[An inverse course of Darier dyskeratosis follicularis].

A 56-year-old female patient had suffered from isolated, unclassified palmoplantar keratosis since childhood. During the climacteric horny papules appeared for the first time on the extremities and trunk. Histologically, the efflorescences on the body and the palmoplantar keratosis were classifiable as keratosis follicularis. Predilection sites of Darier's disease were spared. Altogether, the usual pattern of involvement was reversed.

[Perforating cutaneous amyloidosis].

Two patients, a 79-year-old man suffering from familial lichen amyloidosus and a 44-year-old women with biphasic cutaneous amyloidosis, developed small, centrally ulcerated papules. Histological and ultrastructural examination confirmed transepidermal elimination of amyloid. In contrast to other transepidermal elimination diseases, the material eliminated in perforating cutaneous amyloidosis is of epidermal origin.

[Ichthyosis and alopecia after maprotiline: corneolysis caused by temporary disorder of keratinization].

A 37-year-old woman developed ichthyosiform desquamation of the skin and a severe diffuse alopoecia 3 weeks after taking the antidepressant maprotilin. No signs of inflammation were present. Histology revealed acanthosis with preserved stratum granulosum, follicular hyperkeratosis and dystrophic changes of the hair follicle. Electron microscopy revealed rarefication of tonofilaments and necrobiotic changes of epidermal keratinocytes with vacuolar degeneration of the cytoplasm and disorganization of the organelles. Pathogenetically this disease represents a drug-induced transitory disorder to keratinization, which had resulted in desquamation of the stratum corneum and alopecia. The authors propose the designation corneolysis for this pathogenetic principle.

[Mibelli's porokeratosis gigantea].

This case report deals with the giant form of porokeratosis of Mibelli, porokeratosis Mibelli gigantea. Over the last 40 years, the 55-year-old man concerned had observed a centrifugal expansion of a single hyperkeratotic lesion until it covered the whole of the right hand and forearm. Besides the characteristic histological alterations, we saw capillaries with unusually thickened walls owing to basal lamina multiplication. In the centre of the lesion atrophy of the epidermis and of the corium was visible. These observations indicate an alteration in two germ layers in porokeratosis.

[Alpha-MSH and pregnancy].

In 32 pregnant women, we repeatedly determined the levels of alpha-MSH, melatonin, ACTH and progesterone. In the radioimmunoassay, all of these hormones showed a significant increase from the first to the third trimester. In contrast to progesterone, neither the level of alpha-MSH nor that of melatonin rises significantly until between the 28th and 36th week of pregnancy. The authors suggest that the increasing pigmentation may be due to the rising levels of alpha-MSH, while the preferential hyperpigmentation of sexual areas in additionally mediated by sex hormones.

[Pagetoid reticulosis of the Ketron-Goodman type].

The case of a 79-year-old woman with widespread pagetoid reticulosis (Ketron-Goodman type) is presented. The nature of the epidermal cellular infiltrate in pagetoid reticulosis of the Ketron-Goodman type was examined by immunohistochemical methods and by electron microscopy. On ultrastructural examination the infiltrating cells proved to be medium-sized and large Sézary-type and blastoid cells. Immunohistochemically, the epidermal infiltrate was composed of approximately 80% immature T cells (OKT 16) and 30% proliferating cells (Ki 67). Mature T cells (CD 5) were found to account for only 15-30% of all cells in the infiltrate. These results suggest that pagetoid reticulosis (Ketron-Goodman type) is a malignant T-cell lymphoma, which is characterized, in addition to the epidermotropic behavior common to all T-cell lymphomas, by innidiation and in-situ spread within the epidermis.

Pustulosis palmoplantaris. How initial ultrastructural alterations change into vesicles and pustules.

The diagnosis pustulosis palmoplantaris describes a clinical aspect. Apart from the pustules, a different number of dyshidrotic vesicles characterize the clinical morphology. This study was carried out to elucidate a facultative relationship between vesicles and pustules. The first ultrastructural changes are small intercellular gaps in the basal layers of the epidermis containing one Langerhans' cell. Larger intraepidermal vesicles are filled with stimulated lymphocytes and interdigitating Langerhans' cells. Below the vesicle a dense infiltrate consisting predominantly of CD 8-positive cells was observed. Mature pustules are situated just beneath the stratum corneum and filled with a lot of granulocytes and only few lymphocytes. The subepidermal infiltrate is almost missing. Between vesicles and neighbouring pustules we could prove a direct connection below the stratum corneum, a direct subcorneal granulocyte pathway. The result is a vesicle with lymphocytes at the bottom and granulocytes at the top of its cavity--a transformed vesicle. The described intercellular cavities are considered as common evolutionary stages in the development of pustules and vesicles.

Combined treatment with calcipotriol ointment and low-dose ultraviolet A1 phototherapy in childhood morphea.

Various therapies for morphea have been used with limited success, including ones with potentially hazardous side effects. When morphea occurs in childhood it may lead to progressive and long-lasting induration of the skin and subcutaneous tissue, growth retardation, and muscle atrophy. We report an open prospective study in which the efficacy of a combined treatment with calcipotriol ointment and low-dose ultraviolet A1 (UVA1) phototherapy in childhood morphea was investigated. Nineteen children (mean age 8.5 years, range 3-13 years) with morphea were exposed to UVA1 (340-400 nm) phototherapy at a dose of 20 J/cm(2) four times a week for 10 weeks. Forty phototherapy sessions resulted in a cumulative dose of 800 J/cm(2) UVA1. In addition, calcipotriol ointment (0.005%) was applied twice a day. After 10 weeks, palpation and inspection showed a remarkable softening and repigmentation of formerly affected skin resulting in a highly significant (p < 0.001) decrease of the mean clinical score from 7.3 +/- 0.9 at the beginning to 2.4 +/- 0.9 (relative reduction 67.1%) at the end of combined therapy. Our results indicate that a combined therapy with calcipotriol ointment and low-dose UVA1 phototherapy is highly effective in childhood morphea. Further controlled studies are necessary to investigate whether this combined therapy is superior to UVA1 phototherapy alone.
