Pregnancy-specific beta 1-glycoprotein (SP1) after in vitro fertilization and embryo transfer.

Recent reports indicate that SP1, a "pregnancy-specific beta 1-glycoprotein", can be used as a biological marker for very early pregnancy and occult abortion. In this investigation, SP1 serum concentrations were measured in the luteal phase of 48 menstrual cycles stimulated for in-vitro fertilization (IVF) and embryo transfer (ET). All patients received hMG for ovarian stimulation. Ovulation was induced by beta-hCG and also administered to support the luteal phase. In the 8 pregnancies arising after ET, SP1 (less than 0.5 ng/ml) was not detected before 13 to 19 days after laparoscopy. In contrast, the pregnancy-dependent beta-hCG increase was detectable earlier than SP1 despite the administration of hCG given for luteal support. However, low SP1 readings (0.5-1.1 microgram/ml) as early as 3 days after laparoscopy were observed in 11 cycles without a positive sign of beta-hCG production. Our results suggest that SP1 determinations cannot be used as a marker for occult abortion; also, positive SP1 readings without increase beta-hCG, especially during the early luteal phase after ET, have to be interpreted with caution.

Plasma aldosterone response to acute stimulation in panhypopituitarism.

The influence of acute stimulation by ACTH, upright posture and angiotensin II on plasma aldosterone levels was assessed in human panhypopituitarism. While stimulation by ACTH in hypopituitary patients induced a plasma aldosterone increase similar to that observed in healthy controls, stimulation by upright posture or by infusion of angiotensin II resulted in a lower plasma aldosterone response than in controls in most of the patients. These results suggest that the presence of an anterior pituitary hormone, most likely ACTH, directly or indirectly exerts a permissive action on aldosterone secretion in man.

Bromocriptine-induced modulation of plasma aldosterone response to acute stimulations.

The influence of the dopaminergic agent 2-bromoergocryptine on plasma aldosterone response to acute stimulations was assessed in 14 normal male volunteers. Without modifying basal plasma aldosterone levels or urinary aldosterone excretion, bromocriptine retarded and diminished significantly the plasma aldosterone response to angiotensin II or ACTH, but did not alter the response to upright posture. These results point to a bromocriptine-induced modulation of the plasma aldosterone response to direct stimulations at the adrenal level. The present work brings further evidence for an effect of the dopaminergic system on the physiological control of aldosterone.

Diagnosis of hyperthyroidism when serum-thyroxine alone is raised.

31 patients admitted to hospital with different non-thyroidal intercurrent diseases were found to have raised total serum-thyroxine (T4) and free T4 together with normal serum-triiodothyronine (T3) concentrations. At admission none was clinically hyperthyroid. Patients were assigned to 3 groups according to clinical course and the laboratory findings. In the first group (14 patients) classic hyperthyroidism developed after recovery from the intercurrent disease. 11 out of these 14 patients had recently received iodine-containing preparations. In a second group (11 patients) the initially raised serum-T4 rapidly returned to normal with recovery from the non-thyroidal disease. In a third group (6 patients) serum-T4 concentrations remained raised well after recovery from intercurrent disease. In this group, there were 2 cases of transient iodine-induced (Jod-Basedow) hyperthyroidism in which raised serum-T4 returned spontaneously to normal after several months as iodine was eliminated. These results indicate that increase in serum-T4 with normal serum-T3 in patients with intercurrent systemic disease is not always the result of hyperthyroidism and in many cases probably reflects changes in peripheral metabolism of T4. It is suggested that careful clinical follow-up is needed in patients with raised serum-T4 and normal serum-T3 for the early detection and treatment of classic hyperthyroidism.

[Recent developments in the etiology and diagnosis of Basedow's disease].

The etiology and diagnosis of Grave's disease are briefly reviewed with particular reference to newer discoveries in the wide range of immunoglobulins which might act as humoral stimulators of the thyroid. Diagnosis of hyperthyroidism has been greatly facilitated by radioimmunologic determination of serum T4 and T3 and indirect estimation of T4 (T4 index). In this way 85-90% of cases can be diagnosed with a single blood sample. TRH tests and T3 suppression tests are only rarely required.

Influence of acute administration of human growth hormone and alpha-MSH on plasma concentrations of aldosterone, cortisol, corticosterone and growth hormone in man.

The effect of acute administration of human growth hormone (HGH) and of alpha-melanocyte stimulating hormone (alpha-MSH) on plasma aldosterone, cortisol, corticosterone and growth hormone has been studied in normal man and in patients with panhypopituitarism. There is no acute effect of exogenous HGH on plasma levels of aldosterone, cortisol and corticosterone in normal man and in patients with panhypopituitarism. The plasma level of immunoreactive HGH measured during acute HGH infusion in man does not seem to be proportional to the dose administred in our study. Alpha-MSH raises the concentartion of plasma HGH, BYT THIS STIMULATION IS NOT DOSE-DEPENDENT. Aldosterone, cortisol and corticosterone concentrations are not influenced by the elevation of HGH mediated by alpha-MSH in normal man. Although in some patients with panhypopituitarism an elevation of plasma aldosterone concenntration following alpha-MSH infusion is observed, it is unlikely that MSH is directly involved in the acute regulation of aldosterone secretion in healthy subjects.
