[Small retinal, cochlear, and cerebral infarctions in the young patient, "SICRET" syndrome of Susac syndrome].

A 22-year-old-lady presented with multiple occlusions of the branches of the central retinal artery, accompanied by neuro-encephalic disorders and deafness. This triad is known as SICRET Syndrome (Small Infarction of Cochlear, Retinal and Encephalic Tissue). This rare syndrome, as well referred to as Susac syndrome, affects only the women and the three tissues mentioned above: eye, ear, brain. The course was characterised by a series of partially regressive evolutive steps. A remission had been obtained since two years with immuno-supressor and anti-coagulant therapy. The neuro-encephalic and cochlear disorder regressed in contrast to the severe sequel on the right eye.

[Chorioretinitis in congenital toxoplasmosis].

The goal of this study was to establish the incidence of chorioretinitis in 100 infants whose mothers presented a seroconversion during the pregnancy. It is a retrospective study over 6.5 years. There were 17 cases of latent congenital toxoplasmosis (56.7%) and 13 cases of clinical congenital toxoplasmosis (43.3%). Three infants presented with chorioretinitis: one at birth in a context of general stroke, the two others at 2 and 17 months after birth, despite treatment. The risk of retinochorioditis was the same in the case of latent or clinical congenital toxoplasmosis. The fall in the antibody titre was not a good criterion of cure; the percentage of lymphocytes in the cerebrospinal fluid could constitute a better criterion. This study confirmed the efficacy of systemic treatment of congenital toxoplasmosis.

[Pneumatic retinopexy using SF6 or C3F8. Results and complications apropos of 56 patients].

56 patients with primary retinal detachment were treated by pneumatic retinopexy. The overall success rate for reattachment with one operation was 66%. Postoperative complications included development of new tears (18%) and P.V.R. (16%). Pneumatic retinopexy is a valuable new technique, however careful patient selection and postoperative management is required.

[Early endothelial complications after treatment using a neodymium-Yag laser].

The authors analysed the corneal endothelium after Neodymium-Yag laser treatment. The Visulas-Yag laser was used. The endothelial study was performed by the specular microscopy (Pocklington). The study included 46 patients divided into 2 groups: in the first group, 33 eyes had a capsulotomy (17 aphakic and 16 pseudophakic): in the second group, 13 eyes had an iridotomy (3 pseudophakic and 10 phakic). In the group of capsulotomies, the energy used for the impact was 2.6 +/- 0.7 mJ and the total energy was 110 +/- 80 mJ. The cellular loss was 7.4% on the third day and 6.5% by the third month. In this series the cellular loss of aphakic patients was 8.1% on the third day and 5.9% by the third month. In the pseudophakic patients, the loss was 6.5% on the third day and 7.3% by the third month. There was a significant correlation between the endothelial cell loss and the total energy delivered (r: 0.42: p less than 0.02). In the group of iridotomies, the energy for each impact was 3 +/- 1.3 mJ and the total energy was 118.7 +/- 92 mJ. The cellular endothelial density decreases to 7.4% the third day and to 3.8% by the third month. There was no significant correlation between the endothelial cell loss and the total energy delivered. Immediately after the treatment by Neodymium-Yag laser, there were round dark endothelium wounds near the treated zone. They may put into question the functional prognosis by the treated eye.

[Uveal effusion syndrome. Apropos of a case].

Uveal effusion syndrome is a rare entity. It associates a nonrhegmatogenous retinal detachment, an annular cilio-choroïdal detachment and peripheral vascular anomalies. It is important to differentiate it from the another retinal detachments and the malignant melanoma.

[Endothelial morphometry and perforating keratoplasties].

50 corneal grafts were studied by specular microscopy. The endothelial cell morphology of the grafts was analysed by using a computerised image analysis system. The endothelial cell density decreased rapidly during the first three post-operative months (17%). It continued to decrease for the first year (52%, after which cell loss occurred at a slower rate (3% of the sample cell count per year). The coefficient of variation in cell area remained constant (27%). The percentage of endothelial cell loss might be closely associated with the reestablishment of the hexagonal cellular pattern. These results suggest that the endothelium of the graft is in a state of transition during the first year after surgery.
