[The feasibility of pretherapeutic diagnosis of malignant pharyngeal processes using high-frequency cineradiography].

The differentiation between stage T3 and stage T4 pharyngeal tumors is of crucial importance for the proper therapeutic approach (surgery, surgery with consecutive radio- or radiochemotherapy, chemotherapy). The clinical relevance of the additional information provided by cineradiography was evaluated in combination with the endoscopic and computed tomographic results. We performed static and dynamic pharyngography with mono- and double-contrast-methods in four planes, including pharyngeal distension (Valsalva's and Müller's maneuver) and phonation. For the dynamic study, high-speed cineradiography (35 mm film, 50 frames/s) was used. Twenty-three patients with histologically proven neoplasms were examined. It was determined that surgery was technically feasible in 4 out of 14 patients who were previously presumed to have inoperable tumors. In 2 cases, an extension of the tumor mass was seen by cineradiography (results proven intraoperatively).

[Incidence of maxillary sinusitis in intensive care patients with nasal intubation--ultrasonic diagnosis as a screening test].

Acute maxillary sinusitis due to nasotracheal intubation during intensive care is seldom considered as a cause of fever of unknown origin. Forty six patients admitted to a post-operative intensive care unit had their maxillary sinuses examined using a mobile "A-scan" ultrasonic scanner. Follow-up examinations were performed regularly. As early as the 5th day of treatment 36 out of 46 patients who had undergone nasotracheal intubation showed pathological ultrasonic results. Bilateral involvement was most frequent. In unilateral findings at the first examination, the intubated side was three times as often affected as the other side. Early extubation, early partial mobilisation and/or early administration of antibiotics did not prevent the occurrence of pathological ultrasonic findings. Acute maxillary sinusitis must thus be considered a frequent complication of nasotracheal intubation.

[A new procedure for covering septum defects].

In 15 cases of nasal septal defects a new technique was used to close perforations up to 3 cm in diameter with a mucoperiosteal flap. The flap was rotated from the end of the inferior turbinate into the nasal septal defect. This method is easy and has a high rate of successful closure.

[Detection of a higher incidence of pathologic somatic findings in globus sensation by use of high frequency cinematography].

Since December 1984 303 patients have undergone examination in our Multidisciplinary Consultation Service for Swallowing Disorders; 117 of them were suffering from typical globus symptoms. We were able to increase the yield of detection of organic lesions by use of the technique of 35 mm film cineradiography with a rate of 50 frames/s. Frame-by-frame analysis and computer-assisted evaluation showed that 80% of the patients with globus symptoms suffered from one or more underlying organic diseases, which could often be treated later with success. We found an increased incidence of early hypopharyngeal diverticula, webs, and motility disorders of the upper esophageal sphincter often associated with gastro-esophageal reflux or weakness of the pharyngeal wall. Cineradiography proved to be a very important tool in the analysis of the pharyngeal swallow in globus pharyngis.

[Maxillary sinusitis in intensive care patients].

Fever of unknown origin is a major diagnostic problem in traumatologic postoperative intensive care medicine. Even when other causes of fever (wound infection, pneumonia, intravenous catheter contamination) have been ruled out, purulent maxillary sinusitis is rarely considered as the initial focus. The maxillary sinuses of 46 patients admitted to a postoperative intensive care unit were examined using a mobile "A-Scan" ultrasonic scanner. Follow-up examinations were performed on a regular basis. As early as on the 5th day of treatment, the initially, most often nasotracheally intubated and artificially ventilated patients exhibited a high frequency of pathological ultrasonic results, whereas only 10 of the 46 patients within the study demonstrated normal findings throughout the follow-up examinations. As a rule, bilateral involvement was observed. When findings were initially unilateral, the nasally intubated side was most often affected. Early extubation, partial mobilisation and/or administration of antibiotics from the first day of treatment onwards did not prevent the occurrence of pathologic ultrasonic results. Fever and leucocyte count were found to be elevated to a higher level in patients with purulent sinusitis. Thus, affection of the maxillary sinuses appears to be a frequent accompanying disease in intensive care patients, and should therefore always be taken into consideration as the initial focus of fever of unknown origin.

[Documentation of mucous membrane changes in military gas (Lost) exposed patients].

Poison gases, such as "Lost" (bichloroethyl sulfide), which have been known since the first World war, and the use of mycotoxins can cause extremely grave alterations of the texture of the upper breathing and digestive system. These alterations are shown by photographs of three persons, and therapeutical measures are documented in a successive space of time. The significance of an early tracheotomy for a successful treatment is also mentioned.

[Endoscopically controlled ultrasound diagnosis of the paranasal sinuses].

261 patients with diseases of the paranasal sinuses were examined by ultrasound (A-Scan). Sinuscopy or surgery was performed on these patients. Samples for histologic examination were taken. The correlation between ultrasound, endoscopy and x-ray was 80-90%. In endoscopic histologic normal maxillary sinuses, however, x-ray examination led to wrong positive results five times as often as ultrasound examination did. X-ray examination was more accurate only in cases where the process was separated from the anterior wall of the maxillary sinus by a layer of air.

[Treatment of vestibular function disorders by a training program].

This article reports on a training programme for vestibular diseases, involving exercises of the vestibulo-vestibular, the visuo-vestibular, the cervico-vestibular and the spino-vestibular reflexes. Twenty-five patients were examined; all patients but two showed good improvement of the vestibular complaints as demonstrated by vestibular diseases, electronystagmography and posturography.

Controlled study of brodimoprim and cephalexin in the treatment of patients with acute sinusitis in general practice.

Brodimoprim, a new dehydrofolate reductase inhibitor, was compared with cephalexin in the treatment of patients with acute sinusitis. A total of 49 patients were randomly assigned to receive either brodimoprim 200-mg tablets once a day (400 mg on the first day as a loading dose) or cephalexin 500-mg tablets three times a day for 8 to 12 days. Nearly all patients treated were judged clinically cured/improved; in fact, only one failure (in the cephalexin group) was noted. In the 45 assessable patients, the time until disappearance of the symptoms and the duration of treatment did not differ significantly between the two groups. A bacteriologic examination was performed in all patients at baseline as well as at the end of therapy. Bacteriologic eradication was obtained in 88% of the patients treated with brodimoprim and in 76% of those receiving cephalexin. Both compounds were generally well tolerated (one patient in the brodimoprim group complained of skin reactions). These results suggest that once-daily treatment with brodimoprim represents safe and effective therapy for adults with acute bacterial sinusitis.
