomiphene citrate (CC)-treated women. A subsidiary aim was to study the relationship between CM quality and plasma progesterone (P). DESIGN: Untreated women were compared with oligo-ovulatory patients given CC. SETTING: Infertility Clinic, Fazakerley Hospital, United Kingdom. PATIENTS, PARTICIPANTS: Fifty-one untreated patients and 31 women given CC. INTERVENTIONS: The treated women were given 50 mg/d CC from days 2 to 6 of their cycle. MAIN OUTCOME MEASURES: A CM assessment, a PCT, plasma E2, and P were performed at the anticipated time of ovulation based on at least two previous basal body temperature charts and menstrual patterns. RESULTS: In untreated women there was a very strong tendency for CM quality to improve with rising plasma E2 levels and to worsen with rising plasma P levels. There was a significant association between CM quality and PCT results. Similar results were found in CC-treated women, except that plasma E2 was very significantly higher and there was a significant inverse relationship between plasma E2 and CM quality. CONCLUSION: High plasma E2 in the periovulatory phase in CC-treated women is a marker for increased sensitivity to and continuing action of the antiestrogen. This impairs the quality of the CM.

Effects of human ANF-(99-126) on sympathetic nerve function, hemodynamic parameters and plasma cGMP levels in anesthetized, ganglion-blocked dogs.

The ability of human ANF-(99-126) (ANF) to affect neuronal function, plasma cGMP levels or various hemodynamic parameters was tested in anesthetized dogs subjected to ganglionic blockade which prevented reflex changes in sympathetic tone. In each group of dogs, heart rate changes due to electrical stimulation of the cardioaccelerator nerve at 0.5, 1, 3 and 10 Hz were measured before and after ANF or vehicle infusion. A possible direct effect of ANF on the beta-adrenoceptor mechanisms was assessed by evaluating changes in heart rate after i.v. administration of isoproterenol (a beta-adrenoceptor agonist). The results showed that i.v. infusion of ANF (200 pmol/kg per min for 60 min), but not vehicle, produced significant inhibition of heart rate responses to low frequency nerve stimulation (0.5-1 Hz) when compared to vehicle 30 min after the end of infusion; high frequency (10 Hz) responses remained unaffected. A significant (P less than 0.05) enhancement in isoproterenol responses was evident in ANF-treated animals at the end of infusion, and 30 min afterward. ANF increased plasma cGMP from pretreatment levels of 28 +/- 4 to 149 +/- 1 pmol/ml at the end of infusion, and falling to 63 +/- 7 pmol/ml 30 min later (t 1/2 = 25.1 +/- 2.2 min). Cardiac output also tended to fall during infusion and remained significantly reduced (-18%) 30 min after infusion's end; blood pressure fell without a decrease in total peripheral resistance. Thus, ANF produced a direct, frequency-dependent depressant action on sympathetic nerve function, reduced both cardiac output and blood pressure, and enhanced chronotropic responses to beta-adrenoceptor stimulation. These persistent changes in hemodynamic parameters and neuronal function may contribute to the mechanism of action of ANF.

Perinephritis hypertension in Macaca fascicularis (cynomolgus monkey): studies of the renin-angiotensin-aldosterone axis and renal hemodynamic function.

The purpose of the present study was to characterize the etiology of bilateral perinephritis hypertension in the non-human primate. Hypertension was induced in female cynomolgus (Macaca fascicularis) monkeys by wrapping both kidneys under sterile surgical procedures. Mean arterial pressure (MAP), plasma renin activity (PRA), plasma aldosterone concentration (ALDO), para-aminohippurate (PAH) clearance, glomerular filtration rate (GFR), urine volume, and sodium and potassium excretion were measured before and weekly after induction of the hypertension. MAP increased progressively from 108 +/- 1 to 135 +/- 4 mmHg during the first 6 weeks; thereafter, MAP remained at this elevated level, PRA was elevated two- to fivefold for up to 10 weeks after the hypertension and ALDO was elevated during 1 (139%), 4 (60%), 6 (196%), 8 (249%) and 10 (148%) weeks of the hypertension. PAH clearance and GFR were significantly reduced during week 1 of the hypertension, but returned to control values by week 2. Urine volume was increased significantly during the first week of the hypertension, while sodium and potassium excretion were not changed. Captopril (15 mumol/kg, intravenously) normalized the blood pressure regardless of the severity or duration of the disease. Additionally, captopril lowered ALDO and increased PRA. It is concluded that bilateral perinephritis hypertension in the monkey is dependent on increased activity of the renin-angiotensin-aldosterone axis.

Centrally mediated pressor response to metiamide as a result of GABA-receptor antagonism.

The mechanism of the pressor response to metiamide was examined in anesthetized cats. Intravenous administration of metiamide had no effect on blood pressure. Infusion into the lateral ventricle (10 mg/kg/min for 10 min), caused a marked pressor response which was due to an increase in sympathetic outflow since pretreatment of cats with guanethidine completely prevented it. In a rat brain synaptosome preparation, both metiamide and cimetidine displaced [3H] CABA. These data strongly suggest that metiamide increases blood pressure in the cat through a central mechanism involving GABA-receptor antagonism.

Effect of propranolol of vascular responses to sympathetic nerve stimulation and plasma renin activity in mongrel dogs.

I.v administration of propranolol (0.2 mg/kg and 1.0 mg/kg) to pentobarbital-anesthetized dogs produced blockade of cardiac beta-receptors and a significant decrease in heart rate. However, only the higher dose of propranolol demonstrated a significant hypotensive effect. Furthermore, this hypotensive action of propranolol was not associated with either adrenergic neruron blockade or changes in plasma renin activity. These results indicate that the initial hypotensive action of propranolol in mongrel dogs is not due to the blockade of beta-receptors, alterations in peripheral sympathetic nervous transmission or plasma renin activity. On the other hand, the action that propranolol is reported to have within the central system may play an important role in accounting for the acute blood pressure lowering action of the compound in mongrel dogs.
