[2 cases of neurinoma of the mastoidal portion of the facial nerve: diagnostic contribution of MRI].

The authors report 2 cases of patients presenting a neurinoma of the 3rd portion of the facial nerve. In the 2 cases, the surgical observations correlated perfectly with the results of the radiological examinations (scanner and MRI). The clinical and paraclinical presentation of this rare tumor is recalled, emphasizing the additional contribution of imaging techniques: the scanner is indeed indispensable for evaluating the osseous destructions on the intrapetrous trajectory of the facial nerve, whereas MRI (in particular with injections of gadolinium) evaluates the exact tumoral limits as best as possible.

[Comparison between ultrasonic and x-ray computed tomographic measurements of femoral antetorsion in children].

The measurements of femoral torsion by ultrasounds have been performed in both hips of 10 patients according to the technique proposed by Zarate et al. The measurements with CT were done in the same patients according to a technique similar to that used by Hernandez et al. The correlation between US and CT is excellent as far as the minutest particulars of the technique are followed for US examination. The authors recommend ultrasounds, as a non irradiating, non aggressive procedure for the measurement of femoral torsion. US must replace both conventional X-ray and CT for femoral torsion assessment.

[X-ray scanning in Marchiafava-Bignami disease].

CT scan in 4 patients with Marchiafava-Bignami's disease in its acute alcoholic form, who died after 12, 18, 30 and 50 days respectively, demonstrated large symmetrical bilateral hypodense areas in the hemispheric white substance. These areas were either widespread or more marked posteriorly, and were independent of the corpus callosum. Pathologic examination of the brains showed necrotic regions, often with cavities, or areas of demyelinization of the centrum ovale correlating with the hypodense areas as seen on CT scans. In two cases CT scan further showed regions of linear hypodensity in front of and medial to the frontal ventricular horns which could have corresponded to the corpus callosum necrosis found on the brain sections. CT scan can thus contribute to the diagnosis of Marchiafava-Bignami's disease.

Acute Disease↗

[Intracranial hemorrhage in infants born at term].

In 5 full-term neonates presenting with signs of acute neonatal brain damage, intracranial hemorrhage was proven by CT-scan performed within a few days after birth. Status epilepticus occurred soon after birth as shown on EEG. Prognosis was most often very poor. These data are compared with the series reported in the literature.

Cerebral Hemorrhage↗

[Lumbar venography in the diagnosis of disc herniation (author's transl)].

The authors have performed lumbar venography on 120 occasions in patients with low back pain. In 58 patients in whom the herniated disc was confirmed, the result of venography was positive and the localisation was precise in 67 cases. It is concluded that the method is safe, reliable and allows precision in diagnosis to allow better selection for surgical treatment. The examination is particularly useful in recurrent sciatica, fractures of the lumbar spine, recent or old, and in spinal stenosis. It may be associated with discography when the result is not clear cut.

Arthritis↗

[Myelopathies cervical arthroses. Apropos of 14 personal cases, 12 of which were operated on].

Reviewing fourteen cases, the authors show that cervical myelopathies present by spastic quadriplegia associated in half of the cases with sensory impairment and in a third of cases with radicular signs in the upper limbs. Tomography always shows posterior discal osteophytosis, sometimes gross, which reduces the lumen of the canal more particularly since it is associated (eleven times out of fourteen) with constitutional stenosis, of variable severity. Opaque or gaseous myelography makes it possible to exclude malformation or new formation and will demonstrate cord constriction within the canal. Surgical treatment (eleven laminectomies, 1 discectomy-artrodesis) resulted in 7 improvements, no change in 3 cases and 2 exacerbations. The myelopathies appeared to be due to repeated contusions of the cord and its arteries against the wall of the canal, caused by neck movements.
