[DNA profile, recurrence rate and progression of superficial G2 cancer of the urinary bladder].

In a retrospective study the prognostic value of flow cytometric DNA analysis was studied in 114 newly diagnosed cases with superficial bladder tumours. G1-tumours were generally diploid, G3-tumours almost exclusively aneuploid. G2-tumours exhibited a bimodal distribution with 50% diploid and 50% aneuploid cases respectively. Recurrence and progression correlated systematically with the ploidy level. Furthermore, the degree of aneuploidy is predictive for proneness to progression.

Short-term perfusion and "equilibration" of canine kidneys with protective solutions.

Kidneys were perfused either with Euro-Collins-solution or with HTK-solution of Bretschneider. The perfusion pressure as well as the perfusion flow were measured during a six-minute perfusion. The perfusion resistance was higher in Euro-Collins-kidneys than during HTK-perfusion. The venous outflow of the kidney as well as the ureteral outflow was measured during each minute of the perfusion and has analysed for osmolality, and for sodium and potassium concentrations. In Euro-Collins-kidneys a complete "equilibration" of the extracellular space was not achieved, while during HTK-perfusion concentrations in the venous as in the tubular outflow, similar to those in the HTK-solution itself, could be reached. At the end of the different perfusions, tissue was analysed for biochemical parameters such as ATP, ADP, AMP and lactate as well as for morphological features. Lactate had increased and ATP had decreased during perfusion with Euro-Collins-solution, while ATP had not changed and lactate had decreased during perfusion with HTK-solution. Normal glomerular, tubular and dilated vascular structures can be seen after HTK-perfusion, while a glomerular and vascular contraction takes place during Euro-Collins-perfusion.

Effects of glucose in protected ischemic kidneys.

Energy reserves (TAN) and anaerobic substrates (glucose, glycogen) are lower in renal than in myocardial tissue. Euro-Collins-solution contains nearly 200 mmol/l glucose, while the HTK-solution of Bretschneider contains none. Therefore the influence of glucose on kidney lactate production, on energy reserves (TAN), intrarenal pH and on morphology during the protection of ischemic kidneys was analysed using either Euro-Collins-solution, or modified "Euro-Collins-solution", containing mannitol instead of glucose, or HTK-solution with and without the addition of 5, 10 and 20 mmol/l glucose. Glucose content changed during kidney perfusion with Euro-Collins-solution from about 60 to 800 mumol/gdw. While intrarenal pH decreased from 7.1 to 5.1 in Euro-Collins-kidneys during 420 min of ischemia at 25 degrees C, pH decreased to 6.7 with the modified, mannitol containing "Euro-Collins-solution". In HTK-protected kidneys intrarenal pH decreased with increasing glucose addition to the solution. Although Total Adenine Nucleotides are highest at the end of ischemia with Euro-Collins-solution, structural protection after the same ischemic stress was best in HTK-protected kidneys without glucose addition. We conclude that glucose stimulated lactate production, reduced interstitial pH in the kidney even in combination with a highly buffered solution and that it might cause greater membrane permeability leading to a structural deterioration. Mannitol seemed more appropriate than glucose in this respect, although other substances, which provide energy substrate and prevent structural damage, may exist.

DNA-fluorescence-cytometry and prognosis (grading) of meningiomas--a study of 104 surgically removed tumors.

A total of 104 meningiomas of various histological types were examined microscopically and with flow fluorescence cytometry using the EBR staining technique and either an ICP 11 or an ICP 22 (PHYWE). Also tissue culture cells of 72 tumors were studied for their DNA content. There was a variable DNA distribution, which allowed a grading of malignancy according to a 3 or 4 grade scale. Useful information on the nature of a meningioma and its proliferative activity can be obtained during the surgical treatment. A majority of meningiomas show slowly proliferative DNA patterns typical for benign tumors. More than one third of the meningiomas show an intermediate configuration, probably associated with more or less semi-benign proliferative activity. 5.8% of our cases showed primary malignant tumors--except meningosarcomas.

[Pulmonary fibrosis following instillation of mitomycin C in the urinary bladder].

Intravesical prophylaxis against recurrence of bladder carcinomas using mitomycin C has proved to be an effective treatment with few side effects. No side effects involving the lungs have so far been observed with systemic administration; with intravesical use, however, this is not the case. We report a 32-year-old man in whom pulmonary interstitial fibrosis occurred after intravesical instillation of mitomycin C. The clinical picture, including the therapy, and the differential diagnosis and practical consequences are all discussed.

[Kidney function parameters following ischemia stress using the Euro-Collins solution or the Bretschneider cardioplegic HTK solution].

Protection-methods, for an improvement of ischemic tolerance of the kidney, can be investigated by intraischemic analysis of metabolism and structure. A definite proof for the effectiveness of a protection method is only postischemic function in combination with postischemic structure-regeneration. For this reason postischemic function was chosen for examination of the protective ability of the Euro-Collins-solution and the HTK-solution during a two-hour reperfusion period. We perfused 57 dog kidneys either with the Euro-Collins- or with the HTK-solution prior to ischemia. Ischemia was 7, 60, 90 and 120 min after Euro-Collins-perfusion and 7, 120, 150 and 180 min after HTK-protection. The protected and ischemic kidneys were left in-situ; the mean ischemic temperature was therefore 20-25 degrees C for the shorter ischemic times and 30-34 degrees C for the longer ischemic times. We compared the protected and ischemic kidneys with 14 untreated kidneys (control). Postischemic renal blood flow (RBF) was measured by an electromagnetic flow probe; renal oxygen consumption (V02/min) was calculated by arterio-venous oxygen content difference and the renal blood flow. If urine could be collected, the glomerular filtration rate (GFR) was measured by an endogenous creatinine clearance. In Euro-Collins-protected kidneys after 60-120 min ischemia the RBF was after 15 min of reperfusion between 20 and 100 ml/min/100 g. After 30 min we got values of 100-200 ml/min/100 g. The V02/min, which was in the control kidneys between 5-6 ml/min/100 g, was about 2 ml/min/100 g.(ABSTRACT TRUNCATED AT 250 WORDS)

[Measures for reducing exposure to scattered radiation in urologic roentgen diagnosis].

A simple device to reduce the urologic examiner's exposure to radiation is presented. The doses the examiner and his assistant staff are exposed to performing various methods of urologic radiodiagnosis were measured and the following exposure to radiation determined with and without radioprotective device. The measurements demonstrate, that already by simple measures considerable reduction of the exposure to radiation can be obtained for examiner and assistant personnel. The frequently neglected importance of scattered radiation resulting from urologic radiography is discussed.

[Lymphography in malignant testicular tumors].

Experience and results are reported on the basis of more than 380 lymphographic investigations in patients with a malignant testicular tumor. In 61% of these patients, there was suspicion of a metastatic infiltration of the retroperitoneal lymph nodes. In 75 patients, histological checking of the lymphographic finding was possible on the basis of a retroperitoneal lymphadenectomy. Lymphography was shown to have a precision of 81%. Lymphography is obligatory as a complement to other techniques of investigation such as sonography and computer tomography in patients with malignant testicular tumor, since the greatest diagnostic information can be expected from combined application of all three methods.

Post-ischemic renal function after kidney protection with the HTK-solution of Bretschneider.

The cardioplegic solution HTK of Bretschneider was used for canine kidney protection. The kidneys were perfused with this solution for 6-10 min prior to the induction of ischemia. The kidneys were left in-situ for 60, 90, 120 and 135 min ischemia time at a temperature of 25-34 degrees C (n = 13). As a control group we used unilateral nephrectomized dogs (n = 9). After unilateral nephrectomy an elevated plasma creatinine in comparison to preoperative values was observed. After 60 and 90 min under HTK-protection the postoperative plasma creatinine was not elevated compared to the control group. After 120 min of ischemia creatinine level was slightly increased to an average of 2.1 mg% on the first and second postoperative day. These experiments indicate the protective effect of the cardioplegic solution for canine kidney preservation in situ.

DNA-flow fluorescence--cytometry of ependymomas. Report on ten surgically removed tumours.

The distribution of DNA is estimated from flow cytometric histograms in surgical specimens of ten ependymomas of different location and varying anaplasia. In three cerebral tumours grade I--II, including one ependymoma of the 4th ventricle, only limited elevation of the 4 C maxima was a prominent feature, corresponding to the microscopical frequency of typical mitoses. Four grade-III ependymomas showed aneuploid or polyploid histograms with stem lines. One frontal tumour was classified as "transitional" because of more numerous mitoses and abnormally elevated S and G2 + M phases, which increased in tissue culture. A correlation between the degree of anaplasia with the DNA pattern was difficult to pursue in two spinal ependymomas obviously lacking microscopical mitoses: Both--one a so-called tanycytic variant of grade I--II, and the other probably a metastasis from a cerebellar tumour--had a clear polyploid DNA histogram with a strikingly increased proliferation index, similar to the more malignant tumours of grade III. Also flow DNA measurements probably allow the decoding of heterogenous mixtures of tumour cells which are not always benign in ependymomas of lower grades of anaplasia microscopically.

[Penile melanomas].

Melanoma of the penis is a rare, very malignant tumor with poor prognosis. At present improvements of therapeutic results can only be achieved by means of early diagnosis, perhaps also by means of radical surgery. Radiation therapy and chemotherapy are used mostly as adjuvant or palliative therapy. Two cases of malignant melanoma of the penis are presented.

Construction and experimental application of a catheter for selective arterial kidney perfusion in situ.

In order to improve dog kidney perfusion in situ with a protective solution, a perfusion catheter was constructed which allowed continuous pressure measurement in the center of the catheter tip during perfusion. Using this catheter, the equilibration of the extracellular space with a protective solution (HTK solution) was found to be pressure dependent. Continuous pressure and resistance control is therefore a prerequisite for reliable organ protection.

[Bezafibrate-induced myolysis and myoglobinuria in patients with impaired renal function].

A muscular syndrome has been described in patients on clofibrate and fenofibrate therapy. The present paper describes four patients with impaired renal function in whom symptoms and signs of skeletal muscle damage developed during treatment with another clofibrinic acid derivative, bezafibrate. The syndrome was characterized by variable degrees of muscular cramps and paresis, excessive elevation of muscle enzymes in serum, myoglobinemia and myoglobinuria. Transient deterioration of renal function was also common. All patients had been overdosed with bezafibrate with regard to their renal function. It is concluded that bezafibrate like other lipid lowering agents of the clofibrate type may induce muscle damage, at least if doses are not adjusted to renal function. Extreme caution is warranted when treating patients with renal impairment with bezafibrate and strict dose adjustment to kidney function is necessary to avoid muscle damage.

[Importance of impulse cytophotometry in the diagnosis and prognosis of prostate cancer].

In 102 cases with hypertrophy and 73 patients with carcinoma of the prostate, all histologically proven, fine-needle biopsies were done and the obtained material investigated by conventional cytology as well as by flow-through cytophotometric determination of DNA content of the nuclei. In prostatic cancer the DNA determinations showed less diagnostic precision than conventional cytology. However as an additional diagnostic procedure the flow-through cytophotometry was able to improve the results of cytologic diagnostic for 10% in all cases with histologically proven cancer of all grades. In hypertrophy of prostate, the exclusion of carcinoma could be done by DNA determination with higher security than by conventional cytology. Flow-through cytophotometry is therefore considered a useful additional test in primary diagnostic and grading of prostatic cancer. In addition it is a method to evaluate the efficacy of drug therapy.

[Unusual forms of retroperitoneal fibrosis].

The numerous forms of secondary retroperitoneal fibrosis which do not belong to Ormond's disease are discussed and some rare iatrogenic clinical pictures described. For the first time polyvinylpyrrolidone granulomas, causing obstructive renal failure are reported.
