Electromyographic findings in various forms of progressive systemic sclerosis.

Electromyographic findings were studied in 32 women and 7 men with progressive systemic sclerosis. Among the 14 patients in group 1, 7 had the CREST syndrome in an early stage while the 14 patients in group 2 had advanced CREST-type lesions. The 18 patients in group 3 had diffuse scleroderma with extensive visceral involvement. In all groups, the mean duration of single potentials was reduced and the percent of polyphasic potentials was above normal in groups 2 and 3. Distal muscles were more involved in group 1, and proximal muscles in group 2. In group 3, involvement of various muscles was nearly equal. Electromyographic changes correlated with the clinical condition of the patients. Overall, the findings in this study are similar to findings from other laboratories.

Plasmapheresis in the treatment of systemic lupus erythematosus.

Plasmapheresis has been used as a supplementary method for the treatment of 11 cases of SLE (7 with lupus nephritis and 4 without renal changes). In 7 cases there was a marked improvement during the plasma exchanges which lasted from 2 months to 3 years of follow up. In one case the direct result was satisfactory but after 3 months there was a severe relapse and the patient died. In 3 cases (two without lupus nephritis) plasmapheresis was without any effect. We believe that plasmapheresis is chiefly indicated in severe cases with high levels of ds-DNA antibody titers.

Immunological and biochemical findings in patients with porphyria cutanea tarda.

Fifteen cases of PCT were followed up during 1-4 years after the chloroquine treatment with 250 mg daily for 5 days. Clinical improvement was noted in 10 patients, and in 7 there was a full remission. The levels of uroporphyrins in the urine did not correlate with the clinical response whereas characteristic immunopathological findings in the skin became negative in six. The immunofluorescence tests on the skin biopsies of patients with PCT may be not only of supplementary significance for the diagnosis, but also in the evaluation of the course of the disease and the effects of the chloroquine treatment.

Plasmapheresis as a supplementary treatment in pemphigus.

Four cases with pemphigus 3 p. vulgaris and one p. foliaceus were treated with plasmapheresis. The volume of plasma exchange was 500-800 ml once a week for 6 weeks. In one patient the procedure was repeated four times, within 8 months. All patients were treated with prednisone and azathioprine or cyclophosphamide at the same time, but in lower dosages. The results were satisfactory and the remission after the treatment lasted for 11 months to 2 years.

Effect of various sources of organic carbon and high nitrite and nitrate concentrations on the selection of denitrifying bacteria. I. Stationary cultures.

The effect of methanol, ethanol, acetic acid and glucose together with NaNO2 or KNO3 (1,000 mg N/l) on the intensity of denitrification and selection of denitrifying bacteria from the bottom sludge of nitrogenous wastewater reservoir was examined. Denitrification was found to be the most efficient in medium with ethanol or acetic acid. The presence of glucose facilitated the selection of Alcaligenes faecalis whereas the other carbon sources enabled the selection of bacteria of the genus Pseudomonas: methanol -- P. fluorescens, ethanol -- P. mendocina. In medium with acetic acid species selection depends on the form of nitrogen: NaNO2 -- P. fluorescens, KNO3 -- P. aeruginosa.

Tissue specificity of antinuclear antibodies in scleroderma.

Studies of antinuclear antibodies (ANA) were carried out in 39 cases of systemic scleroderma and for comparison in 19 cases of systemic lupus erythematosus (SLE) using indirect immunofluorescence (IF) methods under standard conditions. The results on three different substrates--monkey esophagus, guinea pig lip and rat liver--are reported. In 48.7% of scleroderma cases ANA showed a substrate specificity. The highest percentage of positive results in scleroderma was obtained on monkey esophagus (97.4%) and the lowest on rat liver (61.5%). In SLE, in contrast, only about 13% of the sera displayed such specificity. If only sera with substrate specificity are considered, the positive results on monkey esophagus and rat liver are 94.7% and 21.1%, respectively. Titers of sera reacting positively on 2 or 3 substrates were mostly in agreement, although some sera both in systemic scleroderma and SLE showed higher titers on monkey esophagus. The IF pattern was usually the same regardless of the substrate. Tests for ANA in scleroderma should be performed on at least 2 substrates simultaneously.

Antibodies against extractable nuclear antigens (ENA) in systemic scleroderma.

Antibodies against nuclear ribonucleoprotein (RNP) were found in 5 of 63 cases of systemic scleroderma, whereas they were present in all but one case of mixed connective tissue disease and in 15 of 67 cases of systemic lupus erythematosus. In all RNP positive cases of systemic scleroderma there were some features of other collagen diseases, and their course was relatively more benign. Studies of RNP antibodies in systemic scleroderma may be of importance for treatment and prognosis.

Mixed connective tissue disease (MCTD)--its immunological characteristics.

The concept of the MCDT is controversial; it is still not settled whether it is nosological entity, or a variety of SLE, or a special variety of scleroderma. From the practical point of view the authors consider it important to perform tests for RNA antibodies whenever the characteristics of different collagenoses coexist. Detection of the antibodies is relevant both to the prognosis and the choice of treatment.

Subepidermal bullous disease of childhood with linear IgA deposits.

It has been shown that a group of bullous diseases of children with overlapping symptoms of dermatitis herpetiformis (DH) and bullous pemphigoid (BP), characterized by linear IgA deposits in the unchanged skin, should be regarded as a nosological entity. From the practical point of view it seems justified to set apart this group because of differences in treatment.

Crithidium luciliae immunofluorescence test (CL test) a simple and reliable method for detecting anti-native DNA antibodies.

Antibodies directed against native DNA using Crithidium lucillae as an antigen substrate were demonstrated in 39 of 72 cases of systemic lupus erythematosus (SLE) with active lesions whereas results were negative in 42 cases of SLE during remission and in 69 cases of other collagenoses, and in 20 healthy subjects. A clearcut correlation with the peripheral type of fluorescence of anti-nuclear antibodies (ANA) was found in the routine method of immunofluorescence on the monkey esophagus. Renal lesions were present in almost all cases of SLE with positive results of the test for Crithidium luciliae. The new technique of detecting antibodies against native DNA is simple and creates new diagnostic and prognostic possibilities in SLE.

[Studies of the role of cellular immunity and angiogenetic changes in the pathogenesis of circumscribed scleroderma].

The studies were performed in 48 patients with morphea and included evaluation of 1) antinuclear antibodies 2) lymphocyte induced angiogenesis 3) natural killer (NK) cell activity and 4) T cell subpopulations in peripheral blood. The presence of antinuclear antibodies was found in 44.4% (8/18) patients with scleroderma linearis and in 21% (4/19) patients with morphea disseminata. Lymphocyte induced angiogenesis was increased in 41.5% (17/41) morphea patients, mainly in cases with pronounced vascular changes. The E rosette forming test showed a decreased percentage of active rosette forming cells (ARFC) and total rosette forming cells (TRFC) in peripheral blood and the NK cell activity was lowered in patients with morphea. These results obtained in patients with morphea show some similarities and differences in comparison to cellular immunity disturbances in patients with systemic scleroderma.

[Studies of the mechanism of disorders of the natural killer cell activity in patients with systemic scleroderma].

The NK activity of peripheral blood mononuclear cells (PBMC) was evaluated in 90 patients with various subsets of systemic scleroderma (SSc). The NK activity, as performed with K-562 as target cell, was found to be significantly lowered in patients with diffuse scleroderma, but did not differ from the healthy control in patients with acrosclerosis. The lowest values in the NK activity assay were obtained in patients with most extensive skin involvement and severe internal organ changes. The NK activity of healthy donors' PBMC was significantly decreased by addition of SSc patients PBMC (50:1) to the cytotoxicity assay, but was not influenced by the patients sera.
