[Cancer mortality in France. New Aspects of the presentation and analysis of data].

In France mortality statistics are the only complete source of information on cancer. Investigation of the annual figures and the application of statistical methods to their presentation and analysis has facilitated fresh studies of geographical pathology, utilizing methods of analysis with simple and multiple correlations (analysis of correlations). Certain groupings were thus revealed, notably in the case of death from bronchial carcinoma, urinary bladder and gall bladder. Two groups of departements appeared the nearest to the national levels for deaths from cancer for all body sites.

Role of FK778 alone or in combination with tacrolimus or mTOR inhibitors as an immunomodulator of immunofunctions: in vitro evaluation of T cell proliferation and the expression of lymphocyte surface antigens.

We evaluated the in vitro capacity of FK778, alone or in combination with other immunosuppressive drugs: Tacrolimus (TRL); Sirolimus (SRL), Everolimus (EVL), to inhibit clonal expansion of T-lymphocytes and expression of lymphocyte-activation surface antigens; secondly, we compared the immunosuppressive potential of FK778 combined with TRL, SRL and EVL with the same combinations using Mycophenolic acid (MPA) as antimetabolite. Lymphocyte proliferation was assessed by 3H-Thymidine incorporation, in whole blood cultures stimulated with ConA. The effect of FK778 on alloresponse was evaluated by MLC and the expression of lymphocyte surface antigens by cytometry. FK778, TRL, SRL and EVL showed a high in vitro capacity to inhibit lymphocyte proliferation in a concentration-dependent way. Combinations of FK778 with TRL, SRL, or EVL presented an additive effect, especially FK778+TRL. Similar inhibition capacity of the clonal expansion was observed, when FK778 was combined with TRL, SRL or EVL, respecting the same combinations but using MPA instead of FK778. In addition, FK778 inhibited the expression of lymphocyte surface antigens involved in activation, co-stimulatory and apoptosis signals. In conclusion, FK778 inhibits the proliferative response induced by mitogeneic and allogeneic stimuli and the expression of surface antigens. Combinations of FK778 with TRL or mTOR inhibitors presented an additive effect and their action on T cell proliferation was similar to that of combinations with MPA. Since FK778, TRL and mTOR inhibitors present different action mechanisms and involve different cellular targets, these combinations may help prevent episodes of allorejection in organ transplants. FK778 and mTOR inhibitors may represent an alternative treatment for patients with renal failure.

[Is there any bimodality of the age distribution of male mammary cancer (author's transl)?].

The authors study the pattern of the curve concerning breast cancer incidence in males as a function of age. The demonstration is made that such a curve is bimodal and, more precisely, could be satisfactorily considered as being made up of the juxtaposition of two normal distribution curves. The interval thus individualized between the mean age of both groups is remarkably steady between 17 and 20 yrs. The phenomenon is not blurred by regrouping the data and the patients population could be divided into two subgroups: one with a mean age of 147 yrs, with a standard deviation of 9 yrs, and bearing on 246 patients, the other one including 1.100 patients averaging 66.5 yrs old with a standard deviation of 9.9 yrs.

Chemonucleolysis vs laminectomy.

Three hundred and nine patients having herniated nucleus pulposus syndrome were treated by chemonucleolysis with chymopapain. We analyzed the results of that treatment in the first 100 patients who had been followed up for two years. No significant allergic reactions, deaths, spinal cord injury or paralysis occurred. One patient had pulmonary embolus and another had a disk space infection. Diskitis did not occur. Of the 100 patients receiving chymopapain, 71% of the results were rated good to excellent; of those in a comparably followed-up group of 100 patients who underwent surgery, 63% of the results were rated good to excellent. The end result was achieved more quickly after chemonucleolysis and with less physiologic stress on the patient. The beneficial result was considered to be long lasting, and in our opinion, the quality of the end result was superior after chemonucleolysis.

[Prognostic histopathological features of malignant melanoma. New statistical approaches (author's transl)].

The records of 203 patients with cutaneous malignant melanoma treated at the Centre René Huguenin, Saint-Cloud, from october, 1959 to december, 1978 were reviewed. Histological data from 155 patients were retained after re-examination. The melanomas, which had previously been classified according to Clark's system of histological type and degree of invasiveness, were re-evaluated according to the thickness, were re-evaluated according to the thickness of the tumour as measured by Breslow's ocular micrometric method. Other histological data, as well as clinical and therapeutic data, were gathered together. One hundred and fifty cases were found to be suitable for standard single and multifactorial analysis, and 96 records contained sufficient information to be analyzed by the new "similarly aggregation" method developed by IBM. Two significant results emerged from the study: the thickness of the tumor is the single most important prognostic factor, as it correlates with the incidence of recurrences, metastases and deaths; by using a small number of clinical and histological variables, it seems possible to draw a profile of every new patient, to classify him in a well-determined prognostic category and, consequently, to institute a complementary treatment when needed.

Ultrastructural study of mouse thymus virus replication.

With the exception of thymocytes, no other cell types have been reported to be involved in mouse thymus virus (MTV) infection. The ultrastructure of thymuses of mice infected with MTV were examined. The earliest sign of infection was detected 5 days p.i.; lymphocytes, epithelial and phagocytic (macrophages) reticular cells were shown to be affected. Viral particles and filamentous structures were present in both the nucleus and the cytoplasm of these cells. At more severe stages of cellular necrosis, 6 and 7 days p.i., cytoplasmic granulation as well as loss in definition of cytoplasmic organelles became apparent. This was followed by nuclear degradation and aggregation of cells. After 9 days p.i. necrotic lesions were still observed but viral particles were no longer detectable. This study provides evidence of the susceptibility of macrophages to MTV.
