[Thymoma. Anatomo-clinical results apropos of 16 cases].

We report a clinicopathologic study in sixteen patients (11 male and five female) with thymomas. The fairly large proportion of children (3 cases: 12.5%) and significant number of advanced forms may at least partly explain the low survival rate (27% at five years) in our series. Advances in histologic studies of tumors have made possible the accurate diagnosis of thymomas, but the value of a histologic classification is still under debate. Conversely, pathologic and surgical classification is at present the best predictor of outcome.

[Pure gonadal dysgenesis with 46 XY karyotyping (Swyer's syndrome) with gonadoblastoma, dysgerminoma and embryonal carcinoma].

We report the clinical and pathologic findings in a 22-year-old woman with XY gonadal dysgenesis (Swyer's syndrome), who had bilateral gonadoblastoma associated on the right side with a dysgerminoma and an embryonal carcinoma. Swyer's syndrome is a distinct type of pure gonadal dysgenesis characterized by a 46 XY karyotype. It shows an abnormality in testicular differentiation. The patients are phenotypic females without stigmas of Turner syndrome. They have also elevated gonadotropins and hypoplastic gonads without germ-cells. The tumor that usually develops in Swyer's syndrome is gonadoblastoma. This tumor arises on dysgenesic gonads with a Y chromosome. Although gonadoblastoma is considered benign, the risk of malignant germ cell development is high. This means that these dysgenesic gonads should be removed surgically as soon as Swyer's syndrome is established.

[Juvenile secretory carcinoma of the breast. Apropos of a case in a 4-year-old girl].

The authors report the case of a juvenile secretory carcinoma of the breast in a 4 year old girl. The study of the first local excision leads to the thought of a "duct ectasia with a luminal benign papilloma". At the age of 6 years, there is a local relapse. A second local excision is performed and connected with simple mastectomy and an axillary node sampling. The pathological examination shows the aspect of a multifocal secretory juvenile carcinoma of the breast without lymph node axillary metastase. The authors emphasize the cystic appearance of the first tumor and the distinctive histologic pattern of this particular breast carcinoma.

[Prenatal echographic diagnosis of conjoined twins after 20 weeks of amenorrhea].

Conjoined twins are a rare entity in human teratology. Because early diagnosis of conjoined twins is so difficult as to be almost impossible it is usually only made in labour. Recent progress in the use of ultrasound as a means of diagnosis and also advances in neonatal surgery have stimulated a renewed interest in this malformation. A recent case at the University Hospital Centre of Habib Thameur in Tunis made it possible for us to diagnose the condition after 20 weeks of amenorrhoea and to terminate the pregnancy.

[Cutaneous epithelioid hemangioendothelioma].

INTRODUCTION: Epithelioid hemangioendothelioma is a tumor of soft tissues arising from the vascular endothelium. It is considered as a low-grade malignant tumor. Cutaneous involvement is rare and often associated with multi-systemic localizations. CASE REPORT: We report the case of a 34 year-old woman with a 6-month history of a 1.5 cm erythematous-violaceous, soft, painful cutaneous nodule involving the right forearm. An abdominal sonography had been performed 1 month before the onset of the nodule, because of epigastric pain and was normal. Histological and immunohistological examinations led to diagnosis of an epithelioid hemangioendothelioma. Assessment of the extension with radiology, sonography, tomodensitometry and magnetic resonance imaging, revealed nodules of the liver. Treatment consisted in the wide and complete excision of the tumor. There was no evidence of local recurrence after one year follow-up, and the hepatic lesions were stable. DISCUSSION: Epithelioid hemangioendotheliomas belong to the epithelioid vascular tumor spectrum. They have in common the morphologic epithelioid aspect of endothelial tumor cells. Cutaneous localization is rare, and to the best of our knowledge, only 20 cases of epithelioid hemangioendothelioma with skin involvement have been reported in the literature. Epithelioid hemangioendothelioma can be isolated or associated with internal visceral involvement. The detection of skin lesions should lead to a complete assessment of the extension of the disease to detect any internal localization. Because of low-grade malignancy of the tumor, and the integrity of the liver observed one month before the onset of the disease in our patient, epithelioid hemangioendothelioma appears to be a multicentric disease rather than metastatic hepatic localizations of a primitive skin cancer.
