P-glycoprotein expression in refractory hematological neoplasms and circumvention of resistance with verapamil or cyclosporine A containing protocols.

Either p-glycoprotein (pgp) or the encoding gene mdr1 expression has been reported to be correlated with multidrug resistance and poor treatment response. To investigate the incidence of pgp in refractory hematological neoplasms we analyzed malignant cells from 40 patients by an immunoperoxidase method using the monoclonal antibody C219. Pgp was positive in 75% of acute nonlymphoblastic leukemia (ANLL) and 50% of acute lymphoblastic leukemia (ALL). Pgp positivity was similarly distributed in both Tdt (-) and (+) ANLLs (64% versus 100%). Addition of Verapamil (VRP) (12 patients) or Cyclosporine A (CsA) (7 patients) to the previous chemotherapy protocol resulted in complete response in 7 (58%) and 3 (43%) of the patients respectively. Partial response was observed in one patient who received CsA. Both chemosensitizers were tolerated well with few reversible side effects. The preliminary results of this study have been presented in the 15th International Cancer Congress, August 1990 Hamburg, Germany.

Stress-induced release of cortisol and prolactin during dilatation and curettage under general and local anesthesia.

Comparison of the effect of general and local anesthesia on serum prolactin and cortisol concentrations in response to the stress of dilatation and curettage revealed that short surgical procedures under general anesthesia resulted in no significant change in mean serum cortisol and in a significant decrease in mean serum prolactin. By contrast, short surgical procedures under local anesthesia resulted in a significant increase in serum prolactin and cortisol concentrations. Our findings support the importance of using general anesthesia during dilatation and curettage.

The endocrinological evaluation of bilateral and unilateral oophorectomy in premenopausal women.

The sequence of changes in serum follicle stimulating hormone (FSH), luteinizing hormone (LH), prolactin (PRL), estradiol (E2), testosterone (T), and progesterone (P) levels immediately following unilateral and bilateral oophorectomy with hysterectomy and hysterectomy alone has been measured in 65 premenopausal women by radioimmunoassay (RIA). In addition, relationship of vasomotor symptoms and serum hormone levels were studied and no correlation was found. We show mathematically the change of the relationship of the pituitary and gonadal hormones after castration. We believe that castration causes change in the activity of the hypothalamo-pituitary and gonadal axis. In view of our results, we recommend leaving at least one ovary to prevent postcastration symptoms and to maintain a good physiological condition.
