Hydatic cyst located intermuscular area of the forearm: MR imaging findings.

Hydatid cyst of the skeletal muscle is rare and can pose various diagnostic problems. The authors report the case of a patient with primary hydatid cysts localized to intermuscular area of the forearm, emphasizing the diagnostic value of MRI. To the best of our knowledge, this localization of the hydatic cyst has not been reported to date.

Ureteral displacement due to a migrated intrauterine contraceptive device.

Perforation of the uterus is rare but potentially fatal. During puerperium when the uterus is small and its wall is thin, the risk of perforation increases. We report a rare complication from an intrauterine contraceptive device (IUD) which caused deviation of the right ureter in a 31-year-old woman who presented with complaints of insomnia and abdominal pain. Our case shows that perforation of the uterus by an IUD can cause a silent urological complication. The possibility of ureteral involvement and displacement should be kept in mind in a woman in whom a missing IUD is encountered.

Penile epithelioid sarcoma: MR imaging findings.

Magnetic resonance imaging findings of a 38-year-old man with epithelioid sarcoma of the penis is presented. It started as a firm, painless and slowly growing nodule at the base of his penis 6 months previously which caused pain radiating to the testis during coitus. It has been well known that sarcomas may well mimic reactive processes. Initial presentation of epithelioid sarcoma may provoke considerable diagnostic difficulty, and its differentiation from benign lesions, such as Peyronie's disease and chronic inflammation, may be a clinical problem. In our present report the MR findings are compared with those of the epithelioid sarcomas of various locations reported in the literature and differential diagnosis of the entity is discussed. To our knowledge, this is the first report regarding the MR findings of the epithelioid sarcoma of penis.

The contribution of nitric oxide and endothelins to angiotensin: II. Evoked responses in the rat isolated uterus smooth muscle.

The present study was designed to determine the role of endogenous endothelin peptides and nitric oxide on angiotensin II (All) responses in the isolated nonpregnant rat uterine smooth muscle. AII (10, 20, or 50 ng/ml) increases rhythmic oscillations dose dependently (32.7 +/- 8.9, 55.96 +/- 10.3, and 62.78 +/- 17.7% increase, respectively). L-arginine methyl ester (L-NAME; 10(-5) M) did not affect the increase in rhythmic oscillations induced by All (10, 20, or 50 ng/ml) (17.5 +/- 12.1, 31.5 +/- 18.3, and 52.5 +/-11.8% increase, respectively, n = 6, p > 0.05). It reduced the contractile responses to AII (10 ng/ml: from 4.63 +/- 0.6 to 1.8 +/-0.7 cm2, p < 0.05: and 20 ng/ml: from 5.59 +/- 0.8 to 2.11 +/- 0.4 cm2, p < 0.05, n = 6). L-arginine (10 mM) decreased the contractile response obtained by AII (10 or 20 ng/ml) (1.93 +/- 1.05, p < 0.05 and 2.14 +/- 0.7 cm2, p < 0.05, respectively, n = 6). BQ 485 (50 ng/ml) decreased both the number of rhythmic oscillations and the contractility increased by AII. Bosentan (10(-5) M) induced an increase in the number of rhythmic oscillations but decreased the contractile responses to the higher concentrations of All. These data show that endogenous NO and endothelin peptides contribute to the motility changes induced by AII and may play an important role in the pathophysiological events of the uterine function.

Diagnostic value of biochemical markers of bone turnover and postmenopausal osteoporosis.

We studied 77 women divided into postmenopausal osteoporotic and premenopausal and postmenopausal non-osteoporotic groups in order to evaluate bone metabolism and diagnostic value of biochemical markers of bone turnover in postmenopausal osteoporosis. Postmenopausal osteoporotic (n: 40), postmenopausal non-osteoporotic (n: 24) and premenopausal non-osteoporotic (n: 13) groups were defined according to bone mineral density (BMD) scores obtained with dual energy X-ray absorptiometry (DEXA). Urinary deoxy-pyridinoline (Dpd), pyridinoline (Pyd), serum total alkaline phosphatase (ALP), bone specific alkaline phosphatase (BALP), osteocalcin (BGP), total calcium, phosphorus, and creatinine levels were determined. Urinary Dpd and Pyd levels of postmenopausal osteoporotic group (8.7 and 18.7 mumol/mg creatinine) were significantly higher than postmenopausal control (5.1 and 11.7 mumol/mg creatinine, p < 0.0001) and premenopausal control (6.0 and 13.0 mumol/mg creatinine, p < 0.0005 and p < 0.001) groups. Bone formation markers were not significantly different between groups, although BGP correlated with Dpd and Pyd (r: 0.26 and r: 0.31, p < 0.05) in osteoporotic subjects. From receiver operating curve (ROC) analysis Dpd had the best diagnostic value (0.846), followed by Pyd (0.802) in evaluation of osteoporosis, whereas BALP (0.570) and BGP (0.528) were relatively inefficient in the discrimination of postmenopausal osteoporosis. This study suggests that bone resorption markers are more efficient than bone formation markers in the diagnosis of postmenopausal osteoporosis. Urinary Dpd/creatinine ratio has the highest diagnostic value.

CT findings in pulmonary hydatid disease.

PURPOSE: The purpose of this study was to present the CT findings of complicated pulmonary hydatid cysts on the basis of the CT appearances. MATERIAL AND METHODS: Preoperative CT examinations of 19 cases with hydatid cysts were evaluated retrospectively for the number and various features of the cysts and secondary changes in adjacent structures. The results were compared with surgical findings. RESULTS: While 7 patients had multiple pulmonary hydatid cysts, 12 patients had solitary cyst. The total number of the cysts was 29. Eleven of them were intact cysts. Two of the 18 complicated cysts were only ruptured, while the remaining 16 complicated cysts were ruptured and infected. The endocyst was separated from a pericyst in a local area in one ruptured cyst. Air-bubbles were seen within 14 infected cysts. Nine of the infected cysts had characteristic appearances of hydatid disease. The other 2 infected cysts showed soft tissue density. CONCLUSION: CT can provide the specific diagnosis of pulmonary hydatid cysts by demonstrating the number and features of the cysts, characteristic appearances of complicated cysts, and secondary changes in adjacent structures. Whether there are characteristic signs or not, the demonstration of air-bubbles within the cyst, together with ring enhancement, are strong indicators for infected hydatid cysts.

[Notes on the medicine of the anatolian seljuk period].

In this study some remarkable medical practices during the period of Anatolian Seljuks are related. We see a widespread use of poisonous snakes as medicine. Ahi Evren Seyh Nasirud-din Mahmud, the leader of the Ahi organization which was a guild of artisans and traders of the period, explains in his book that he produced an antidote from poisonous snakes. Ahi Evren was the most famous doctor of the period. He complied a work on anatomy. He used to go to the mountains in the winter and utilized snake poison for preparing medicine. During the period of Anatolian Seljuk, in almost all the cities there were hospitals as health centers. At the time it was too difficult to bring patients from the village to the city; therefore the doctors of the health centers travelled from village to village, from town to town to treat the patient. The statesmen paid doctors for treating patients in certain regions. During this period, it was assumed that artificial fertilization could be made outside the womb. If the problem of access to the sources of this period is solved, there may be found interesting information on this subject.

Automatic analysis of the control of metabolic networks.

In this paper we apply computer algebra techniques to analyze the control of metabolic networks. For this purpose, a computer program based on metabolic control theory was developed. When a stoichiometry matrix of the metabolic networks is given, the program calculates all the control coefficients (flux and metabolic control coefficients, summation and connectivity relationships) using elasticity coefficients. The program can be applied to any metabolic network which includes unlimited steps and intermediate metabolites.

Ultrasound examination of scrotal masses.

The technique of renal time ultrasound of the scrotum is described. The examination was performed in 50 patients and 15 normal males at the Sahinbey Hospital. Given the clinical diagnosis ultrasound proved reliable in almost all cases. In this study ultrasonography of the scrotum has been found to be the most useful method demonstrating testicular diseases.

Using computer algebra to determine rate constants in biochemistry.

In earlier work we have described how computer algebra may be used to derive composite rate laws for complete systems of equations, using the mathematical technique of Gröbner Bases (Bennett, Davenport and Sauro, 1988). Such composite rate laws may then be fitted to experimental data to yield estimates of kinetic parameters. Recently we have been investigating the practical application of this methodology to the estimation of kinetic parameters for the closed two enzyme system of aspartate aminotransferase (AAT) and malate dehydrogenase (MDH) (Fisher 1990a; Fisher 1990b; Bennett and Fisher, 1990): aspartate + alpha-ketoglutamate <--> glutamate + oxaloacetate; oxaloacetate + NADH <--> malate + NAD. In this paper we present a fuller (although not yet complete) analysis of the system. We show how symbolic estimates of the error behaviour of the parameters can be made, and used to identify those which are of kinetic significance. Finally we consider how metabolic control analysis can be applied directly to such a system.

Inhibition of cholecystokinin-induced gallbladder contraction by atropine and pirenzepine in man.

The effect of pirenzepine on cholecystokinin (CCK)-stimulated gallbladder contraction in man was compared with that of atropine. Following pretreatment with atropine, pirenzepine or physiological saline in a random order, 6 healthy male volunteers were infused for 75 min with CCK-octapeptide in doses that are reported to mimic physiological plasma concentrations. Gallbladder volumes were determined by real-time ultrasonography. Pirenzepine inhibited CCK-induced gallbladder volume reduction significantly up to 30 min (p less than 0.01) and the magnitude of this inhibition was similar to that of atropine. The results demonstrate that M1 neural muscarinic receptors are involved in the anticholinergic inhibition of CCK-induced gallbladder contraction in man.

[Relation of genital tuberculosis and simple cysts of the ovary].

86 cases of genital tuberculosis were evaluated retrospectively by clinical, anatomical, laparoscopic and HSG findings. The authors observed in 24 cases out of 86 (27.9%) unilateral simple ovarian cyst. They conclude that genital tuberculosis must be suspected and investigated when simple ovarian cyst has been found in infertile women.

[Tescher's metroplasty and its results].

16 women with uterine septum, between 20 and 32 years old, have been operated by Tescher operation and the results are exposed. 5-6 months after the operations the control x-ray examination have been applied to thirteen cases. In 12 of them, we saw excellent physiologic cavities. In ne last one, there was an sufficient size cavity but it was not excellent. We suggest that the results of Tescher operation are better than those of others technics.

Unusual malformations in occult spinal dysraphism.

Two cases of occult spinal dysraphism with different clinical symptoms, signs and congenital pathologies are presented. One had malformations including scoliosis, dermoid tumor, hydromyelia, diastematomyelia, dermal sinus, low conus, vertebrae anomalies and dextrocardia. The occurrence of dextrocardia in association with occult spinal dysraphism was found to be extremely unusual. The second case is presented in relation to the rarity of teratoma with dermal sinus and tethered cord in the lumbar area. Myelography, computed tomography, (CT), Myelo CT and magnetic resonance were used in making a diagnosis.
