Spiral CT during arterial portography.

Spiral computed tomography (CT) performed during arterial portography offers several advantages compared with portographic studies based on conventional CT technique. Because all hepatic images are derived from a volume data set acquired during a single 24-32-second breath hold timed to coincide with the phase of peak hepatic enhancement, motion artifacts and section misregistration are eliminated and high liver-to-lesion attenuation value differences are present on all sections. These factors, in conjunction with the ability to retrospectively acquire thin, overlapping axial sections, result in improved lesion detection. The ability to produce high vein-to-liver attenuation value differences and two-dimensional multiplanar reconstructions simplifies the identification of hepatic segments and therefore lesion localization. A limitation of all CT portographic methods is the frequent occurrence of nontumorous perfusion defects that, in most cases, demonstrate characteristic locations and appearances. Performing delayed CT following portography is one method by which such pseudolesions may be characterized and differentiated from focal pathologic entities.

Pseudofractures due to Nec-Loc cervical immobilization collar.

The plastic rivets attached to the commonly used Nec-Loc cervical immobilization collar produce linear lucencies which often traverse the cervical vertebrae. Linear lucencies in this location often simulate a fracture. In most instances, the complete outline of the object causing the artifact may be identified. However, identification may not be possible in all cases. Radiologists should familiarize themselves with the appearance of the artifact produced by this particular immobilization device so as to avoid the erroneous diagnosis of a cervical fracture. Whenever the diagnosis is in doubt, the collar should be removed and a repeat radiograph should be obtained.

Occult carpal pathology: tomographic evaluation.

Pluridirectional tomography in coronal and sagittal planes has been used to identify occult carpal pathology in 23 patients with chronic wrist pain following recent or remote trauma. The tomographic studies have detected or further defined the carpal pathology in 22 of the 23 patients studied adding useful information concerning their clinical management.

Role of chest CT in non-Hodgkin lymphoma.

A retrospective study of 48 patients with non-Hodgkin lymphoma (NHL) who underwent 54 computed tomographic (CT) examinations of the chest evaluated the role of chest CT in the management of this disease. Of 18 cases in which chest radiographs were not indicative of NHL, CT scans showed abnormalities consistent with NHL in five (28%). Of 11 cases where radiographs were questionable, CT confirmed NHL in five (45%) and excluded it in six (55%). Of 25 cases where radiographs were consistent with NHL, CT confirmed the findings in 23 (92%) and added information in all 25. Chest CT affected management in eight of 25 treated and five of 19 untreated patients. It appears to be useful in untreated patients with stage I or II NHL but no definite radiographic abnormalities, or with abnormal radiographs but no extrathoracic spread, and in treated patients with questionable radiographs. CT is not helpful in untreated patients with stage III or IV NHL or treated patients with normal radiographs.

Bilateral avulsion fractures of the anterior superior iliac spines in sprinters.

Three cases of bilateral pelvic avulsion fractures in adolescent sprinters are reviewed. An-acute avulsion fracture of the anterior superior iliac spine (ASIS) occurred during active running in three teenaged sprinters. Radiography confirmed a healed fracture of the contralateral ASIS. The clinical features and radiologic appearances are characteristic. The pathophysiology of this type of unique pediatric fracture is discussed. Conservative treatment is successful.

CT of obstructive lobar collapse.

The varied computed tomographic (CT) appearance of obstructive lobar collapse is described in 25 cases. CT is helpful in understanding the morphology and mechanisms of lobar collapse and in diagnosing atypical cases. The final shape of the collapsed lobe is dependent on the size and location of the obstructing tumor and on the amount of retained lung fluid. Focal bulging of the fissure (S-sign) is the most helpful sign in identifying the obstructing tumor. Differential enhancement could not separate the tumor from collapsed lung in six of eight (75%) of the cases studied with intravenous contrast, a finding at variance with previous reports. The limitations of CT in evaluating mediastinal or pleural invasion in the presence of lobar collapse are discussed.

A comparison of Hexabrix and Renografin-60 in knee arthrography.

Forty patients were randomized in a double-blind study to compare the safety and diagnostic quality of Hexabrix vs. Renografin-60 without epinephrine in knee arthrography. Hexabrix is a monoacidic dimer of meglumine and sodium salts of ioxaglic acid with a lower osmolality than conventional contrast agents of similar concentration (32% iodine). Initial films were of equally high diagnostic quality for both groups, but delayed films at 20 and 30 min significantly favored Hexabrix for diagnostic quality. Hexabrix proved to be a safe and effective contrast agent for knee arthrography.

Thoracic cryptococcosis: immunologic competence and radiologic appearance.

Cryptococcosis, a potentially fatal pulmonary infection, is being recognized with increased frequency in both immunologically compromised patients and those without obvious predisposing factors. This review of 24 cases of proven pulmonary cryptococcosis includes 15 immunologically compromised patients and nine noncompromised hosts. The noncompromised hosts tended to have a radiographic pattern of a peripheral nodule or nodules. The compromised hosts demonstrated a wider variety of radiographic abnormalities including single nodules, multiple nodules that progressed to confluence and/or cavitation, segmental consolidation, bilateral bronchopneumonia, or mixed patterns. Adenopathy, cavitation, and pleural effusions were limited to the compromised hosts.

Cystic artery pseudoaneurysm: a rare cause of hemobilia.

Gallstone-induced cystic artery pseudoaneurysm is a rarely encountered cause of hemobilia. A few reports have appeared in the radiologic literature. We report such a case as a reminder of its uncommon occurrence.

Magnetic resonance characteristics of the LGM vena cava filter: technical note.

The LGM (Vena Tech) IVC filter is a recently introduced device for caval interruption. The magnetic resonance imaging safety and imaging characteristics of this filter were evaluated. The filter was proven to lack ferromagnetic properties. It was imaged with minimal artifact and no detectable motion in the magnetic field.
