Comparative efficacy of rice-based and glucose-based oral rehydration salts plus early reintroduction of food.

The use of oral rehydration salts (ORS) to restore fluid balance in children with diarrhoea is universally accepted. However, there is uncertainty about whether glucose-based ORS or ORS based on precooked rice powder is more effective. In a randomised trial we compared the two types of ORS in children who were given food immediately after completion of rehydration. 460 boys aged 3-18 months, admitted to hospital with acute diarrhoea and signs of dehydration, were randomly assigned to groups receiving rice-based and glucose-based ORS solution (230 to each group). After full rehydration (4-12 h), a weaning food consisting of rice and mixed vegetables was given until the diarrhoea stopped. Continuing losses of liquid stool and vomitus were replaced with the assigned ORS solution. There were no differences between the groups during the rehydration phase in stool volume, volume of ORS solution taken, duration of rehydration phase, or weight gain. However, after initiation of feeding, the glucose-based ORS group had significantly lower stool volumes than the rice-based ORS group (142 [95% CI 117-173] vs 96 [77-120] g/kg); they also took a smaller amount of ORS solution (153 [127-185] vs 111 [90-136] mL/kg) and had a shorter duration of diarrhoea (55 [SD 35] vs 44 [35] h). Glucose-based ORS solution was more effective than rice-based ORS solution for the treatment of diarrhoea in children when feeding with a rice-based diet was started soon after correction of dehydration. These results support the continued recommendation of glucose-based ORS solution as standard therapy for treatment of children with acute diarrhoea and emphasize the importance of resuming feeding as soon as dehydration has been corrected.

[Chlamydia trachomatis IgA and IgG antibodies in prostitutes and women having spontaneous abortion].

In this study, the sera taken from 34 prostitutes, 34 women having spontaneous abortion and 34 control group were tested for C. trachomatis IgA and IgG antibodies. C. trachomatis antibodies were found positive in 30 (88.2%) of prostitutes, 11 (32.3%) of women having spontaneous abortion, 9 (26.4%) of controls. When the positivity rate of prostitutes and women having spontaneous abortion compared with control groups statistically, it was found significant in prostitutes (p < 0.05), but not found significant in women having spontaneous abortion (p > 0.05).

[The histopathological effects of different treatments on lung tissue of mice pretreated with cyclophosphamide and steroids in experimental E. coli sepsis].

The incidence of gram-negative bacteremia is significantly increasing in recent years by the wide-spread use of cytotoxic and immunosuppressive drugs. Although, the effective antimicrobial drugs are being used in treatment, the mortality rate is still high. In this study, we searched for the histopathological changes occurring on lung tissue in E. coli sepsis, and their severity in different models. Microscopically, all the specimens were examined by the presence of interstitial and peribronchiolar inflammation, congestive atelectasis, and emphysema. The differences between the ratios of histopathological changes in treatment subgroups were not statistically significant. However, the ratio of interstitial and peribronchiolar inflammation and emphysema was significantly decreased in mice received cyclophosphamide, when compared with control group. Besides, the ratio of peribronchiolar inflammation was significantly increased in mice received steroid when compared with control group.

[IgG therapy of experimental E. coli sepsis in neutropenic mice].

The treatment of Ceftriaxone and IgG in neutropenic (Group A), and in normal (Group B) mice were investigated in experimental E. coli sepsis. IgG was obtained from the rabbits immunized with same strain. The inoculum dose of infection was 4 x 10(8) bacteria/ml in the experiment. The two groups were divided into four subgroups; Control, IgG. Ceftriaxone and Ceftriaxone + IgG groups. The mortality rates of mice in both groups and bacteria growing in organ cultures are investigated and compared. IgG treatment reduced the mortality in the normal mice, but no significant difference was found between two groups. An addition of IgG to Ceftriaxone treatment significantly decreased the mortality rate in both of the groups (p less than 0.05). But a significant difference was not observed between two subgroups treated with IgG and Ceftriaxone.

[Antibiotic and specific immunoglobulin G in the treatment of E. coli sepsis in mice previously given steroids].

Experimental E. coli sepsis was constituted in two groups of mice. One of the groups have been immunosuppressed by steroid therapy of ten days (group A) and the other group was normal (group B). Ceftriaxone and Ig G, prepared by vaccinating rabbits with the same strain of E. coli, were used in therapy. The results of the therapy in both were compared. The mortality rate of mice on Ig G therapy was % 70 in group A and % 80 in group B and that on ceftriaxone therapy % 60 and % 50, respectively. The mortality rate was % 30 in both groups with Ig G + ceftriaxone therapy. There wasn't any significance between these. On this research, the treatment of antibiotic together with Ig G decreased the mortality rate. The statistical value of the mortality rate among the treatment groups was not found significant.

Treatment of tuberculous meningitis in adults with a combination of isoniazid, rifampicin and streptomycin: a prospective study.

15 patients with tuberculous meningitis were treated with isoniazid, streptomycin and rifampicin and 14 with isoniazid, streptomycin and ethambutol for 12 months. Both groups received prednisolone at the beginning of treatment. The two groups were compared with regard to clinical improvement, presence of neurological sequelae and mortality. No difference in recovery rate between the groups was observed. 6 patients (21%) died (5 in group I and 1 in group II). Residual sequelae developed in 9 cases (5 in group I and 4 in group II; 31%). The difference between the groups was not significant. The regimen including rifampicin for tuberculous meningitis did not result in any superiority compared to standard therapy.

A case of cutaneous anthrax with toxaemic shock.

We describe a patient who presented with a necrotic black eschar 2 x 2 cm on the neck, extensive erythema around the lesion and massive oedema extending from the lesion to the umbilicus, and involving the whole face. Severe toxaemia and shock developed. Bacillus anthracis was isolated from the lesion. Prednisolone and dopamine failed to reverse the shock. The patient recovered completely with penicillin therapy.

Circular dichroism conformational study of the chemotactic peptide formyl-L-methionyl-L-leucyl-L-phenylalanine.

The solution circular dichroism of the chemotactic agent formyl-L-methionyl-L-leucyl-L-phenylalanine was found to be highly solvent dependent. In relatively nonpolar solvents (fluoroalcohols) the circular dichroism is that characteristic of a folded conformation. In H2SO4 the tripeptide remains intact and displays a circular dichroism typical of highly solvated and totally disordered peptides. The order-to-disorder transition can be followed by addition of water to trifluoroethanol.

Single daily dose of digoxin for maintenance therapy of infants and children with cardiac disease: is it reliable?

Between July 1990 and September 1991, 30 infants and children, most of whom had a congenital heart defect and who had been treated at least during the previous 20 days by two daily doses of digoxin and were in a stable clinical condition, were selected at random. A maintenance dose of digoxin was administered at 24-h intervals for 7 days in the study group (n = 15); no change was made in the 12-h dosage interval in the control group (n = 15). When the serum digoxin concentrations were compared, no significant difference was found between pre- and poststudy values in the study group (1.0 +/- 0.6 and 0.8 +/- 0.3 ng/ml, respectively) or between the control and study groups (0.9 +/- 0.6 and 0.8 +/- 0.3 ng/ml, respectively) in terms of trough serum digoxin concentrations. Although the peak serum concentrations in the study group were increased significantly (2.3 +/- 0.8 ng/ml) compared with prestudy peak levels (1.6 +/- 0.7 ng/ml, p < 0.05) and with the level in the control group (1.5 +/- 0.8 ng/ml, p < 0.05), a toxic concentration was not reached, and toxicity symptoms were not observed clinically. Blood pressure, heart rate, and liver size did not change significantly in any patient during the study.

Serum TNF-alpha, sCD8 and sIL-2R levels in childhood tuberculosis.

In this study we have determined the serum tumor necrosis factor-alpha (TNF-alpha), soluble CD8 (sCD8) and soluble interleukin-2 receptor (sIL-2R) levels in children with active pulmonary tuberculosis (n = 66) and healthy controls (n = 20). Measurable serum TNF-alpha levels were detected in nine of 86 children (10.5%), all of whom belonged to the group with active disease. Serum sCD8 and sIL-2R determinations revealed a significant difference between the group with active pulmonary tuberculosis and the controls (p < 0.05). Deeper insight into the involvement of cytokines and T cells will provide a better understanding
