T-cell depletion does not affect long-term human bone marrow culture.

Bone marrow long-term cultures were studied for 8 weeks in four experiments before and after T-cell depletion with a mixture of monoclonal antibodies directed against CD2, CD5 and CD7. No differences in cellularity or granulomonocytic progenitor numbers appeared during the period of culture. We conclude that in vitro treatment in itself is not likely to explain non-take or graft rejection associated with T-depleted bone marrow transplantation.

Persistence of bone marrow lymphocytosis after induction treatment in common acute lymphoblastic leukemia. Marker analysis and significance.

In a series of 22 adult acute lymphoblastic leukemias expressing the common acute lymphoblastic leukemia antigen (cALLA +), eight had marrow lymphocytosis greater than 30% (43 +/- 12%) with disappearance of marrow lymphoblasts and correction of bone marrow insufficiency at the end of the induction treatment. Four of these patients were studied for the cALL antigen at this time and had persistence of this immunological marker (62 +/- 15% of the mononuclear bone marrow cells). The evolution of these patients suggests that these patients were not in complete remission and emphasizes the usefulness of cALLA determination in cases with borderline excess of marrow lymphocytes.

[Expansion of erythropoiesis and granulopoiesis after bone marrow grafts].

Erythropoietic and granulopoietic expansion after bone marrow transplantation (BMT) are reviewed in 60 allogeneic and autologous BMT. Morphological dyserythropoiesis was more prominent than dysgranulopoiesis. Dyserythropoiesis was present with increased HbF synthesis and i antigen expression. We did not find nocturnal paroxysmal hemoglobinuria and leukocytic alkaline phosphatase level was high. CFU-GM progenitors were still reduced in number 1 year after BMT. The clusters/colonies ratio, increased at day 12 after BMT was normal by day 100. No difference between autologous and allogeneic BMT was observed.

[Acute nonmyeloid leukemia in adults. Study of membrane antigens and terminal transferase. Diagnostic and prognostic value].

Between January, 1982 and April, 1983, 92 adult patients with acute leukaemia were investigated in our department. According to classical criteria (cytology and optical cytochemistry), 34 were classified as "non-myeloid". These were further tested with a panel of monoclonal antibodies against cell membrane, including ALB1-2 (anti-CALLA), ALB6 (anti-p24), ALB7-8-9 (BA1), OKT and HLA DR; they were also tested for E rosettes, Slg and terminal transferase (TDT). When all these markers but HLA DR were negative, patients were investigated for ultrastructural peroxidases which were found to be present in 2 cases. Among all non-myeloid leukaemias, 18 (56%) were CALLA-positive and TDT-positive acute lymphoblastic leukaemias (ALL), 2 (6%) were ALL T, 7 (22%) were CALLA-negative and TDT-positive ALL and 5 (16%) were acute leukaemias null for our markers, a phenomenon the significance of which is discussed. Patients with the CALLA-negative TDT-positive phenotype were peculiar with regard to age (mean: 50 years), female predominance, L2 cytological pattern according to the FAB classification, good prognosis (complete remission in 100% of the cases) and median survival (p less than 0,03).

Prognostic value of cytologic parameters in acute myelogenous leukemia.

The prognostic value of some current cytologic characteristics was assessed in 174 adult patients with acute myelogenous leukemia (AML) treated according to the AML-5 protocol of the EORTC Leukemia and Hematosarcomas Group. A significantly higher rate of complete remission (CR) was observed in patients with low bone-marrow (BM) cellularity, with BM blasts less than 80%, and with Auer rod positive cells more than 2.5% of the total blast cell population. A leukocyte count of less than 50 X 10(9)/1 in the peripheral blood was also associated with a higher CR rate. No significant difference was found between the various French-American-British (FAB) subtypes, in spite of a trend towards a lower CR rate in patients with an M1, myeloblastic, poorly differentiated, subtype. The leukocyte count and the percentage of Auer rod positive cells were the only significant parameters for duration of survival from the beginning of maintenance treatment. However these features had no prognostic value for the duration of remission. It seems therefore that patients with a higher percentage of Auer-rod-positive cells and lower peripheral leukocyte counts can enter remission more easily, and can also more readily achieve subsequent remissions following relapse. The prognostic value of these routine cytologic features is probably related to their relationship with proliferative activity and tumor burden: the percentage of Auer-rod-positive cells correlates inversely with the leukocyte count, whereas leukocyte count, BM cellularity, and percentage of BM blasts are linked together.

[Biermer's disease without anemia, nor megaloblastosis. Value of the study of antibodies, vitamin assays, and the dU suppression test for the early diagnosis].

In two patients, a diagnosis of pernicious anemia was made without anemia, megaloblastosis and even macrocytosis. The diagnosis of pernicious anemia was suggested by the autoimmune abnormalities which are frequently associated with this disease: goitre with anti-thyroid and anti parietal cell antibodies, and besides this, idiopathic thrombocytopenic purpura in the first patient, vitiligo in the second. The morphological abnormalities were limited to slight macrocytosis in the first patient and to hypersegmentation of polymorphonuclear leucocyte in both. Vitamin B12 deficiency was demonstrated by serum assay as well as deoxyuridine suppression test ("dU suppression"). The diagnosis was confirmed by demonstration of atrophic gastritis, failure of secretion of Intrinsic Factor and Schilling test. These observations show that the Addison-Biermer disease can be detected early in persons at high risk by looking for vitamin B12 deficiency and specific antibodies.

[Ultrastructural peroxidases and immunological markers in acute adult leukaemia: therapeutic applications (author's transl)].

In a retrospective study of 112 adult patients with acute leukaemia (AL), the percentage of undifferentiated leukaemia was reduced to almost 1% by an analysis of routine cytochemical reactions (PAS, peroxidase, esterases and esterase inhibition), B and T lymphocyte markers (IgS, E-rosettes, HuTLA) cAll antigen and ultrastructural detection of peroxidase activity (PO-ME). A simultaneous study of cALL antigen and PO-ME showed reciprocal exclusion of these markers, except in one case of mixed leukaemia. Therapeutically, the value of these tests lies in that patients with typical PAS reaction and cALL antigen consistently respond to vincristine- corticosteroid treatment, whereas patients without cALL and with PO-ME are frequently resistant to that combination of drugs.

[The prognosis factors of subacute and chronic myelo-monocytic leukemias (author's transl)].

The myelo-monocytic leukemias (MML) can be of acute, subacute or chronic type, according to the level of bone-marrow blast cells and to the spontaneous course. We have compared the clinical, hematological and biological characters of 11 cases of subacute MML-defined by a survival of less than 12 months - to 20 cases of chronic MML. Anemia, hyperleucocytosis, monocytosis, number of circulating granulocytes and blast cells, level of bone marrow blasts, were superior in acute MML, with significant difference for hemoglobin, circulating and bone-marrow blasts. A high urine lysozyme output, a decrease of granulo-monocytic colonies after bone-marrow culture on semi-solid media were further arguments in favor of the subacute type. This variety, which evolves into acute MML in near 50 p. cent of cases appears consequently as the first step of an acute leukemia, or represents a very close aspect. Chronic MML on the contrary, although with still 30 p. cent blastic transformation, is characterized by a steady course, with a median survival of 3.3 years, and number of untreated patients surviving many years beyond this median time.

Lymphoblastic lymphoma/leukemia with convoluted nuclei: the question of its relation to the T-cell lineage studied in 13 patients.

This work is devoted to the analysis of the nature of lymphoblastic lymphoma/leukemia with convoluted nuclei which were initially described by Barcos and Lukes. Ultrastructural, cytochemical, and immunologic features of tumor cells were investigated in patients chosen according to known morphologic criteria. Through results of the E rosette test, the patients were divided into two groups (E+ and E-). In the E+ group, the predominant features were sex (only men), the mediastinal localization, and the focal positivity of the acid phosphatase reaction. Cytotoxicity tests with rabbit antihuman T-lymphocyte anti-serum confirmed the results of the E rosette test in the 3 patients of the E+ group who were tested and were also positive in 2 patients from the E- group (1 of these 2 patients had the characteristics found in the E+ group and can thus be related to this group; the other patient had none of these characteristics). This raises the question of a leukemia arising from a less differentiated T-cell but this interpretation is limited by the specificity of the anti-T-serum. Ultrastructural study defines more precisely the convoluted aspect but does not at present allow a distinction between the two groups.

[Diagnostic and prognostic value of the assay of lysozyme in acute leukemia (author's transl)].

The diagnostic and prognostic value of the assay of lysozyme in serum and urine was appreciated in 184 cases of acute leukemia. The levels were decreased in the lymphoblastic, mainly of the non B-non T type, and undifferenciated varieties, markedly raised in the monoblastic and myelo-monocytic varieties, while in the myeloblastic ones they were found normal, decreased or slightly increased, and, on the average, significantly higher in the well differenciated than in the poorly differenciated types. For a given cytological type, the level of lysozyme is not correlated with the frequency of the induction of complete remission. However, in the acute myeloblastic leukemia, a significantly higher frequency of infection during or after the induction treatment was observed in the cases presenting initially without a raised serum lysozyme level.

Cell volume measurements in acute leukaemia: method and value for diagnosis and prognosis.

Cell volume was measured using a Coulter counter in 42 cases of adult acute leukaemia, before any therapy. In every patient, the distribution was log-normal. A breakdown of the results with regard to the cell-type gives the following informations: The mean modal volume (M.M.V.) differs significantly from one type of leukaemia to another: the cell size in the myeloblastic type is larger than in the lymphoblastic, and smaller than in the monoblastic and myelomonocytic types. The dispersion of cell volumes, for a single patient, is low in acute lymphoblastic leukaemia, wider in myeloblastic cases and is even larger in myelo-monocytic patients. In 9 of the 10 acute myelo-monocytic leukaemias studied, the existence of two populations was shown by a double peak. Once relapse occurs, the cell volumes seem greater than during the first phase of the disease, at least in acute lymphoblastic leukaemia. No correlation has been found between the mean modal volumes and the labeling index, after incorporation of 3H-TdR.

[Contribution of cytochemistry and delayed hypersensitivity skin tests in the diagnosis and prognosis of chronic lymphoproliferative syndromes].

Cytochemistry is disappointing in lymphoproliferative syndromes for it does not permit one to classify the various diseases with certainty. In the early stages, if the three indices are lowered, the prognosis seems poorer. A study of glucuronidase permits, in hyperlymphocytosis, one to differentiate benign from malignant lymphocytes, but does not permit one to differentiate from one another, the other chronic lymphoproliferative syndromes. The acid phosphatase is interesting in the study of hairy cell leukemia. Finally, it was not possible to distinguish chronic lymphoid leukemia from leukemia with lymphosarcomatous cells, nor from the cytochemical point of view nor using tests for delayed hypersensitivity.

[Cytological classification of adult acute leukemia (author's transl)].

134 cases of acute leukemia in adults were classified according a "double blind" cytological diagnosis, a cytochemical study, and lysozyme assay. Each type has distinct characters, allowing a good nosological definition. The histochemical methods, as well as lysozyme assay are usually unnecessary, particularly in well differenciated myeloblastic, promyelocytic or myelo-monocytic acute leukemia. They are, on the contrary, frequently useful in poorly differenciated myeloblastic or monoblastic leukemia. But, they cannot help to solve every problem in cytological diagnosis: there are still 10% of undifferenciated acute leukemia, the lymphoblastic acute leukemia are not clearly defined and 4% of cases of acute leukemia have atypical characters leading to difficulties in cytological classification. The need for new methods and markers is emphasized.

[Waldenstrôm's macroglobulinemia. Developmental and prognostic study].

The course of 27 cases of Waldenström's macroglobuilinemia was observed during at least 2 years or until death. A worsening occurred in half of the patients, with 7 deaths before 2 years. 10 patients improved under therapy, with only two deaths later. The more frequent complications were infections, but other complications, haemorragic, neurologic, osseous and renal were also observed. A haemoglobin initial level higher than 9 g % and a significant decrease of lymphocyte blood count under chemotherapy seem to be of good prognosis.

[Sarcomatous and leukemic forms of Waldenström's macroglobulinemia].

Four cases of sarcomatous or leukaemic types of Waldenström's macroglobulinemia are presented. They are characterized by tumoral and compressive localizations of lymph node or spleen, or by a hyper-leukocytosis with many circulating abnormal cells. These cells are different from the lympho-plasma cells regularly observed in Waldenström's macroglobulinemia, and can be assimilated to malignant immunoblasts. They have a proliferative aspect and seem to produce less macroglobulin and to recover some beta-glucuronidase activity. The sarcomatous and leukemic types of Waldenström's macroglobullinemia have a poor prognosis and may appear as terminal transformation of the disease.

[Culture of human bone marrow. Counts of colony forming granulocytes and macrophages in 100 patients without hematologic diseases].

Bone marrow suspensions from non hematologic patients have been cultured in methyl cellulose, with conditioned medium prepared with normal peripheral blood cells. The granulocytic-monocytic colony forming cell (G-CFC) can give rise to colonies, which were counted on the 14th day. The mean G-CFC number is 38 +/- 26 when 2.10(5) cells are cultured. The striking disparity from one patient to an other cannot be explained by the culture conditions, nor by the clinical status. There is no sex difference; older patients have a significant reduction of the colony numbers compared to younger patients.
