Clinical results of a split sample liquid-based cytology (ThinPrep) study of 4,322 patients in a Turkish institution.

PURPOSE: A prospective study was carried out to compare the efficacy of liquid-based cytology (ThinPrep) with the conventional Pap smear using a split-sample design in a Turkish university hospital outpatient gynecology clinic. METHODS: 4,322 consecutive patients were recruited for the study between 2002 and 2003. All the patients underwent conventional Pap tests followed by a ThinPrep test for screening of cervical cancer. The results were evaluated in terms of the Bethesda III classification. All the patients with abnormal tests underwent colposcopy and directed biopsy. RESULTS: While 2.3% of the specimens were unsatisfactory for evaluation in the conventional Pap test group, this rate was 1.7% for the ThinPrep group. Epithelial cell abnormalities were observed in 42 (1.0%) patients in the conventional Pap test group and in 36 (0.8%) patients in the ThinPrep group. ASCUS was observed in 26 patients in the conventional Pap test group whereas the ThinPrep group had 20 cases of ASCUS as the leading cause of abnormal cytology. Biopsy of these cases revealed CIN 1 in two CIN 2-3 in three and cervical/endometrial adenocarcinoma in three patients. The ThinPrep application led to diagnoses of one additional case of CIN 2-3 and one case of adenocarcinoma among the negative or unsatisfactory for evaluation categories of the conventional Pap test group. CONCLUSION: Despite an adverse bias introduced by the split-sample study design, application of ThinPrep showed an improved rate of specimen adequacy and increased sensitivity for more significant cervical precursor lesions over the conventional Pap test.

Comparison of sucrose and human milk on pain response in newborns.

UNLABELLED: The aim of this study was to compare the analgesic effect of 2 ml 25% sucrose and human milk in a group of healthy term newborns. Healthy infants (n = 102) were randomly allocated to receive one of three solutions (sucrose, human milk, sterile water) 2 min prior to taking a heel prick blood sample. The median values of crying time, recovery time and percentage change in heart rate at 1, 2 and 3 min were recorded in response to the heel prick. Median crying times were 36, 52, and 62 s in the sucrose, placebo and human milk groups, respectively (P = 0.0009). In the sucrose group, there was a significant reduction in crying time compared to human milk and placebo groups. Similarly, the median recovery time in the sucrose group (72 s) was shorter than that in the human milk (112 s) and placebo groups (124 s) (P = 0.004). The percentage change in heart rate at 1, 2 and 3 min was also significantly lower in the sucrose group (P = 0.008, P = 0.01, P = 0.002 at 1, 2, and 3 min respectively). CONCLUSION: The orosensorial antinociceptive effect of human milk is not as effective as an analgesic as a 25% sucrose solution.

Transcranial Doppler assessment of cerebral blood flow velocity in term newborns.

Using the transcranial Doppler technique to assess postnatal changes in cerebral blood flow velocity, we studied the anterior cerebral artery, middle cerebral artery, and internal carotid artery of 31 healthy, term newborn infants. Normative values for the 1st, 3rd, and 5th days of life were determined. Cerebral blood flow velocity values in all three arteries examined correlated well with each other and we observed a statistically significant increase only in middle cerebral artery blood flow velocity between the 1st and 3rd, and 1st and 5th postnatal days. Thus, if we assume that flow velocities in various cerebral arteries undergo similar change, only one representative artery need be examined. The middle cerebral artery appears to be the vessel of choice. This choice simplifies the recording procedure, particularly in repeated examinations.

Flow cytometric DNA studies and AgNOR counts in patients with transitional cell carcinoma of the bladder.

Flow cytometry appears to be a promising diagnostic method which may influence the therapeutic approach to transitional cell carcinoma (TCC) of the bladder. The number of silver-stained nuclear organizer regions (AgNORs) seems to correlate with the growth fractions of the cells. In this study, we report the results of combined flow cytometric analysis and AgNOR in 37 patients with TCC of the bladder. A positive correlation was observed in the histological grade, stage and growth pattern in relation to the ploidy of tumors. There were statistically significant differences among the mean AgNOR counts of the different groups as defined by DNA content, histological grade, growth pattern and disease outcome. In different stage groups, the AgNOR counts were related both to recurrence and progression. It was concluded that AgNOR counts performed on routine formalin-fixed paraffin sections furnish significant kinetic information. According to our preliminary results AgNOR counts and the DNA content of the tumors should also be measured to decide on more aggressive treatment in some cases.

[Direct antigen detection for group A streptococcal pharyngitis: comparison of throat cultures and the direct antigen test].

In this study, traditional throat culture results and direct antigen detection from throat swabs have been compared with respect to diagnosis of Group A Beta hemolytic streptococcal infections in children with acute pharyngitis who admitted to Marmara University Hospital Pediatrics Department. It has been found that direct antigen test has a sensitivity of 92% and specificity of 98%.

Spontaneous cytotoxicity against measles virus infected and K562 cells in normal subjects and subacute sclerosing panencephalitis.

Spontaneous cytotoxicity towards LU106 (heteroploid human lung cell line) cells persistently and acutely infected with measles virus was studied in measles immune and nonimmune healthy subjects and SSPE patients. K562 (myeloid cell line) cells were also used to detect the NK activity. Specific cytotoxic responses for measles immune and nonimmune donors were not significantly different for both persistently and acutely measles infected LU106 cells indicating that our assay system detected NK activity. Cytotoxic responses of SSPE patients against virus infected and K562 cells were found to be within normal limits in 3 and 5 patients respectively.
