[Validity and reliability of the migraine self-questionnaire Alcoi-1995].

INTRODUCTION: In epidemiological investigation valid, reliable means of diagnosing the illnesses studies are essential. OBJECTIVE: To validate the self-questionnaire 'Alcoi 1995' for the diagnosis of migraine. METHODS: We used the questionnaire Alcoi-1992 (which had been validated previously) to find the prevalence of migraine in homogeneous populations. After this, we selected the significant items to obtain a shorter, more specific questionnaire for diagnosis of migraine, which we called 'Alcoi 1995'. We selected a population of patients and healthy volunteers. The diagnosis of migraine (neurologist and self-questionnaire) was made on criteria of the International Headache Society (IHS, 1988). RESULTS: There were 71 persons included in the study, 40 patients (5 men and 35 women those average age was 39.7 +/- 14.4 years) and 31 healthy volunteers (15 men and 16 women with an average age of 36 +/- 14.4). The sensitivity, specificity, positive and negative predictive value, and the Kappa concordance index for migraine (100%, 100%, 100%, 100% and 1); migraine with no aura (75%, 100%, 100%, 86% and 0.79) and migraine with a typical aura (100%, 88%, 63%, 100% and 0.71) were calculated. CONCLUSIONS: Our results show a high degree of validity and reliability when the self-questionnaire is used for the diagnosis of migraine. It is a tool for use in epidemiological studies which is quick and easy to use, and is cheap.

[Prevalence of migraine in a homogeneous population using the self-questionnaire 'Alcoi-1995'].

INTRODUCTION: There are numerous studies of the prevalence of migraine and very varied findings. Amongst the many reasons for this would seem to be the use of questionnaires for screening populations, absent or incorrect validation of these questionnaires and the use of impracticable diagnostic criteria. OBJECTIVE: To determine the prevalence of migraine in a homogeneous population using the self-questionnaire 'Alcoi 1995' which had previously been validated for the diagnosis of migraine. A self-questionnaire was given to all workers in the same company. Diagnostic criteria of the International Headache Society (IHS, 1988) were used for diagnosis of migraine. RESULTS: The study included 140 persons (43.6% men and 56.4% women with an average age of 26.7 +/- 4.8 years). There was a prevalence of headache and migraine (IC 95%): 90.8% (75.01-100) and 24.3% (16.3-32.3) respectively. The sex prevalence of headache and migraine was 88.5% (88.3-88.8) and 11.5% (3.4-19.4) for men and 34.2% (21.5-47) for women respectively. CONCLUSIONS: There are a large number of studies of the prevalence of migraine with wide variability in the figures obtained (1-2 at 35%). These variations depend on the population studied, age and sex sampled, diagnostic criteria used and the method employed in the survey. After validation of the questionnaire for diagnosis of migraine, this should be used in homogeneous populations before use in broad population studies. Our findings support the use of a suitably validated questionnaire as a useful method for the diagnosis of migraine in epidemiological studies.

[Laparoscopy for the undescended testis].

The authors show their experience in the use of the laparoscopic approach in 18 cases of unilateral exploratory anorchia. They highlight the advantages that this method offers in efficiency, speed, security and minimal aggression compared with conventional exploration of the inguinal channel in the search of the undetectable testis. In the 18 cases studied through the insertion of 5 mm lense in the umbilical area, it was established that a testicular hypoplasia was present in 8 cases. In another 5 cases testicular agenesia was diagnosed, and in the remaining 5, intraabdominal testis were found. In four of these testicles, a laparoscopic approach helped in their correct placement and also in the removal of the fifth in a patient with recurrent leukemia.

[Pulmonary biopsy by thoracoscopy: techniques and results in 9 patients].

Video-assisted endoscopic techniques have decreased the surgical aggression in abdominal and thoracic surgery. In our country, pediatric laparoscopy has been developing slowly, but this is not the case for thoracoscopy. The aim of this paper is to present the techniques and results of thoracoscopy pulmonary biopsy in our first patients. Pulmonary biopsies with this approach had been done in 9 patients (5 males, 3 females). Their age ranged between 30 months and 16 years. In all cases this was the last resort for the diagnosis of pulmonary condensation of unknown etiology. The biopsies were done with the pretied knot in 5 cases, stappler in 1 case and with biopsy forceps in 2 cases. Thoracotomy was necessary in one patient, due to intraoperative haemorrhage. Enough tissue for bacterial and pathological diagnosis was obtained. There was not mortality nor important morbidity related with the technique. Postoperative recovery is better when compared with conventional thoracotomy. Thoracoscopy is an adequate approach to perform pulmonary biopsies in children. The advantages if we compare with open thoracotomy are: 1. The possibility to choose the are to perform a minimally invasive biopsy. 2. To take samples of different pulmonary lobes. 3. Less postoperative pain and shorter hospital stay (36-48 hours).

[The usefulness of manometry in the determination of the morphology of the anal canal].

The appearance of the new lecture systems for the manometry studies by computer, like the Polygram by Synectics (vector volume), offer the possibility to see the circumferential pressure forces, that even in rest conditions as in voluntary contraction, they keep coaptation of the anal canal, and this will act as a continent closure system in the most distal part of the G.I. tract. The study is with the normal parameters obtained in 14 individuals, considered as normal, getting the mean +/- sd pressure of the anal canal convey in mm Hg, from de anal canal profile in rest as in voluntary contraction, and the maximum variability that could exist between the six profile waves, that are obtained in the same individual to develop an image of the anal canal. This valves will allow the author's to get to the bottom of fecal incontinence derivative from anorectal malformations, defining the pressure valves of muscular hypoplasia or surgical outcomes of the malformations.

[Validation of Allen's scale for the diagnosis of stroke].

INTRODUCTION: The importance of cerebrovascular pathology demands the most exact and complete diagnosis possible, by general physical and neurological examination and complementary techniques including computerized tomography (CT). Without CT, diagnosis may be delayed, for which reason such authors as Allen have put forward scales which by means of diagnostic scoring distinguish between stroke types. AIM: To validate the Allen scale as opposed to CT scan in a series of patients suffering from haemorrhagic and ischaemic stroke. MATERIALS AND METHOD: Sixty-two consecutive hospitalized patients suffering from stroke were studied, who all underwent CT scan within fifteen days of admission. RESULTS: The group consisted of 62 patients, 33 males (53%) and 29 females (47%) having an average of 70.2 +/- 11.2 years. The most frequent type of stroke was ischaemic (63%) as opposed to haemorrhagic (37%). Globally for all strokes we obtained the following results: Sensitivity (S) = 61%, Specificity (Sp) = 40%, positive predictive value ( + PV) = 84% and negative predictive value (-PV) = 17%. For ischaemic stroke the results were as follows: S = 76%, Sp = 66%; +PV = 67% and -PV = 76%. Finally for haemorrhagic stroke the results were S = 43%; Sp = 100%; +PV = 100% and -PV = 75%. CONCLUSIONS: The Allen scale has not proved to be a valid instrument in stroke diagnosis due to its low sensitivity and specificity especially in cases of haemorrhagic stroke.

[Quality of life and migraine].

OBJECTIVE: Nowadays, quality of life is considered to be important both in clinical investigation and patient care. Migraine is a particularly disabling illness, with a pronounced effect on general health and on the ability to take part in social activities. We review the instruments used for measuring quality of life and the published studies in which the relationship between migraine and quality of life is analyzed. MATERIAL AND METHOD: In spite of increasing interest in quality of life and the means by which this is measured, there are many problems involved in studying this correctly since there is no agreement as to its definition or how to measure it. In recent years, most of the studies carried out have assessed the socio-economic cost of headache. However, at present, studies tend to focus on the human cost of the quality of life when suffering from headache. Migraine patients have a poorer quality of life than the general population and other populations with chronic illnesses which have always been considered to be severely disabling. CONCLUSION: Research should be aimed at finding the specific and/or generic measures necessary to improve quality of life while suffering from headache. In the future, the aim of new treatments should be to eliminate the pain of the acute crisis and restore functional capacity, reduce the period of disability and improve quality of life.

[Management of primary congenital megaureter with early diagnosis].

We present our follow-up protocol of congenital megaureter (PCM) diagnosed early thanks to showing prenatal ultrasound the dilatation in the urinary system. 47 PCM were studied retrospectively in 35 babies, out of 240 cases of prenatal renal dilatation, from 1984 to 1993. The final diagnosis was established with the early use of ultrasound, cystography, intravenous urography (IVU) and isotope test. The latter two gave hints as to the existence of obstruction, which was the main criterion for early surgery to be prescribed. The surgical treatment, when it was necessary, consisted of ureter reimplantation (Cohen type), following the resection of the stenotic segment. When it was necessary short distal tapering (3-4 cm) was performed. For the follow-up ultrasound, IVU and isotope test were used. Out of the 35 patients observed, 11 were girls and 24 boys, 9 (25.7%) of the PCM were from the right side, 14 (40%) from the left side and 12 (34.2%) bilateral. 36 PCM (76.6%) were non-obstructive and only follow-up was performed. 5 of them, in 3 patients, had infections and were operated on together with the 11 obstructed PCM. In total, 16 PCM were reimplanted, and associated tapering was need in 7 of them. Other 3 patients were operated on for associated pathology. The post-surgical complications were 2 cases or reflux and 1 uric stone that did not need surgery. The development of all cases was favorable. The protocol has been useful for the selection for early surgical intervention of the subsidiary megaureters from those that can be develop without risk.(ABSTRACT TRUNCATED AT 250 WORDS)

[Esophagogastroplasty in childhood: experience in 4 patients].

The most common indications of esophageal replacement in childhood are the esophageal atresia with long gap and caustic strictures. Several techniques has been described, using different components of the digestive tract. Total gastric transposition was performed in 4 children. The indications were esophageal atresia without tracheoesophageal fistula in 3 cases and a caustic stricture in other patient. Follow-up was 12 to 32 months. All patients are normally feed. One patient presented an anastomotic leak that closed spontaneously with total parenteral nutrition but ultimately led to an anastomotic stricture requiring pneumatic dilatation. Other patient presented stricture by sternal compression and required surgical repair. Total gastric transposition is a easy and physiologic technique, with low morbidity and good functional results.

[Passerini-Glazel technique of vulvovaginoplasty: a solution for upper outlet of the vagina].

Since 1988, we have performed the Passerini-Glazel technique, in 5 patients, between 7 months and 3 years old. All showed female pseudohermaphroditism with severe signs of masculinization, and consequently, high outlet of the vagina in the urethra, secondary to Congenital Adrenal Hyperplasia (CAH). The technique use is to get a functional and esthetically acceptable vagina at the expense of a urethral flap and two flaps of skin, coming from the hypertrophic clitoris. The results were excellent in all the patients. In one occasion it was necessary to insert a cylinder of vesical mucous to get a suture without tension of both hemivaginas. In one case there appeared a late vaginal stenosis that was solved with dilations. The present technique is indicated in those cases of CAH, with great testosterone impregnation, in which the vagina ends very high and her primary descent is difficult. It is also indicate in those cases in which there exists the risk of hurting the external sphincter, which would damage the urinary continence mechanism.

[Biliary complications in hepatic transplantation in children].

Biliary reconstruction continuous to be a source of severe complications in paediatric liver transplantation (PLT). Five (7%) biliary complications in 70 PLT in patients aged from 8 months to 13 years with a body weight of 4-37 Kg. were studied. End-to-end common duct anastomosis (group I) with Kehr drainage was performed in 18 cases. End-to-side common duct jejunostomy (group II) was performed in the remaining 52 cases. In this group, two patients were found to have stenosis and biliary stones or sludge respectively and one obstruction with sludge without evident stenosis. All cases required surgery and there was no mortality. Transparietohepatic cholangiography has provided early diagnosis of these complications and has proved to be a reliable method of choice. The use of inert biological material in Kher tubes and steroids slow down the organization of a fistulous tract, thereby causing the complications which arose in group I.

[Role of echography in septic arthritis of the hip in the newborn].

Pyogenic arthritis of the hip in neonates remains a serious problem. The sequelae of delay in diagnosis and treatment can be crippling. Five hips, in four neonates, with a presumed diagnosis of septic arthritis were examined with ultrasonography. In two hips ultrasonography found intracapsular effusion. In the remaining three hips there were no pathological findings. In the two hips with joint effusion an arthrotomy was performed, dislodging pus in both cases. Follow-up radiographs and ultrasonographies of the five hips showed normal joint anatomy. In neonates, ultrasonography provides an early and reliable evaluation of hips in which septic arthritis is suspected.

[Abdominal trauma at a third-level pediatric hospital: study of 150 patients].

All paediatric patients with abdominal trauma admitted in our hospital from 1975 to 1989 were retrospectively studied. We present 150 children with ages ranging from 1 to 14 years, mean age 6 +/- 2.6 years. Sixty-five percent of them were male. Trauma was classified as severe (37%) and mild (63%) and relationated with associated extra-abdominal injuries. The 42% of patients could be attended in other less specialized centres. Since 1982, severe abdominal trauma patient with parenchymal lesions of liver, kidney and spleen were treated according to a non-surgical protocol without secondary complications.
