Acute and chronic effects of cadmium have been widely described for different aquatic organisms and exposure routes. However, there is clearly a lack of information on the potential of cadmium to cause genotoxic effects. This work presents genotoxic and nongenotoxic parameters analyzed in cadmium-exposed rainbow trout. The assessment was performed for sublethal levels after long-term exposure using six intraperitoneal injections of 0.5 mg/kg (Day 1), 1 mg/kg (Days 3, 7 and 11), and 2 mg/kg (Days 15 and 19) to allow precise estimation of the dose. Cadmium accumulation in target tissues, essential metal mobilization by cadmium at the subcellular and tissue levels, and induction of metallothioneins were selected as exposure and effect parameters. Induction of micronuclei and variation in DNA content (expressed as variation coefficient in the G1 phase of the cell cycle) in blood cells, determined by flow cytometry, were selected as biomarkers for genotoxic effects. Cadmium accumulation, induction of metallothioneins, and mobilization of essential metals at the subcellular level were observed in different organs in response to cadmium exposure. The highest metallothionein induction was observed in liver, reaching 270+/-90 nmol/g wet tissue in treated fish versus 2.68+/-1.1 nmol/g wet tissue in controls. The highest cadmium accumulation was also observed in the liver (27.8+/-9.5 microgram Cd/g wet wt in treated animals versus 1.0+/-1.7 in the control group). However, no genotoxic effects were observed in blood cells. The frequency of micronuclei was 0.012+/-0.008 for the control group and 0.013+/-0.021 for treated animals. The variation coefficient of G1-phase nuclei was 3.61+/-0.66 and 3.22+/-0.29 for control and cadmium-exposed groups, respectively. Thus, it is concluded that under the experimental conditions employed here, treatment of rainbow trout with cadmium chloride at doses that produce significant toxicological alterations at the tissue and subcellular levels does not provoke observable alterations in the genotoxic parameters considered in this study.
The paper gives a brief overview of a wide spectrum of health issues and problems, ranging from communicable disease to mental health and family formation, which affect migrants and host countries.
OBJECTIVE: The initial phases and transitional periods of sleep facilitate electroclinical manifestations of juvenile myoclonic epilepsy (JME). The period at the time of waking is identified in the different phases of sleep by cyclic electrophysiological oscillations, which in turn are synchronous with the spike-and-wave or multiple spike-and-wave activity of the electric crises seen in JME and other epileptic syndromes. This observation supports the theory of an alternating cyclical pattern (ACP) as the trigger of these discharges. PATIENTS AND METHODS: Following classical macrostructural and microstructural patterns of EEG sleep analysis, we investigated electrophysiological behavior in 13 patients diagnosed as having JME, and 5 persons of homogeneous epidemiological characteristics as controls. RESULTS: In our study we observed that both groups had similar macrostructural sleep parameters. However, in patients prone to crises, these were concentrated in the first two hours recorded and showed no differences in sleep-waking transition or ACP of phase I sleep. These were significant when both periods were compared with the remaining NREM sleep. CONCLUSION: We conclude from this study that ACP is the microstructural element of sleep which modulates and permits classification of epileptiform anomalies, mainly in studies at the time of wakening.
To determine the nature of the mechanism by which certain derived ruthenium (Ru) complexes induce regression in tumour growth, we have investigated the possibility that this mechanism was associated with an increase of superoxide anion (O2-. production by phagocytic cells, which are usually found in tumour nodes. Here we present evidence that a newly synthesized complex, Ru3+-propylene-1, 2-diaminotetra-acetic acid (Ru-PDTA), derived from Ru and the sequestering ligand (PDTA), specifically stimulates O2-. production. This increase was associated with the translocation of cytosolic factors p47(phox) and p67(phox) of NADPH oxidase to the plasma membrane. The Ru-PDTA-complex-dependent O2-. production was abrogated by staurosporine, partially inhibited by diphenylene iodonium, and it was insensitive to pertussis toxin or dibutyryl cyclic AMP pretreatment. An increase of cytosolic Ca2+ levels were also detected in neutrophils treated with the Ru-PDTA complex. Also, Ru-PDTA complex induced the phosphorylation of tyrosine residues of several proteins as assessed by Western blotting. Present data are consistent with the possibility that Ru-PDTA-dependent antitumour effects are due in part to the complex's ability to stimulate the release of toxic oxygen metabolites from phagocytic cells infiltrating tumour masses.
Part of the impact of the war in ex-Yugoslavia and especially Bosnia and Herzegovina was to limit the supply of therapeutic drugs they had used before the war. The difficulties encountered made the health care system temporarily dependent on humanitarian assistance agencies which applied the concept of essential drugs; and, after initial difficulties, national health staff adapted to the need to prescribe from a very limited range of drugs. Meanwhile, national drug policy and procurement and prescribing practices were reviewed by working groups and a national List of Essential Drugs was drawn up by national experts with international support. This list has now been passed into legislation.
We report the case of a patient with nocturnal paroxysmal dystonia (NPD) meeting not only the diagnostic criteria described by Lugaresi et al. in 1981, but also presenting with features suggestive of underlying epilepsy as well as other differences from the diagnostic criteria originally described for NPD, such as short- and long-lasting episodes and poor response to antiepileptic drugs. The case we describe suggests a different entity, one that is unrelated to epilepsy or true NPD.

It has been demonstrated that neutrophils from healthy donors or from patients with inflammatory disorders can bind immunoglobulin (Ig) E proteins through binding to Mac-2/epsilon bp. Functional responses to allergens were assessed by measuring the respiratory burst and intracellular Ca2+ levels, and binding of allergens to neutrophils was assessed by flow cytometry analysis and fluorescence microscopy. In this article, we demonstrate that neutrophils sensitized to specific allergens (from allergic patients), but not from healthy donors, are sensitive to allergens of the same type as those that produce clinical allergic symptoms. The activation of neutrophils was analyzed by the induction of a respiratory burst that was detected with luminol-dependent chemiluminescence. Intracellular Ca2+ levels increased parallel to those of the inducing allergens. In addition, the specific binding of allergens on the cell surface was revealed by flow cytometry and allergen-FITC-labeled staining analyses. The present data suggest a restricted recognition of allergen by sensitive neutrophils, probably associated with the specific binding of the allergen to its corresponding IgE molecule, which is bound to the Mac-2/epsilon bp structure. These findings demonstrate a functional role of allergen-associated neutrophils during the allergic state.
Mutations in the rhodopsin gene have been sought in a family with autosomal dominant retinitis pigmentosa. Screening for mutations in the rhodopsin gene was carried out by polimerase chain reaction and denaturant gradient gel electrophoresis. Direct DNA sequencing was performed for the characterization of punctual mutations. A base substitution in the exon 2 of the rhodopsin gene was detected. Direct DNA sequencing revealed a CGC to CTG change in codon 135, that substitutes arginine for leucine residue in rhodopsin. The mutation segregates with the disease phenotype in the family. The mutation Arg-135-Leu causes the retinitis pigmentosa phenotype in the family, where the disease is inherited following an autosomal dominant pattern.

A large family affected with autosomal dominant retinitis pigmentosa (ADRP) with a sectorial phenotype showed a previously described (G to A) mutation in the rhodopsin gene resulting in the substitution of a glycine residue by an arginine in codon 106 of rhodopsin. This mutation shows some unusual characteristics, such as initial pathology of the inferior retina, superior visual field with normal disc and retinal vessels, and ERG findings that show a modest reduction in both cone and rod amplitudes with normal implicit times. The Gly 106 Arg mutation has been previously reported in American and British patients. Its presence in a Spanish ADRP family confirms that it and its homogeneous associated phenotype are geographically widespread.
The need to migrate is usually a function of the complex interaction of economic, social, familial and political factors. Among the most important, however, are the denial of access to education, employment, goods and services and the lack of respect for basic human rights. Because in many societies women are marginalized from these rights, migration to more economically and educationally open societies can often help improve their personal situation and their professional opportunities. On the other hand, because the status of women is usually linked to their role and status within the family and is defined in relationship to their male partners, migration can place women in situations where they experience stress and anxiety due to the loss of their traditional social entourage and environment. Their social integration in new settings may be equally limited by their initial lack of education and occupational experience. The higher vulnerability of women to sexual abuse and violence also places them at risk of STDs, including HIV, and a range of post-traumatic stress disorders associated with sexual violence. Their reproductive health needs often go unnoticed and unprotected even in well organized refugee and migrant situations, and the insensitivity of health staff to the needs of women is often more pronounced in refugee and migrant contexts than it is in general. Health monitoring of women in all migration-related situations has to be given greater priority. Similarly, much more attention at a health policy level is called for if the rights of women refugees and migrants are to be protected, and their contribution to health and social development is to be acknowledged and promoted.

The effects of toxic exposures on the susceptibility of rainbow trout (Oncorhynchus mykiss) to saprolegniosis were evaluated. Fish were exposed to sublethal concentrations of copper (0.25 mg/liter), cyanide (0.07 mg/liter), ammonia (0.5 mg/liter), and nitrite (0.24 mg/liter) for 24 h. After exposure, the fish were challenged by Saprolegnia parasitica (3.6 x 10(sup6) zoospores per liter) for 10 min. Cortisol and cholesterol were used to indicate stress response. Similar increases of cortisol were found for the four tested chemicals. All fish with cortisol levels higher than 370 ng/ml developed the disease, while only 24% of the fish with cortisol levels lower than 370 ng/ml were infected. Cholesterol levels remained unchanged after toxic exposure. Increased susceptibilities to the pathogen were observed for ammonia (71%), copper (57%), nitrite (50%), and cyanide (33%). The increases in susceptibility as a result of cyanide and nitrite exposure could be explained by the stress response. For copper and ammonia, the combination of two different effects, the stress response and specific impairments of the defense mechanism of trout against saprolegniosis, should be considered.
A combination of DNA macrorestriction analysis using pulsed-field gel electrophoresis and a serotyping method using three panels of monoclonal antibody was used to discriminate 43 epidemiologically unrelated Neisseria gonorrhoeae isolates requiring proline, citrulline, and uracil (PCU-) into 35 groups. This indicates that PCU- isolates of N. gonorrhoeae are not clonal.
