Persistent hyponatremia and inappropriate antidiuretic hormone secretion in children with extensive burns.

Three children aged 2 1/2 to 5 1/2 yr, with burns covering 30%--45% of body surface area, developed hyponatremia and serum hypotonicity on the 5th--6th day following the burn injury. The hyponatremia persisted for 10--15 days. During this period, all three passed inappropriately concentrated urines. One child also demonstrated marked and inappropriate thirst. All three children demonstrated persistent respiratory alkalosis, which appeared and disappeared concomitantly with the hyponatremia. There were no signs of dehydration, and plasma volumes, measured in two children, were normal to high. These children are believed to show evidence of inappropriate antidiuretic hormone (ADH) secretion. In the absence of those conditions known to produce this syndrome, it is postulated that in these children it may have resulted from prolonged pain, anxiety, and/or pyrexia.

Acidosis

Lupus nephritis. Electron-dense and immunofluorescent deposits and their correlation with proteinuria and renal function.

Thirteen patients who had systemic lupus erythematosus were studied for the purpose of correlating the findings of light and electron microscopy and immunofluorescent studies with renal function and the presence of proteinuria. Subendothelial deposits of electron-dense material were found in all biopsy specimens, whereas mesangial and subepithelial deposits were not always present. IgG and beta 1c were constant findings in the glomerular membrane. The seven patients who were found to have extensive subendothelial deposits had moderate to massive proteinuria, and four of these patients had decreases in renal function. Most of the patients who had smaller subendothelial deposits had slight or insignificant proteinuria. Massive subendothelial deposits were mainly found in specimens showing histologic evidence of active lesions. Correlation was also found between proteinuria and the overall amount of deposits and their distribution. The prognosis was dependent on the severity of subendothelial deposits, the overall amount of deposits, and the morphologic form of lupus nephropathy.

Adolescent

Immunofluorescent and electron-microscope findings in the uninvolved skin of patients with systemic lupus erythematosus.

Specimens of uninvolved skin of 20 patients with systemic lupus erythematosus (SLE) were studied using a direct immunofluorescent technique (IF) and the electron microscope (EM). EM examination revealed deposits of electron-dense material below the basal membrane, among collagen fibers in the dermis and in the walls of dermal blood vessels. The direct IF technique revealed granular "band" deposition of immunoglobulins and complement at the dermo-epidermal junction as well as granular deposition in the walls of blood vessels. There was close correlation between the IF and EM findings. It was concluded that the EM is of particular value in precisely delineating the location and ultrastructural pattern of skin deposits. The IF technique is not limited in relation to the size of area which can be studied and makes it possible to identify the specific immune fractions present. It is suggested that these two methods are mutually complementary in providing information concerning the deposition of immunoglobulins taking place in the uninvolved skin of SLE patients.

Adult

Pleural effusion--presenting sign in multiple myeloma.

A patient with multiple myeloma in whom recurrent right pleural effusion was the presenting sign of the disease is reported. An IgA (k) monoclonal component was found in both the pleural effusion and the serum. The bone marrow specimen was interpreted as typical for multiple myeloma and the pleural fluid contained numerous plasma cells. Treatment with cyclophosphamide was followed by clinical improvement and the disappearance of the pleural effusion.

Long-term follow-up of clefts of the secondary palate repaired by von Langenbeck's method.

In analysing our small series with caution we tend to support the findings of Lindsay (1971) and Blocksma et al. (1975). Thus the combination of no facial growth deformity and a reasonable rate of velopharyngeal competence has encouraged us to continue using this simple and safe procedure for the treatment of cleft palates with intact lip and alveolus.

Adolescent

IgA-IgG nephropathy: a clinicopathologic entity with slow evolution and favorable prognosis.

Renal biopsy specimens were obtained from nine patients with proteinuria and persistent macroscopic or microscopic hematuria. Histologic examination either disclosed no abnormality or showed moderate mesangial thickening and occasionally, evidence of focal segmental glomerulonephritis. Immunofluorescent studies revealed diffuse generalized mesangial deposits of IgA, IgG and betalc in all specimens. Fibrinogen deposits were present in the mesangial space in four specimens only, while IgM was uniformly absent. Serial sections of identical glomeruli allowed the localization of betalc within both IgA and IgG deposits. Ultrastructural studies of the renal biopsy specimens showed accumulation of electron-dense material in the subendothelial region of the capillary loops and the mesangium, with thickening of its matrix. Follow-up data indicated a generally good prognosis.

The use of the CO2 laser for partial nephrectomy: an experimental study.

The CO2- laser knife was used for partial nephrectomy in an experimental study of 20 mongrel cats. Of these, 16 survived, two died of peritonitis, and two died of unrelated causes. The inherent characteristics of precision and hemostasis of this modality, already found useful in controlling complications in other fields of surgery, proved to be a safe and effective method for partial nephrectomy as shown by angiography and histopathologic examinations of the operated kidneys at 0, 7, 14, 21, and 90 days after surgery. Complications such as hemorrhage or urinary fistula were nonexistent.
