Intestinal pseudo-obstruction in mucocutaneous lymph-node syndrome.

Mucocutaneous lymph-node syndrome (MCLS) is an acute exanthem with specific clinical features, sometimes complicated by involvement of internal organs. Two patients with MCLS had clinical and radiographic evidence of mechanical small-bowel obstruction, probably on the basis of focal vascular insufficiency, as anatomic obstruction was not documented in either instance. The cases indicate that intestinal involvement in patients with MCLS can simulate intestinal obstruction, and conservative management of such patients is appropriate.

Maternal oral herpes: isolation policy.

Current official recommendations are that parents and personnel with oral herpes lesions be isolated from newborn infants. A survey of 110 neonatal centers, 50% of them replying, showed that most centers do not isolate mothers with oral herpes from their newborn infants. A review of the literature suggest that exposure of infants to personnel or family with oral herpes lesions might occasionally result in disseminated neonatal herpes simplex virus disease. Since the separation of a mother from her newborn infant up to 1 week of ages is a decision that should not be taken lightly, more information is urgently needed to decide whether or not to isolate mothers and hospital personnel with oral herpes lesions from newborn infants. Recommendations to obtain more information concerning these infants are provided.

Failure of the RA 27/3 strain of rubella virus to induce intrinsic interference.

Unlike other strains tested, the RA 27/3 vaccine strain of rubella virus, attenuated in human fibroblasts, failed to inhibit superinfection with Newcastle disease virus (i.e. induce intrinsic interference). Since proteins of the input virion are known to lead to intrinsic interference, these may differ in the RA 27/3 strain from those in other rubella strains.

Support for the medical profession among the aged.

National survey data are utilized to delineate areas of confidence among the aged in the medical profession. Four key specifying variables were used in the analysis-race, sex, socioeconomic status, and size of community. Findings indicate that confidence in medical leaders is lowest among that group which in past studies has been found to visit the doctor most often: the elderly of low socioeconomic status. This finding suggests that improvement in the medical care received by the aged could be an important first step in restoring the elderly's confidence in medical leaders.

The infectious disease consultant and the telephone consultation.

The details of 105 telephone consultations made by 62 community physicians to a subspecialist in Pediatric Infectious Disease were prospectively evaluated. Of physicians seeking consultation 95% were pediatricians. Only one inquiry did not concern the immediate management of a patient. Sixty percent of inquiring physicians were consultants to primary physicians. Of 104 patients 51% were ambulatory and 49% were hospitalized. Most questions concerned topics that were commonly encountered in practice and not newly described illnesses or therapies. The primary basis of the consultant's advice was a larger experience with similar problems rather than convenient access to the literature. The telephone consultation is an effective educational tool for the community physician; it appears to be a barometer of the types of problems confronting the practicing physician and vignettes based upon such consultations may be an effective approach for continuing medical education courses.
