A provocative internal carotid artery balloon occlusion test with 99mTc-HM-PAO CBF mapping--report of three cases.

The balloon occlusion test (BOT) of the internal carotid artery (ICA), combined with induced hypotension and cerebral blood flow (CBF) mapping, was carried out in three patients with a large or giant aneurysm of the ICA. Occlusion of the ICA for 10 minutes in the normotensive state was followed by 5 minutes of induced hypotension. During the last 2 minutes of hypotensive occlusion, technetium-99m-hexamethyl-propyleneamine oxime was administered to study the CBF. All patients tolerated the procedure well. One patient with moderate CBF reduction developed ischemic complications 24 hours after permanent ICA occlusion. Another showed no significant change in CBF and tolerated permanent ICA occlusion well, while the third refused permanent occlusion. The provocative BOT combined with CBF mapping is a promising predictor of complications of ICA occlusion secondary to perfusion abnormalities.

Effect of aminoguanidine on functional and structural abnormalities in peripheral nerve of STZ-induced diabetic rats.

Nonenzymatic glycosylation of structural proteins and the formation of advanced glycosylation end products (AGEs) have been involved in the pathogenesis of diabetic complications. We examined the effect of aminoguanidine, a potent inhibitor of AGE formation, on functional and structural abnormalities in peripheral nerve of streptozocin-induced diabetic rats. Diabetic rats were treated with daily injections of 25 mg/kg body wt s.c. aminoguanidine (AG)-sulfate for 16 wk and compared to untreated diabetic rats. AG treatment improved motor nerve conduction velocity in 12- and 16-wk diabetic rats despite no changes in body weight, blood glucose, and HbAIc levels. AG treatment inhibited an accumulation of fluorescent AGE in diabetic nerves, and morphometric analysis of the sural nerve showed a partial effect on myelinated fiber size and axonal atrophy. These findings suggest that AG may have a beneficial effect on diabetic neuropathy and nonenzymatic glycosylation of peripheral nerve proteins.

Mechanism of anti-human immunodeficiency virus action of polyoxometalates, a class of broad-spectrum antiviral agents.

Various polyoxometalates proved inhibitory to the replication of a number of enveloped DNA and RNA viruses, i.e., herpesviruses (herpes simplex and cytomegalo), togaviruses (Sindbis), paramyxoviruses (respiratory syncytial), rhabdoviruses (vesicular stomatitis), arenaviruses (Junin and Tacaribe), and retroviruses [human immunodeficiency virus type 1 (HIV-1) and type 2 (HIV-2), simian immunodeficiency virus, and murine sarcoma virus]. The most potent compounds, i.e., JM1590 [K13[Ce(SiW11O39)2]. 26H2O] and JM2766 [K6[BGa(H2O)W11O39]. 15H2O], inhibited HIV-1 and simian immunodeficiency virus at concentrations as low as 0.008-0.8 microM. The polyoxometalates also inhibited giant cell formation in co-cultures of HIV-infected HUT-78 cells and uninfected MOLT-4 cells. Studies designed to unravel the mechanism of action of these compounds revealed that they inhibit the reverse transcriptase activity associated with HIV. The polyoxometalates also proved inhibitory to the binding of HIV-1 virions to the cells. From "time of addition" experiments, whereby the polyoxometalates were added at different times after virus infection, their mechanism of anti-HIV action could be attributed to inhibition of virus-cell binding. There was a good correlation (r = 0.84) between the inhibitory effects of the compounds on HIV-1-induced cytopathicity and their inhibitory effects on syncytium formation and a close correlation (r = 0.902) between their inhibitory effects on syncytium formation and their interaction with gp120, whereas there was no correlation between their anti-HIV-1 activity and their inhibitory effects on HIV-1 reverse transcriptase. In flow cytometric studies, the compounds did not interfere with the binding of OKT4A/Leu-3a monoclonal antibody to the CD4 receptor of uninfected cells, but they inhibited binding of anti-gp120 monoclonal antibody to HIV-1-infected cells. Thus, the binding of the polyoxometalates to the viral envelope glycoprotein gp120 is responsible for their anti-HIV activity.

Chemotherapeutic approaches to human immunodeficiency virus infections.

Since the discovery of human immunodeficiency virus (HIV) as the causative agent of acquired immune deficiency syndrome (AIDS), various attempts have been made to control this fatal disorder. In the replicative cycle of HIV, several steps have been identified as attractive targets for antiviral chemotherapy. Sulfated polysaccharides can block the virion binding to the CD4 receptor. 2',3'-Dideoxynucleosides including 3'-azido-3'-deoxythymidine (AZT) act as potent inhibitors of reverse transcriptase after intracellular phosphorylation. Only AZT and 2',3'-dideoxyinosine are available so far for the treatment of AIDS and AIDS-related complex. Non-nucleoside reverse transcriptase inhibitors of HIV-1 and viral protease inhibitors are the new classes of compounds that are now extensively studied. These compounds may add a new dimension to the prospects of anti-AIDS chemotherapy.

[Pulmonary function and exercise tolerance in patients treated with bone marrow transplantation (BMT)].

We assessed pulmonary function and exercise tolerance in 10 BMT patients. Their underlying disorders were as follows; chronic myeloid leukemia 5 cases, acute lymphoblastic leukemia 2 cases, aplastic anemia, acute myeloid leukemia and non-Hodgkin's lymphoma one case each. Their mean age was 26 +/- 9 years old. When the patients were healthy and free of serious complications and anemia, arterial blood gas examination, pulmonary function tests and incremental treadmill exercise test were examined repeatedly. Although %VC and FEV1.0% kept within normal range, PaO2 at rest, %DLCO, VO2max, VO2max/kg and O2-pulsemax remained low at one year after BMT. There were significant correlations between VO2max and O2-pulsemax [r = 0.955 (p < 0.001)], %VC [r = 0.758 (p < 0.02)], VE/VO2max [r = -0.749 (p < 0.02)] and delta SaO2/VO2/kg [r = -0.731 (p < 0.02)], suggesting that exercise intolerance in BMT patients may be based on both cardiac and gas exchange abnormalities. To evaluate cardiac dysfunction, we compared exercise parameters obtained at an exercise level of 75% predicted heart rate max in five age-matched normal subjects to those in six BMT patients who did not demonstrate desaturation during exercise. As a result, the mean values of VO2max/kg and O2-pulse/m2 in BMT patients were significantly lower than those in normal subjects, suggesting that cardiac dysfunction may be due to insufficiency of stroke volume during exercise. It is concluded that exercise intolerance in BMT patients may be mainly due to cardiac dysfunction.

[High-dose intravenous immunoglobulin treatment in chronic inflammatory demyelinating polyneuropathy].

We treated two patients with chronic inflammatory demyelinating polyneuropathy (CIDP) with high-dose intravenous immunoglobulin (HIG). The patients received 400 mg/kg of immunoglobulin a day for five days. One patient, who had failed to respond to prednisolone before, was treated with HIG, 18 months after the onset. His motor symptoms resolved immediately after the commencement of HIG. Electrophysiologically, the compound muscle action potentials increased in amplitude in all nerves examined and F wave reappeared in the left median nerve. The electrophysiological changes were compatible with improvement of conduction blocks. This patient had headache and exanthema during the HIG therapy, but they settled after cessation of the infusion. The other patient was administered HIG as an initial treatment, four months after the onset. HIG was of no effect in this case, but he showed remarkable recovery during the following prednisolone therapy. Although corticosteroid therapy is the first choice for CIDP, there are CIDP patients who do not respond to steroid or can not complete the steroid therapy because of adverse effects. HIG is an expectative and recommendable treatment for the steroid resistant CIDP patients.

[Recent advances of surgical treatment in elderly lung cancer patients].

A total of 1,291 resected cases with primary lung cancers were used to demonstrate the differences of the result of surgical treatment between elderly cases 70 years old or more and non-elderly cases, and the results obtained herein were as follows: 1) Indication for surgery in elderly cases is considered to be similar with non-elderly cases, however, definite mediastinal lymphnode metastasis should be excluded. 2) Postoperative respiratory complications, which were often fatal, were significantly increasing in incidence with the advance of age. Per cent FVC, FEV1.0, SK/SD delayed type cutaneous hypersensitivity, operative bleeding volume and operative time are considered to be valuable predicted parameters. 3) Survival curve in elderly cases were significantly worse than that in non-elderly cases, however, there were no significant differences of survival rates between these 2 groups when cases dead within 3 months postoperatively were excluded. 4) Transfer factor is considered to be effective as an adjunct to surgical treatment in elderly cases. In conclusion, survival curve in elderly cases can be improved by deminishing respiratory complications and maintaining general condition with immunotherapeutics for long postoperative period.

Adenocarcinoma

Growth behavior of low passage cultured cells derived from human non-small cell lung carcinoma in deepithelialized rat tracheas transplanted into nude mice.

Proliferation behavior of six low passage cell lines derived from human non-small cell lung carcinoma was investigated both by cell inoculation into deepithelialized Fisher 344 rat tracheas xenotransplanted into nude mice and by direct cell injection into the subcutis of nude mice. In the tracheal transplants, all the cell lines showed a comparable or higher degree of differentiation than in the primary lesion, while most cell lines showed a lower degree of differentiation in the mouse subcutis. The surfaces of the tracheal transplants repopulated with the cells derived from squamous cell carcinomas were initially lined with a layer of flattened cells, two to three cells thick, while those repopulated with the cells from the three adenocarcinoma cases (HKT-3, HKT-5, HKT-6) were initially lined with simple epithelia comprising cuboidal-, round-and columnar-shaped cells, respectively. Subsequently, the cells from HKT-3 increased in height and proliferated in a pseudostratified pattern, those from HKT-5 proliferated in a papillary growth pattern and those from HKT-6 proliferated in a cribriform pattern. Ultrastructurally, they resembled ciliated bronchial epithelial cells, non-ciliated bronchiolar epithelial cells and bronchial goblet cells, respectively. Histological examination of cultured cells using this system was considered to be advantageous in the evaluation of differentiation since it provided unique proliferation patterns unobserved in primary tumors or tumors formed after s.c. direct cell injections.

Adenocarcinoma

The role of natural killer cells and lymphokine activated killer cells in the pathogenesis of hepatic injury in hepatitis A [corrected].

In order to clarify the mechanism of liver damage in hepatitis A, we studied the role of Natural Killer (NK) cells and Lymphokine Activated Killer (LAK) cells in non-specific immunological reactions using hepatitis A (HAV) infected cells (JTC-12.P3 cell) using the 51Cr release assay. No significant difference in specific cytotoxicity was observed between uninfected cells (JTC) and HAV-infected cells (JTC-HAV) to fresh and Poly I:Cor rIL-2 pretreated peripheral blood mononuclear cells (PBMC) from healthy donors or patients with acute hepatitis A. But in an experiment employing fresh and rIL-2 pretreated PBMC from cynomolgus monkey as effectors, a significant difference in NK and LAK sensitivity between JTC and JTC-HAV was noticed. Cytotoxicity assay was then carried out using as effectors fresh or rIL-2 pretreated cells of B, NK and T cell fractions obtained after separation of PBMC from monkey by the E-rosette formation method. Both fresh and rIL-2 pretreated B/NK cells showed significantly higher cytotoxicity for JTC-HAV than JTC, but neither fresh or rIL-2 pretreated T cells showed cytotoxicity for JTC-HAV or JTC. These results imply that both NK and LAK cells may play a part in the mechanism of hepatocellular injury.

Acute Disease

Common features in the interaction of tetrahydroimidazo[4,5,1-jk][1,4]benzodiazepin-2(1H)-one and -thione and 1-[(2-hydroxyethoxy)methyl]-6-(phenylthio)thymine derivatives with the human immunodeficiency virus type 1 reverse transcriptase.

Recently, several classes of compounds have been shown to be potent, selective, and specific inhibitors of human immunodeficiency virus type 1 replication in vitro. These include the tetrahydroimidazo[4,5,1-jk][1,4]-benzodiazepin-2(1H)-one and -thione (TIBO) and the 1-[(2-hydroxyethoxy)methyl]-6-(phenylthio)thymine (HEPT) derivatives. Both the TIBO and HEPT derivatives specifically inhibit human immunodeficiency virus type 1 reverse transcriptase (RT). From a comparative study of the characteristics of RT inhibition by TIBO and HEPT, and from the competition between TIBO and HEPT for RT inhibition, we infer that both classes of compounds, although structurally unrelated, are targeted at the same site of the enzyme. Detailed functional and kinetic analyses indicate that this target site is functionally and possibly also spatially related to the substrate binding site.

Antiviral Agents

[Evaluation of nutritional status in patients with esophageal cancer by measurement of sternocleidomastoid, rectus abdominis and quadriceps muscles on ultrasound imaging].

To assess the nutritional status of the patients with esophageal cancer, we measured the areas of sternocleidomastoid and rectus abdominis muscles (SMA, RAMA) on real time ultrasound imaging and calculated muscle index (MI = (SMA+RAMA/Height)). Eighty-three patients with esophageal cancer were included in the present study. Preoperatively, significant Spearman's coefficients were found between MI and the percentage of standard arm circumference (R = 0.52) and between MI and the percentage of standard arm muscle circumference (R = 0.51). Postoperatively, patients with wide muscle area showed high values of prealbumin, transferrin and fibronectin. To study the changes after surgery, we also measured the area of quadriceps muscle in patients with esophageal cancer. The areas of quadriceps muscle in patients with dysphagia were much diminished than in those without dysphagia. Measurement of areas of muscles such as SMA, RAMA and quadriceps muscle was proved to be a useful procedure in evaluating the nutritional status of patients with esophageal cancer.

Abdominal Muscles

[Chronic inflammatory demyelinating polyneuropathy--an electrophysiological and histological study of the ulnar nerve from a case with intractable persistent conduction block].

We report a 71-year-old male with CIDP, in whom complete motor conduction block persisted for more than several years. Corticosteroid and plasma therapy showed little effect to his weakness. He died of pneumonia. The ulnar nerve in which complete conduction block had persisted was taken out immediately after death and studied with conventional histological and morphometrical techniques. In the transverse section, many thinly-myelinated fibers and some cluster formations were confirmed. Active axonal degeneration was scarce. Myelinated fiber density was 7,417/sgmm. Fiber diameter histogram showed a two-peaked normal distribution. It is thus concluded that demyelination with little axonal degeneration can be the main pathological process causing intractable weakness and poor prognosis of CIDP. Low CMAPs should not lead to therapeutic nihilism, because it may simply be caused by demyelination without exonal degeneration in CIDP.

[Histopathological features of the lymph node metastases in patients with thoracic esophageal cancer].

Histopathological features of the lymph node involvement were studied in 104 patients with thoracic esophageal cancer who underwent subtotal esophagectomy combined with extended radical lymph adenectomy in cervicothoracoabdominal region. Metastatic involvement was found in a total number of 503 lymph nodes from 73 patients by histologic examination. The mean of long and short diameter was found to be less than 5mm in 125 (24.9%) of these 503 nodes. The involved area on the section was less than one third in 149 nodes (29.6%), and was significantly smaller in mediastinal lymph nodes than those in cervical or abdominal ones. Sixty-seven (13.3%) of 503 nodes were partially invaded by micrometastasis of 1mm or less in diameter. Micrometastasis also more frequently occurred in mediastinal nodes with a statistically significant difference. Extranodal proliferation (ENP) of cancer cells was found in 106 nodes (21.1%), and extranodal lymphatic and/or blood vessel invasion (ENly, v) was also recognized in 60 nodes (11.9%). Micrometastasis and ENP with or without ENly, v were found in 24 (32.9%) and 29 (39.7%) of 73 patients with positive lymph node metastasis, respectively. Postoperative survival rate in patients with micrometastasis and/or ENP with or without ENly, v was inferior to that in patients with neither of them.

Esophageal Neoplasms

[Evaluation of Oriton IgE, a new kit for measurement of allergen specific IgE antibodies].

To determine whether Oriton IgE kit, a new kit for the measurement of allergen-specific IgE antibodies, is useful in screening allergen-specific antibody, we measured the titers of IgE antibodies against 11 different allergens (house dust 2, Dermatophagoides farinae, Japanese cedar, ragweed pollen, egg white, milk, cat epithelium, dog epithelium, Candida, Alternaria and Aspergillus) with the Oriton IgE kit, and the results were compared to those of intradermal tests and RAST in 103 allergic patients and 10 normal subjects. There was a clear correlation between IgE antibody titers measured by the Oriton IgE kit and the RAST. The correlation coefficient was 0.76 (p less than 0.01) and the total correspondence rate was 85.9%. We also found strong correlation between the Oriton IgE kit and RAST in IgE antibody titer against 5 different allergens, Dermatophagoides farinae, Japanese cedar, ragweed pollen, egg white and Candida. The correlation coefficient was over 0.70. The correspondence rate, sensitivity and specificity of the Oriton IgE kit to intradermal tests was 71.8%, 45.3% and 87.8% respectively. The sensitivity of the Oriton IgE kit was slightly higher, while the specificity was slightly higher in RAST, although the differences were not statistically significant between these methods. Correspondence rate of the Oriton IgE kit was similar to that of RAST. These results suggested that the Oriton IgE kit is useful in screening allergen specific IgE antibodies.

[A rapid measuring technique for allergen-induced IL2 responsiveness of lymphocytes by the propidium iodide-staining method. Detection of the etiological antigen in patients with allergic diseases].

A method for rapidly measuring the interleukin 2 (IL-2)-responsiveness of allergen-stimulated lymphocytes has been newly developed using propidium iodide (PI) staining and drawing ink quenching of the fluid medium fluorescence. There was a linear correlation between the number of PI-stained cells and the fluorescence intensity. The background was less than 5 percent. This fluorochromasia assay reflected the cell number for the quantitative measurement of lymphocyte proliferation. Antigen-activated patient cells added to IL2, showed greater increase in 3H-TdR uptake than IL-2-untreated cells, and were capable of acquiring IL2 responsiveness. Increased numbers of the cells were observed by both the PI- and trypan blue-staining methods. In contrast, unstimulated cells also showed increased 3H-TdR uptake response without increased cell numbers on stimulation with r-IL2 by a 6.8 to 19.3 fold stimulation index compared to the antigen-activated cells. The results indicated that the unstimulated cells in addition with r-IL2 still remained in the initial phase of the DNA-synthetic (S) period through the cell cycle, whereas the activated cells had passed through the post-synthetic gap (C2) and/or cell division at mitosis (M). 3H-TdR uptake of cultured cells usually demonstrates the presence of antigen-sensitized lymphocytes by in vitro proliferative response, and shows an increased stimulation index in 4 or 7 day cultured cells on stimulation with allergens. However, proliferation of some cell populations decreased; furthermore no distinct differences in cell proliferation between allergic and normal lymphocytes were observed, as is usual in this assay, although the present method was capable of measuring increased-IL2 responsiveness of patient lymphocytes. The results indicate that the 3H-TdR uptake method can not be substituted for cell enumeration in the evaluation of the antigen-specificity of induced-IL2 responsiveness of activated cells. Therefore, cell enumeration using the PI-staining method may be preferable in this system. The induced response was observed in lymphocytes from patients with atopic dermatitis, bronchial asthma and/or allergic rhinitis specifically on stimulation with the antigen causing clinical symptoms. The results obtained using the PI-staining method were very similar to those in previous reports where the trypan blue staining method was employed. The present method appears to be capable of rapidly screening etiological antigens for disease and easily monitoring clinical activity.

Allergens

[Classification of allergens by positive percentage agreement and cluster analysis based on specific IgE antibodies in asthmatic children].

Classification and characterization of allergens is important because allergic patients are sensitized by a variety of allergens. One hundred and sixty-one sera from asthmatic children were investigated for specific IgE antibodies against 35 allergens including 20 inhalants and 15 foods by means of the MAST method. We assessed the allergenic properties of the allergens based on positive percentage agreement and cluster analysis. There was a high positive percentage agreement of specific IgE antibodies between house dust and Dermatophagoides spp., a relatively high agreement between 5 molds, cat and dog epithelium, mugwort and wormwood and 5 grasses. Among the food allergens, the positive percentage agreements were relatively high, especially between cow's milk, casein, cheese, and between 3 cereal grains. In the cluster analysis, house dust and Dermatophagoides spp. made a big cluster; therefore 32 allergens except house dust and mites were analyzed. From the results of the cluster analysis, the major cluster consisted of (1) ragweed, (2) mugwort and wormwood, (3) timothy, sweet vernal, velvet and cultivated rye, (4) wheat, barley and rice, (5) molds, (6) cow's milk, casein, soybean and cheese, (7) shrimp and crab, (8) egg white, (9) Japanese cedar, (10) dog epithelium, (11) cat epithelium. The cluster of grass pollens and cereal grains made one cluster. These results tend to confirm the presence of species cross-reactivities within the major classes of allergens.

[The relationship between reactivity and asthmatic attacks before and after acetylcholine inhalation test].

Sixty two asthmatic children participated in this study to clarify the relationship between the number of the days of asthmatic attack and the respiratory threshold of acetylcholine (RT-Ach) and % fall of FEV1.0 at the RT-Ach. In the age group 5 to 11, there was a statistically significant correlation between the RT-Ach and % fall of FEV1.0 at RT-Ach. However there was no such relationship in the age group 12 to 15. In the age group 5 to 11, there was a statistically significant correlation between the % fall of FEV1.0 and the number of the days of asthmatic attack during 10 days and one month before the inhalation test. However there was no such relationship in the age group 12 to 15. There was a statistically significant correlation between the % fall and the number of the days of asthmatic attack over 12 months following the inhalation test in the age group 12 to 15. There was no such relationship in the age group 5 to 11.
