Progression of multiple sclerosis is associated with exon 1 CTLA-4 gene polymorphism.

OBJECTIVES: Multiple sclerosis (MS) is a chronic demyelinating disease of the central nervous system which is widely believed to have a T-cell-mediated etiology. The cytotoxic T-lymphocyte antigen-4 (CTLA-4) antigen molecule plays a key role in the downregulation of T-cell responses. To examine the genetic association of the CTLA-4 gene locus with MS, we analyzed an exon 1 (A49G) transition. MATERIAL AND METHODS: One hundred and fifty-two MS patients and 154 controls were examined. The A/G transition was genotyped by a polymerase chain reaction followed by labeling with a SNaPshot kit and detection using a capillary genetic analyzer. RESULTS: The genotype, allele and phenotype frequencies did not differ significantly between MS patients and controls. Those MS patients with AA and AG genotypes had 4.36 times greater risk of progressing from the relapsing-remitting to the secondary progressive form of the disease than those with the GG genotype. CONCLUSION: The results of our study indicate that CTLA-4 (A49G) exon 1 polymorphism is associated with MS progression.

[Multimodal evoked potentials in malabsorption syndrome].

The multimodal evoked potentials (visual, somatosensory and auditory brainstem) in 23 patients with malabsorption syndrome of different origin were investigated. The diagnosis of the disease was confirmed on the basis of histological examination and result of D-ksyloza test. The control group consisted of 30 healthy persons. Examination of visual evoked potentials revealed significant prolongation of latency of P 100 component in examined group in comparison with controls. Latency of N13 and N20 of somatosensory evoked potentials in patients with malabsorption syndrome were also significantly prolonged when compared to controls, otherwise transit time to cortex (TTC) was merely prolonged. Auditory brainstem potentials were also abnormal. Changes involved prolongation of latency of I, III and V responses and prolongation of interpeak latencies of I-III, III-V, I-V as well. The amplitudes of the examined evoked potentials between patients and controls did not differ significantly. On the basis of obtained results in was pointed out that different specific afferent systems are affected in patients with malabsorption syndrome what seems to be connected with vitamin's deficiency, especially B12 and E. Authors conclude that multimodal evoked potentials examinations are useful in the diagnosis and monitoring of the disease, specially in subclinical cases.

Delayed attenuation of myocardial ischemia with repeated exercise in subjects with stable angina: a possible model for the second window of protection?

AIMS: A delayed myocardial protection extends between 24 and 96 h after ischemic preconditioning in animals. To test for this phenomenon in humans, subjects with stable angina were subjected to exercise test-induced myocardial ischemia and the effect of this "preconditioning" ischemic insult on the exercise-induced myocardial ischemia with the re-exercise after 24-96 hours was studied. METHODS AND RESULTS: Forty-eight males with a history of infarction and positive exercise test were recruited to the study. After baseline symptom-limited exercise test, the subjects were randomized to four experimental groups (n = 12/group). The groups were allowed to recover for 24 h, 48 h, 72 h or 96 h before performing the second exercise test. Variables analyzed were heart rate-systolic blood pressure product at 1 mm ST segment depression, time to 1 mm ST segment depression, maximum ST segment depression, exercise duration, and the total ischemic time. There were no intergroup differences in baseline values for these variables. All variables were significantly improved at 24 h, the improvement peaked usually at 48 h (maximum increase in the variables by 31-46%), and the variables returned to baseline by 96 h after the first test. CONCLUSIONS: The exercise-induced ischemia caused transient attenuation of myocardial ischemia with re-exercise. Although the time-window and the time-course of this effect shows striking resemblance to those of the delayed preconditioning in animals, its mechanism remains speculative. The most probable mechanisms that may be involved include increased myocardial perfusion and/or some adaptive changes in the myocardium, the delayed preconditioning being one possibility.

[The incidence of neuropathy in patients with diabetes type 1].

The aim of the study was to evaluate the incidence of diabetic neuropathy. The examined group consisted of 32 patients (14 boys, 18 girls) 16 to 19 years old with diabetes duration of 5 till 17 years, treated actually with multiple injections of human insulin. Diabetic neuropathy was observed in 56% of patients. The incidence of neuropathy was higher after 15 years of diabetes duration, in patients with bad metabolic control - HbA1c>8%, multiple ketoacidosis (p=0.003) and higher total cholesterol levels (p=0.003), with microalbuminuria (64.3%) and macroalbuminuria (83.3%) and in patients with systolic and diastolic blood pressure equal and/or above 90 % percentyle.

[The level of soluble forms of interleukin-2 receptor and adhesive molecules ICAM-1 and VCAM-1 in platelets of multiple sclerosis patients].

The levels of sIL-2R alpha, sICAM-1, and sVCAM-1 in sera of patients with clinically probable multiple sclerosis (ms) during the first relapse were assessed. In ms patients the levels of these 3 soluble molecules were significantly increased in comparison with the results obtained in controls. The results reflected activation of lymphocytes T and enhancement expression of adhesion molecules on brain endothelial cells, therefore they could serve indirectly as markers of immunological activation in multiple sclerosis. Any of the levels of soluble molecules in sera did not correlate with the severity of the disease.

[Evaluation of cardiovascular and sudomotor functions in Alzheimer's disease].

The assessment of some aspects of cardiovascular and sudomotor system was performed in the group of patients with Alzheimer's disease. No one of patients complained of disturbances arising from these two parts of the vegetative system. The autonomic functions were evaluated by means of: clinical examination, heart rate variability tests at rest, measures of blood pressure and heart rate in supine and standing position and skin sympathetic response. At rest heart rate and its variability as well as blood pressure in the patients did not differ significantly from the results obtained in the healthy controls. On standing, changes in systolic blood pressure and heart rate were recorded in the controls, but not found in the patients. The majority of patients presented with abnormalities in skin sympathetic response: lack of response or prolongation of its latency. The results of motor and sensory nerve conduction studies excluded neuropathic changes in the patients. We conclude that autonomic disturbances accompanied Alzheimer's disease and were probably caused by damaged control mechanisms within the central vegetative system.

[Primary and secondary intraventricular hemorrhage--clinical analysis].

In the recent decades an evident increase in the number of diagnosed intraventricular haemorrhages has been noted. The trial was undertaken of assessing of the incidence of primary and secondary intraventricular haemorrhages, the role of aetiological factors in various types of haemorrhages, the risk factors and the results of imaging procedures, as well as the clinical symptoms and treatment methods. The analysed group comprised 90 patients in hospital with the diagnosis of intraventricular haemorrhage in the years 1989-1996. The diagnosis was based on the results of computerized tomography /CT/ and on autopsy in one case. The clinical course of the cases varied considerably: very serious mainly in secondary intraventricular haemorrhages with high mortality. No signs pathognomic for intraventricular haemorrhage were found. The prognosis in primary intraventricular haemorrhage was better. Hypertension as risk factor was present in 71 cases. Pathological changes of blood vessels were found in 23 cases.

[Measurement of acetylcholine receptor antibodies in serum of patients with myasthenia gravis].

The aim of this study is the evaluation of the level of antiacetylcholine receptor antibodies in patients with different types of myasthenia (according to Osserman's classification) and establishing of correlation between the level of such antibodies and the clinical state of the patients. 63 patients with diagnosed myasthenia and 30 healthy controls without immunological diseases were evaluated. In all of them standard neurological examination was performed and the levels of antiacetylcholine receptor antibodies were measured. In 45 patients electrophysiological investigations were carried out. In 43 cases elevated levels of antiacetylcholine receptor antibodies were noted. In 20 patients the levels were within normal range. The level of antibodies showed correlation with the clinical type of myasthenia according to Osserman (the more severe was myasthenia, the higher was the level of antibodies). But the clinical state of the patient at the moment of examination did not show any clear correlation with the level of antibodies. Acetylcholine receptor antibodies measurement has significant diagnostic value in myasthenia. Nevertheless it should be interpreted with other diagnostic techniques.

[Evaluation of sweating function, changes in heart function and postural blood pressure in patients with multiple sclerosis].

The aim of the study was electrophysiologic assessment of sudomotor function and some aspects of cardiovascular system function in patients with multiple sclerosis and an attempt of referring assessed variables to duration of the disease and EDSS score. The study included 24 patients with clinically definite, relapsing-remitting multiple sclerosis and 22 healthy, age-matched controls. Autonomic functions were evaluated by means of clinical examination, heart rate variability tests, measures of blood pressure in supine and standing position and skin sympathetic response examination. Clinical symptoms in patients with multiple sclerosis were scarce. Electrophysiologic tests showed no abnormalities in heart rate variability and blood pressure response to standing in studied group. The majority of patients presented with subclinical dysfunction of sudomotor system, indicated by an abnormal sympathetic skin response, which did not correlate with duration of the disease or EDSS score. Sympathetic skin response examination proved to be a sensitive indicator of autonomic disturbances in patients with multiple sclerosis.

[Results of treatment with plasmapheresis in myasthenic crisis and severe polyneuropathy].

Results of plasmapheresis on myasthenic crisis, Guillain-Barre syndrome (GBS) and paraneoplastic polyneuropathy are presented. In the cases of myasthenic crisis which did not respond to steroids addition of plasma exchange (PE) improved dramatically respiratory failure. Also good results were obtained in GBS treated with PE. In the case of paraneoplastic polyneuropathy PE did not influence the course of disease.

[Guillain-Barré syndrome].

A case of the Gullain-Barré syndrome was observed in a woman aged 18 years. The course was acute and fatal. Sedimentation plasmaferesis was applied in the treatment. A peculiarity of the case was the ascending course of paralysis and coexistence of encephalomyelitis.

[Sensory conduction and central somatosensory conduction time in toxic polyneuropathy].

The "transit time to cortex" was estimated in the group of 15 patients living around the copperworks. The somatosensory evoked potentials method has been applied. Clinical and neurographic disturbances (polyneuropathy with prolonged sensory conduction velocity) in this group of patients was accompanied by disturbances of the central nervous system ("transit time to cortex" was prolonged). The examination of averaged evoked potentials has a great value in estimation of early stages of toxic damages of the central nervous system and can be applied in the prevention of chronic industrial poisonings.
