Pharmacokinetics of a sustained-release dosage form of clomipramine.

In this study, the pharmacokinetic parameters of the sustained-release (SR) dosage form of clomipramine (CMI) were compared with those obtained after the administration of the immediate-release (IR) dosage form. Two studies were performed. In the first study, a single oral dose of both products was administered in 12 healthy volunteers, and in the second study, multiple doses of both products were administered in 6 patients with depression in which steady-state plasma levels (Css) were determined. Plasma levels were assayed using an HPLC method. In the single-dose study, the mean Cmax and AUC values of CMI were 63.37 ng/mL-1 and 1375.38 ng.h/mL-1 for the reference product and 32.55 ng/mL-1 and 1285.26 ng.h/mL-1 for the test product, respectively. The mean beta and MRT values of CMI were 0.030 h-1 and 47.21 h for the reference product and 0.026 h-1 and 55.63 h for test product, respectively. Results showed that after the multiple-dose study, the mean clomipramine plus demethylclomipramine values of Cavss were 406.2 ng/mL-1 for the reference and 328.6 ng/mL-1 for the sustained-release dosage form. This product also presented less fluctuation in steady-state plasma levels (1.08 vs. 1.23). The values of MRT and tmax were higher for the SR dosage form, showing that the drug was maintained longer in the body. The mean values for the ratio metabolite/drug were 2.06 and 2.41 for the IR and SR dosage forms, respectively. Neither AUC nor elimination half-life was affected significantly after the administration of the sustained-release dosage form.

Bioequivalence study for diazepam commercial tablets in psychiatric patients.

The bioequivalence of three different commercial diazepam tablets (5 mg) using the innovator product (Valium, Roche) as a reference was evaluated in eight psychiatric patients, following a multiple-dose regimen (5 mg t.i.d.). Each patient received the four treatments sequentially according to a Latin square design, with balance for first residual effect. Blood samples were taken once the steady state of the drug had been reached, and the plasma levels were determined using a HPLC method. No statistical differences were found in diazepam steady-state levels after the administration of the four formulations, indicating that they could be considered equivalent.

Influence of ouabain of electrolyte and water transport in the rabbit ileum and colon in vitro and in vivo.

We studied the influence of the Na(+)-K+ pump inhibitor ouabain on water and electrolyte transport in the rabbit distal ileum and proximal colon in vitro and in vivo. Under in vitro conditions, ouabain markedly reduced the absorption of water, sodium, chloride and bicarbonate, and reduced potassium secretion in both the ileum and the colon. In vivo results were similar but less marked than those obtained in vitro, with significant differences only at the level of the distal ileum.

A comparative study of electrolyte and water transport in the rabbit ileum and colon in vitro and in vivo: influence of D-glucose.

The influence of D-glucose on electrolyte and water transport in the distal ileum and proximal colon of the rabbit in vitro and in vivo has been studied. The magnitude of electrolyte and water transport was found to differ in vitro and in vivo. The replacement of mannitol by D-glucose in the intestinal lumen had no effect on ileal and colon transport of the ions studied, i.e., movement of the ions was independent of the supply of exogenous glucose. The distal ileum and the proximal colon of the rabbit in vivo showed a net secretion of bicarbonate, aimed at neutralizing the cecal contents.

A comparative study of bicarbonate secretion by ileum and colon of Oryctolagus cuniculus. Influence of caecal pH.

The influence of caecal pH on movements of bicarbonate ion in rabbit colon and ileum has been studied. A net secretion of bicarbonate is noted in both intestinal segments (at higher rates in colon than in ileum) in our experimental conditions. The bicarbonate secretion rate is lowered when bicarbonate is added to caecum. The introduction of acetic acid at caecal level increased bicarbonate secretion rate both in ileum as in colon.

Acetates↗

Active secretion of bicarbonate by rabbit colon.

A serosa-mucosa movement of bicarbonate against concentration gradient was detected at this intestinal level. This movement is partially dependent on oxidative phosphorylation since it is abolished by DNP administration, but as it is not affected by acetazolamide administration it can be supposed as dependent on carbonic anhydrase activity.

[A method to quantify diazepam with high resolution liquid chromatography in patients undergoing multiple therapy].

A rapid, sensitive and specific method useful in monitoring diazepam in patients receiving multiple therapy is presented. Plasma was extracted in the presence of borate buffer with hexane and the drug was quantified by HPLC in a Novapak C 18 column. The limit of detection in plasma was about 30 ng/mL. Analytical recovery of the drug added to plasma was about 70%. Within-day variation and between-day variation ranged from 5.9 to 8.3 and 5.9 to 10.7%, respectively. The method was applied in the determination of diazepam levels in patients treated with this drug in combination with other drugs commonly used in our hospital. The concentrations found in most of the patients were in the therapeutic range. It was not possible to detect the drug in one patient with concomitant administration of magnesium hydroxide, which can be attributed to a reduced absorption of diazepam in presence of the antiacid. Average steady-state concentrations of diazepam in psychiatric patients, submitted to 5 mg doses t.i.d. during four weeks, were in the range of 200 to 450 ng/mL.
