Teaching medical ethics symposium. Medical ethics in Manchester.

Manchester's multi-disciplinary approach to medical ethics combines established methods and new initiatives. There is a longstanding Medical Group and also, plans are evolving for the inclusion of medical ethics teaching in the undergraduate curriculum, the start of an MA in Health Care Ethics in October 1987 and the establishment of the Centre for Social Ethics and Policy to act as a focus within the university for research and study in a wider context.

[Comparison of the fluoride bioavailability from two oral preparations of monofluorophosphate disodium in combination with various calcium salts].

We studied, in twelve healthy volunteers, the pharmacokinetics of inorganic fluoride and calcium variations in serum and urine, parathormone variations in serum after administration of two oral preparations containing 100 mg of disodium monofluorophosphate (13.2 mg F as element) with different calcium salts (500 mg Ca as element). Fluoride was estimated in serum and urine with an ion specific electrode. The fluoride bioavailability from two preparations is identical with an areas under the curve corresponding to 61.05 and 62.53 mumols.l-1 h (after deduction of physiological fluoride concentrations) and urinary fluoride excretion after 72 hours corresponding to 266.6 and 246.1 mumols. The plasma peak appearance is rapid (one hour) and similar. The significant increase of urinary Ca-Creat ratio (70 to 100%) is identical four hours after drug administration. In the same way, a significant and early decrease of intact PTH in serum, measured with chemiluminometric method, was observed from two drugs. From these observations we may conclude that the two preparations are biologically equivalent.

Effects of arginine butyrate and tributyrylxylitol on cultured human sarcoma cells.

The effects of arginine butyrate and tributyrylxylitol were studied comparatively in a human sarcoma cell line. Both induced important structural and functional modifications suggestive of cell differentiation, as shown by the dose-dependent increase of alkaline phosphatase activity and total protein content, at concentrations ranging from 1mM to 5 mM expressed in butyrate equivalents. hCG beta-subunit present in the culture medium increased with differentiation. Our results show that most of the differentiation changes previously reported for sodium butyrate in cancer cell lines are also produced by both drugs. Tributyrylxylitol appears to be the more potent and effective inducer of differentiation but its use is limited << in vitro >> on account of its relative toxicity at concentrations above 3 mM butyrate equivalents.

Follow-up study of biological parameters in elderly institutionalized patients more than one year after discontinuation of calcium-vitamin D supplementation.

Laboratory tests were done 15 to 19 months after completion of a six-month clinical trial of oral supplementation with 1 g elemental calcium and 800 IU vitamin D per day in elderly institutionalized patients. Serum 25-OH-vitamin D and intact parathyroid hormone levels returned to normal during the trial, indicating efficacy of the supplementation in correcting the vitamin D deficiency and secondary hyperparathyroidism present before the trial. Data were available before, during, immediately after and 15 to 19 months after the trial in 37 patients. Recurrence of the vitamin D deficiency was observed after discontinuation of the supplementation.
