[An unusual cause of arterial and venous pulmonary hypertension].

A 49 year old woman with syphilis of recent onset was repeatedly admitted to the hospital on account of recurrent dyspnoea and hemoptysis. The symptoms and radiological signs indicated the presence of a "cardiac lung". The ECG disclosed right ventricular overload which was confirmed by echocardiography. The left heart was normal, not only on echocardiography but also on cardiac catheterization, which documented severe pulmonary hypertension, partly precapillary, partly postcapillary. The presence of chronic lung congestion in the absence of pathological findings pertaining to the left heart lead to the diagnosis of pulmonary venous occlusive disease. However, pulmonary angiography showed notable and widespread arterial amputations which are not present in this syndrome. Autopsy provided the correct diagnosis: sclerosing mediastinitis localized at the hilum with prevalent involvement of both arterial and venous pulmonary vessels.

[Comparative evaluation of mexiletine and propafenone by dynamic electrocardiography].

We have compared the efficacy of Propafenon and Mexiletine in 12 subjects (7 males and 5 females, age range of 22-61 year) affected by chronic ventricular extrasystolic beats. Propafenon and Mexiletine were orally administered in doses of 900 mg/day and 600 mg/day respectively in a single blind, cross-over fashion. Our results show that: Propafenon is better tolerated; Propafenon has caused a statistically significant reduction of the total number of ventricular ectopic beats/24 hours; the efficacy of Propafenon has been greater than that of Mexiletine (66% and 25% respectively); with both drugs, in 2 cases, there was an increase of the total number of ventricular ectopic beats/24 hours; Propafenon has induced clear-cut electrocardiogram changes (statistically significant lengthening of P-Q and Q-T intervals).

Usefulness of xerotomography in the diagnosis of acute pulmonary embolism.

The Authors report their experience in the study of pulmonary embolism by xerotomography. The use of tomography for the study of pulmonary vascularization is well known, but this method is by far less frequently used than pulmonary angiography and pulmonary perfusion and ventilation scintigraphy. The enhancement of contrasts and contours and the ample area of exposure are the features which render the images obtained by xerotomography similar to those achieved by pulmonary angiography. Our experience up to the present time allows us to state that xerotomography may represent a valid diagnostic means in cases of acute pulmonary embolism, when scintigraphy is not feasible or following scintigraphy, when doubts regarding the clinical diagnosis still remain.

[Threatening arrhythmia by uncommon digitalic toxicosis (author's transl)].

The authors describe the case of a 38 year old woman who ate some buds of "Helleborus viridis" collected in the mountains mistaken for wild asparagus. The clinical patterns were like in the digitalic toxicosis: the woman presented both ventricular and supraventricular threatening arrhythmias, gastro-enteric troubles, systemic hypotension. The patient improved with the glucose, potassium chloride and magnesium sulphate infusion and intravenous xylocaine. The authors conclude that in case of patient with threatening arrhythmias of unknown genesis it is useful to pay attention to the wild vegetables ingestion.

[Precordial ST segment elevation mapping: a new method for a technically easier recording (author's transl)].

Precordial ST segment maps are currently recorded using a 35 electrode blanket connected to an electrocardiograph through a switch box. Problems in adjustment to patients' chests and artifacts are frequently encountered. Other techniques using a single suction electrode placed on different reference points are time wasting. The authors suggest the use of 35 self-adhesive electrodes (3M "Red Dot" chloride free gel pediatric electrodes--cat N. 2243) connected by a press button to a multichannel EKG recorder. This technique prevents artifacts, assures constant arrangement of the electrodes, is suitable for different thoracic structures and does not need any additional device. The authors consider that such a practical method should make precordial ST mapping a more widespread clinical procedure.

Coronary Disease

[Pre- and post-operative echocardiographic evaluations of mitral stenosis].

46 patients, affected by pure mitral stenosis, underwent echocardiographic study before and after mitral commissurotomy. The following echocardiographic parametres were considered: 1) closing velocity of anterior mitral leaflet (E-F slope); 2) amplitude of the mitral valve motion; 3) intensity and number of echoes coming from the mitral leaflet; 4) posterior leaflet motion; 5) left atrial diameter index; 6) right ventricolar diameter index; 7) left atrial and aorta diameter ratio. Moreover mitral diameter, mitral fibrosis or calcification, and possible mitral incontinence were evaluated intraoperatively before and after commissurotomy. The pre-operative echocardiographic examination of the mitral stenosis showed the following specific aspects: 1) reduced E-F slope, significantly correlated with the severity of the stenosis; 2) abnormally moving posterior mitral leaflet; 3) enlargement of the left atrium, directly correlated with the degree of the mitral stenosis. Correspondence was noted between the echocardiographic and the intraoperative evaluation of the anathomical pattern of mitral valve. Postoperatively the echocardiograms showed: 1) improved E-F slope, directly correlated with the surgical dilatation of the valve; 2) persistent abnormal movement of the posterior mitral leaflet; 3) disappeared correlation between the surgical diameter of mitral valve and the left atrial index. The ECHO proved to be a good diagnostic method for both a qualitative and a semiquantitative evaluation of mitral stenosis. The ECHO is also quite usefull in evaluating modifications induced by commissurotomy.

[Sublingual isosorbide dinitrate-induced severe hypotension, bradycardia and prelypothymia in patients with acute myocardial infarction (author's transl)].

100 patients with acute myocardial infarction were given 5 mg of sublingual isosorbide dinitrate within 36 hours by the onset of their symptoms. 86 patients did not show any unusual effect after the administration of the drug; their heart rate was only slightly increased and their arterial pressure slightly reduced. 14 patients developed severe systemic arterial hypotension, associated with absolute or relative bradycardia, within 30 minutes of receiving the drug. All the patients complained of fainting and sweating, 1 patient developed a syncope. Symptoms were relieved by raising patients legs in 10 cases, by 0.5 mg Atropina e.v. in 4 cases, 1 case required also external cardiac massage. There was no significant difference between the two groups as regard to the location of myocardial infarction nor to the functional class (according to Killip classification). Possible mechanism producing bradycardia, hypotension and lipothymia after nitrates administration are considered. A vagally mediate reflex possibly elicited by a fall in venous return is the most acceptable hypothesis. The study emphasizes the importance of carefull observation of patients receiving sublingual nitrates during acute myocardial infarction, and the rapid response of bradycardia, hypotension and lipothymia following nitrates administration, to a simple therapy, which avoids other potentially hazardous treatments.

[The mitral prolapse syndrome. Observation of 100 cases (author's transl)].

100 patients with mitral prolapse syndrome (MPS) were studied with electrocardiogram, phonocardiogram, thorax radiography and echocardiogram. In 44 of the patients, the effort test and dynamic electrocardiogram were performed; 25 underwent hemodynamic examination, and 10 underwent selective coronarography. The most frequent symptomatologies were palpitations and vertigo; the most-registered electrocardiographic alterations were ventricular extrasystoles and altered electrical repolarization in the inferior seat. The symptomatic patients with rhythm disturbances showed reduced work capacity (RWC) with aggravation of the effort arrhythmias. The phonocardiogram showed variability of the phonoauscultatory reports, not only from patient to patient, but also in the same patient. The apicocardiogram presented a characteristic systolic re-entry which coincided with the non-ejective click. In all cases, the echocardiogram demonstrated a systolic movement backwards from the mitralic strips with different aspects. The endocavitary pressures in the studied cases were normal or only slightly altered. The left ventriculography frequently demonstrated assynergic areas, and constantly, eversion of the posterior mitralic strips in the left atrium. In most of the cases, the selective coronarography showed the absence of the ventricular atrial tract of the circumflex. The sustained role of the mixomatous degeneration in the pathogenetic mechanism of the MPS must be emphasized. The beta-blockers were the most effective drugs against arrhythmias, which in some cases showed themselves impervious to any medicinal treatment. MPS is considered a very diffuse disease which can run its course without detection or which can be accompanied by serious rhythm disturbances that can threaten the patient's life. The importance of echocardiography in the diagnosis of MPS is emphasized. The SPM patient showed undergo a series of wave tests which reveal malignant potentialities, so that the best therapeutic principles, both for the treatment and prevention of the arrhythmias, can be applied.
