[Ambivalent bacteriophages of different species active on Escherichia coli K12 and Salmonella sps. strains].

A study was made of several bacteriophages (including phages U2 and LB related to T-even phages of Escherichia coli) that grow both on E. coli K12 and on some Salmonella strains. Such phages were termed ambivalent. T-even ambivalent phages (U2 and LB) are rare and have a limited number of hosts among Salmonella strains. U2 and LB are similar to canonical E. coli-specific T-even phages in morphological type and size of the phage particle and in reaction with specific anti-T4 serum. Phages U2 and LB have identical sets of structural proteins, some of which are similar in size to structural proteins of phages T2 and T4. DNA restriction patterns of phages U2 and LB differ from each other and from those of T2 and T4. Still, DNAs of all four phages have considerable homology. Unexpectedly, phages U2 and LB grown on Salmonella bungori were unstable during centrifugation in a CsCl gradient. Ambivalent bacteriophages were found in species other than T-even phages and were similar in morphotype to lambdoid and other E. coli phages. One of the ambivalent phages was highly similar to well-known Felix01, which is specific for Salmonella. Ambivalent phages can be used to develop a new set for phage typing in Salmonella. An obvious advantage is that ambivalent phages can be reproduced in the E. coli K12 laboratory strain, which does not produce active temperate phages. Consequently, the resulting typing phage preparation is devoid of an admixture of temperate phages, which are common in Salmonella. The presence of temperate phages in phage-typing preparations may cause false-positive results in identifying specific Salmonella strains isolated from the environment or salmonellosis patients. Ambivalent phages are potentially useful for phage therapy and prevention of salmonellosis in humans and animals.

[Evaluation of a cadaver workshop for education in regional anesthesia].

BACKGROUND: The aim of the present investigation was to survey former participants (n=869) of a cadaver workshop using a mail questionnaire to assess the demographic data and the impact of these courses on daily practice. METHODS: The deadline for acceptance of return mail was 60 days. Descriptive statistics were employed for analysis of results. RESULTS: The response rate was 36.7% and the course was judged to be recommendable by 98.2%. The average course attendant was board certified and had spent a mean time of 9+/-6 years in anesthesiology. The highest quality and degree of subsequent practicability in daily routine was attributed to peripheral nerve block training on cadavers. Of the course participants two-thirds performed regional anesthesia procedures more often following attendance. The majority of course attendants had to defray at least a part of the course fee themselves, and one-third was required to invest leisure time to attend. CONCLUSION: Attendance of a cadaver workshop increased knowledge of clinical anatomy and enhanced performance of regional anesthesia procedures. Courses of this format constitute a currently underestimated adjunct to contemporary regional anesthesia education.

Testing of a new pneumatic device to cause pain in humans.

BACKGROUND: Surgical pain typically combines superficial and deep pain. We wished to generate pain that resembled surgical pain, reliably and reproducibly, in volunteers. METHODS: We constructed a computer-controlled pneumatic device to apply pressure to the anterior tibia. The reproducibility of the pain was tested by rating the pressure that caused pain rated 4-5 on a visual analogue scale (VAS) on days 0, 7, and 24 in 10 volunteers. The effect of remifentanil (0.025, 0.05, 0.075, and 0.1 microg kg-1 min-1) on pain tolerance in another set of volunteers (n=11) was used as an indirect measure of the reliability of pain production. RESULTS: The pressure needed (0.7 (0.3) to 0.9 (0.4) atm (mean (SD)) to induce pain rated 4-5 (VAS) did not vary, showing long-term reproducibility of the method. When pressure was applied to cause increasing pain in volunteers (n=11) 0.05 microg kg-1 min-1 remifentanil increased pain tolerance by 50%. An approximate doubling of the dose (0.1 microg kg-1 min-1) increased pain tolerance significantly more. The linear logarithmic dose-effect relationship shows that the device causes pain reliably, and this can be reduced with opioid treatment. CONCLUSION: This pneumatic device can apply pain reliably and reproducibly.

Midterm outcome of endovascular abdominal aortic aneurysm repair in octogenarians: a single institution's experience.

OBJECTIVES: We analyzed midterm durability of endovascular abdominal aortic aneurysm repair (EVAR) in octogenarians compared with younger patients. METHODS: Data for 182 consecutive patients who underwent elective EVAR between 1999 and 2003 were retrospectively reviewed. Forty-nine patients (27%) were 80 years or older (study group [SG]; mean age, 84 years; range, 80-89 years), and 133 patients (73%) were younger (control group [CG]; mean age, 72 years; range, 53-79 years). Chi2 analysis, Fisher exact test, Student t test, and Mann-Whitney U test were used as appropriate to test for intergroup differences. Kaplan-Meier curves, log-rank tests, and multivariate Cox models were used for time-to-event analysis, with P < or =.05 considered significant. RESULTS: Mean follow-up was 16 months (range, 1-43 months). Body weight was higher (P <.001), and elevated plasma lipid levels (59% vs 43%; P =.042) and use of nicotine (47% vs 29%; P =.015) more frequent in the octogenarians. Baseline aneurysm size, procedure-related data, and hospital stay were comparable between groups. Aneurysm-related mortality was 0% in the study group and 0.7% in the control group (P =.740). Systemic complications occurred in 22% (SG) versus 11% (CG) (P =.035), owing to a rise in serum creatinine concentration greater than 30% of baseline in 14% in the octogenarian group (vs 5% in the CG; P =.048). Groin lymphoceles developed in 12% (SG) versus 2% (CG; P =.013). Technical success was 96% (SG) versus 98% (CG; P =.408), and clinical success was 86% versus 90% (P =.269). No aneurysm rupture occurred during follow-up, and aneurysm-related adverse events were comparable between groups. The estimated risk for any type of endoleak (2.2; 95% confidence interval [CI], 1.1-4.2; P =.023) or type II endoleak (2.1; 95% CI, 1.0-4.3; P = 0.51) was higher in the study group versus the control group; however, this did not affect secondary procedure rates (SG 16% vs CG 12%; estimated risk, SG vs CG,: 1.5; 95% CI, 0.6-3.6; P = 0.420) or aneurysm remodeling (97.2% combined aneurysm sac stabilization or decrease in both groups; P =.592). Aneurysm enlargement occurred in 2.8% (SG 1 vs CG 4; P =.592). CONCLUSION: Elective EVAR in octogenarians appears safe and effective over midterm follow-up, with a temporary decrease in renal function (14%) and postoperative lymphoceles (12%) being the most common postoperative adverse events. Advanced chronologic age is not associated with diminished procedural outcome, clinical success, or postoperative survival, compared with younger age. Because of low perioperative mortality and high procedural success, EVAR may be the preferred approach to abdominal aortic aneurysm treatment in selected elderly patients.

[Comparison of genomes of new gigantic Pseudomonas aeruginosa phages from native populations from different regions].

To study the genome diversity of bacteriophages from geographically distant natural populations, new giant phi KZ-like Pseudomonas aeruginosa phages isolated in two different regions were compared with earlier known phages of three species (phi KZ, Lin68, EL). A broad spectrum of lytic activity was demonstrated for all phi KZ-like phages. Phages of the phi KZ species proved to be common in natural populations of various regions, while IL- and Lin68-related phages were extremely rare. Most phi KZ-related phages had unique DNA restriction patterns, but the differences between these were only minor, and the genomes did not contain nonhomologous fragments. The spectrum of capsid polypeptides proved to be conserved in each species, and was proposed as a character necessary and sufficient for express classification of phages with an accuracy of species. Phages isolated in different geographical regions showed no substantial difference. Some phages only slightly differing in DNA restriction pattern from phi KZ may be used to study the origin of phi KZ genes coding for orthologs of proteins of unrelated species (other phages, pathogenic bacteria, eukaryotes).

Analysis of neurogenesis and programmed cell death reveals a self-renewing capacity in the adult rat brain.

The adult central nervous system was thought to be very limited in its regenerative potential; however, the discovery that stem cell populations produce neurons in the adult brain highlights the dynamics of a previously assumed 'static' organ. The continuous generation of new neurons in the adult brain, nevertheless, leads to the question of whether neurogenesis is counterbalanced by an accompanying cell death in the same regions. The objective of this study was to stereologically analyze neurogenesis and programmed cell death in adult brain regions with known neurogenic activity. Using bromodeoxyuridine (BrdU) to identify newborn cells we find that within a few days of BrdU-labeling the adult dentate gyrus and olfactory bulb generate high numbers of newborn neurons. More importantly, dUTP-nick end labeling (TUNEL) reveals that areas of adult neurogenesis also contain high numbers of apoptotic cells. We conclude that programmed cell death may have an important regulatory function by eliminating supernumerous cells from neurogenic regions and may thus contribute to a self-renewal mechanism in the adult mammalian brain.

[Thoughts on the prevention and therapy of psychological diseases in children and adolescents].

The consideration of special aspects of family structures and development of personality are a matter of equal importance for prevention and therapy of adolescents. In this publication we want to show some basic attitudes for the therapy of adolescents with psychosomatic as well as other severe psychiatric disorders. We also discuss the differentiation of major affective depression.
