Follow-up report on burned hands, treatment plans.

A patient initially seen more than a decade ago with deep second-degree burns on both hands returns for long-term follow-up to determine the extent of damage present after regeneration of the dermal-epidermal elements. At the time of his burns, the patient had undergone a nonsurgical treatment program designed to prevent deformity and to protect the unburned and regenerating epithelium. Multiple biopsies done during the follow-up examination demonstrated that the regenerated skin could not be differentiated, on gross histologic examination, from normal skin.

Direction or inertia: the future for regulation of surgical implant devices.

The time has arrived when certain standards are to be introduced to control the introduction of and appropriate use of surgical implant devices. Now is the time for the professional groups involved in standards and medical device legislation to become active and develop surgical implant device standards to be applicable for the future. A commission could be established to work with FDA in respect to the requirements for surgical implant devices. It is time for the clinical organizations involved in writing consensus standards to cooperate in the introduction of surgical implant device standards.

"Acupuncture anesthesia"--a clinical study.

Forty-two patients who were to undergo plastic surgical procedures were asked whether they would accept acupuncture as a substitute for local anesthesia. Eight patients agreed to acupuncture; one of these had 2 operative procedures with acupuncture. Five of the 9 procedures were successful; the remaining 4 required conversion to local anesthesia. After interviewing the patients, we felt that the success of "acupuncture anesthesia" was largely dependent on patient motivation, and that a patient may experience pain during surgical procedures without any change in facial expression or vital signs. We concluded that "acupuncture anesthesia" is of little value in our patient population at present. Its results are unpredictable; therefore, we anticipate that patient acceptance will be small.

Combined cranio-orbital surgery for extensive malignant neoplasms of the orbit.

Extensive, invasive neoplasms of the cranio-orbital region are generally considered to be inoperable and are treated with local radiation therapy and occasionally with chemotherapy. Upon local recurrence of these neoplasms, further treatment efforts are usually deemed futile. Over the past 4 years, 10 patients with extensive neoplasms of the cranio-orbital region that were previously thought to be inoperable were referred to us for further evaluation. Five of these patients had failed an intensive course of radiation therapy. Radical regional cranio-orbital resection was performed. The resection included the skin, globe, sphenoid wings, and orbitofrontal bone. The maxillary sinus and nasal septum were left exposed. The exposed dura mater and the frontal and temporal bones were resurfaced with split-thickness skin grafts. All patients in this series have survived free of local disease to date. Combined cranio-orbital resection offers both palliation and a possible cure for patients with extensive, invasive neoplasms of the cranio-orbital region. Reconstructive surgery can be performed using full-thickness flaps after 2 years of observation for local recurrence.

Periocular congenital melanocytic nevi.

Large congenital melanocytic nevi (CMN) are cutaneous malformations of neural crest origin that have a notorious reputation because of their reported propensity to undergo malignant degeneration. Traditionally, complete surgical excision has been advocated based on the premise that large CMN are a precursor to melanoma. Two cases of large periocular CMN are reported to emphasize the importance of distinguishing them from other congenital skin lesions. Data from the literature support the contention that these pigmented skin lesions spawn malignancies and that prophylactic therapy may be indicated.

A study of autologous cancellous bone particles in long bone discontinuity defects.

Long bone discontinuity defects in dogs were restored by particulate autologous cancellous bone grafts and a Dacron-urethane mesh implant. In six months the discontinuity defects were filled by new bone formation, which was analyzed histologically, radiographically, and by densitometric methods. The postulated mechanism is a field phenomenon of bone induction. The discontinuity defects remained unrestored in the control groups without bone graft material. In the dogs killed at three months incorporation of the graft material was incomplete in the central area of the defect.

Electrodynamometric evaluation of nasal escape in speech disorders.

A new method of detecting the degree of nasal air emission in patients with speech disorders during "continuous" speech. The method employs the use of an electronic sensor placed in front of the nasal opening, which then picks up nasal escape and transfer it to an electrical signal that is recorded on an LED digital readout. The degree of nasal air emission, as portrayed on the LED, is then charted, permitting differentiation of the various speech disorders. This newly described method of nasal speech evaluation is of the utmost importance in the rehabilitation of patients with speech disorders and has proven to be quick and reliable in clinical use. It is also useful in patient selection for the various reconstructive operative procedures used to correct speech abnormalities. The clinical device is helpful to the speech therapist in detecting nasal speech abnormalities and evaluating the clinical progress of a given patient during speech therapy.
