Successful use of a laser for human embryo biopsy in preimplantation genetic diagnosis: report of two cases.

PURPOSE: The use of Tyrode's acid to drill the zona pellucida for embryo biopsy is the most widely used methodology in preimplantation genetic diagnosis. Instead of this, we propose the use of a 1.48-micron diode noncontact laser, which is quicker, simpler, and safer. METHODS: The laser beam was tangentially guided to the zona pellucida of the embryo. Depending on zona pellucida measurement, two to four consecutive shots of 8-22 msec were necessary to drill the zona pellucida of the 13 embryos biopsied for two patients (hemophilia carriers). RESULTS: Female embryos were replaced into the uterus of the patients (1.5 embryos/replacement). One single pregnancy was established (33.3% implantation rate). Coculture of untransferable embryos showed a blastocyst rate of 66.7% (4/6) for male embryos and 25% (1/4) for abnormal ones. CONCLUSIONS: These results demonstrate the safety and usefulness of laser methodology in preimplantation genetic diagnosis.

Parental human leukocyte antigens and implantation failure after in-vitro fertilization.

At present, it is well accepted that maternal recognition of paternally derived fetal antigens occurs during normal pregnancy and may be beneficial for implantation and maintenance of gestation. Thus, we have investigated the compatibility of human leukocyte antigens (HLA) in couples with successive failed in-vitro fertilization (IVF) cycles. Study group 1 included 50 couples with prior primary infertility who had not achieved a pregnancy after > or = 3 (range 3-7, mean 3.7) IVF cycles where at least two embryos (mean 3.3, range 2-4) were transferred in each attempt. An infertile control group (group 2) included 50 infertile couples undergoing IVF with the same indications as couples in group 1, who achieved a viable pregnancy with their first IVF attempt. The results were compared with those found in a population sample including 100 men and 100 women from the local population (group 3). We found a statistically significant (P < 0.05) excess of HLA sharing (> or = 2 antigens) between partners in group 1 as compared to groups 2. There was a trend toward increased HLA sharing in group 1 when groups 1 and 3 were compared. We conclude that some cases of implantation failure after IVF and embryo transfer might be caused by underlying close histocompatibility between partners.

Laser blastocyst biopsy for preimplantation diagnosis in the human.

A new methodology for blastocyst biopsy that uses a 1.48 microm diode laser is described. Trophectoderm cells are biopsied after laster zona drilling and culture, fixed and processed for fluorescent in situ hybridisation (FISH) analysis. Preliminary results on the efficiency of the procedure and blastocyst recovery rate are promising. Blastocyst laser biopsy is a useful tool in preimplantation genetic diagnosis (PGD) as it allows a more reliable diagnosis and widens the diagnostic possibilities on account of the higher number of cells obtained in the biopsy.

Twin pregnancy after preimplantation diagnosis for sex selection.

A twin female pregnancy was obtained in a haemophilia carrier after two preimplantation diagnosis cycles. The embryonic sex of biopsied blastomeres was determined with the use of dual fluorescence in-situ hybridization with directly labelled DNA probes specific for the X and Y chromosome. A twin female pregnancy was confirmed by means of ultrasonography and amniocentesis at the 14th week of amenorrhoea. The patient delivered two healthy females by Caesarian section at the 37th week of pregnancy.

A simple method for processing individual oocytes and embryos for electron microscopy.

A simple method for handling individual specimens that must be processed either for scanning or transmission electron microscopy studies is described. For scanning microscope processing, dehydration is carried out with samples enclosed in small cages made from TAAB capsules in which top and bottom are substituted by plankton nets, and for transmission electron microscopy, samples are preembedded in agarose. This procedure significantly reduces mouth pipetting, dissecting microscope observations, is less labour intensive and, most importantly, reduces sample loss.

Indications for oocyte donation.

Donated oocytes were transferred on 92 occasions to 87 women without gonadal function and 5 with functional ovaries. Twenty-three pregnancies were established (25% pregnancy rate), 9 after transfer of fresh embryos in 30 synchronous donor and recipient cycles and 14 after transfers of frozen-thawed embryos in 62 asynchronous donor and recipient cycles. Twenty-two pregnancies were obtained in agonadal patients (25.3% pregnancy rate) and 1 in a gonadal woman (20% pregnancy rate). Pregnant women were younger than those who did not become pregnant, but the difference was not significant. The pregnancy rate was higher when intra-Fallopian transfer was performed (46%) as compared with intrauterine transfer (21.5%) and when micronized progesterone was given intravaginally (pregnancy rate 30.3%) as compared to intramuscularly injected natural progesterone in oil (pregnancy rate 22%). Twenty healthy infants have been born including one set of twins; four pregnancies miscarried.

[Carotid artery disease induced by irradiation].

We have evaluated 39 patients with ischemic strokes secondary to atherosclerotic disease of the extracranial carotid artery. Seven of them had been treated in the past with cervical radiotherapy for neoplastic diseases (radiotherapy group); the remaining 32 patients had not received radiation therapy (non-radiotherapy group). When the prevalence of risk factors was compared between the two groups, there was a significantly higher prevalence of peripheral arterial disease (p less than 0.05), hypertension (p less than 0.05) and atherogenicity index lower than 21.5% (p less than 0.05) in the non-radiotherapy group. Regarding angiographic data, the radiotherapy group showed a significantly higher occurrence of localized findings than the non-radiotherapy group (p less than 0.005). It is concluded that radiation-induced carotid disease is a clinical condition which may be individualized from the remaining patients with atherosclerotic carotid artery disease.

Effects of aging on the zona pellucida surface of mouse oocytes.

In a scanning microscopy study of mouse oocytes from immature, young, and aged females and of oocytes aged in vivo or in vitro, we have observed four types of zona pellucida, which we classify as types A, B, C, and D. Oocyte aging gives rise to a significant increase in predegenerative (type C) oocytes; this type of zona pellucida surface could result from a zona hardening effect and decrease the rate of fertilization of the oocytes affected.

Myelin basic protein and creatine kinase BB isoenzyme as CSF markers of intracranial tumors and stroke.

In patients with intracranial tumors (ICT) and acute cerebral infarctions (CI), both necrosis and reversible changes occur in central nervous system (CNS) tissue. The damaged CNS cells release specific substances into the cerebrospinal fluid (CFS). Radioimmunoassay (RIA)-determined myelin basic protein (MBP) and RIA-determined creatine kinase BB (CK-BB) are markers of damage to CNS specific structures. The elevated CSF level of MBP is considered a marker of myelin damage and the increased concentration of CSF CK-BB may be of combined neuronal and astrocytic origin. CSF was collected from 57 patients with the diagnosis of CI (n = 30) and ICT (n = 27) and the concentration of MBP and CK-BB were measured by RIA. Our study shows increased CSF levels of MBP and CK-BB in patients with CI and patients with ICT. We have also found a linear correlation between MBP and CK-BB in both CI and ICT, and for a given CK-BB level, MBP was significantly higher in patients with ICT than in patients with CI. These facts suggest that lesion markers behave differently in the different pathologic processes affecting the CNS.
