Evaluation of the role of surgery in 130 patients with neuroblastoma.

The role of surgery for children with neuroblastoma was evaluated by using a recently proposed TNM staging system. One-hundred thirty patients were retrospectively assigned a TNM clinical stage (CS) preoperatively and a pathologic stage (PS) postoperatively. Patients with CS 4 were separated into CS 4A and CS 4B according to their age and pattern of metastases. Patient survival was analyzed according to CS, age, location of primary, and PS. Actuarial survival of patients was as follows: CS 1, 100%; CS 2, 82%; CS 3, 63%; CS 4A, 50%; and CS 4B, 5%. For all stages, patients younger than 1 year old survived longer than those older than 1 year (72% v 32%). Prognosis for CS 1 was the same regardless of age. For CS 2 and CS 3, patients younger than 1 year old lived longer. CS 4A had better survival than CS 4B. Survival by site was 100% for cervical, 62% for mediastinal, 45% for pelvic, and 36% for retroperitoneal primaries. The role of surgery was evaluated by analyzing survival according to the postoperative PS. PS 1-2-3 A were regarded as satisfactory resections since all macroscopic tumor was removed. PS 3B as a debulking procedure, and PS 3C as an unresectable lesion which was biopsied. Patients with nonmetastatic disease (CS 1-3) with PS 1 and PS 2 disease had a 100% survival rate; PS 3A, 93%; PS 3B, 58%; and PS 3C, 21%. This proves the value of total resection in nonmetastatic disease. The role of surgery could also be proven in metastatic disease.(ABSTRACT TRUNCATED AT 250 WORDS)

[Esophageal plasty in children. Retrospective study of 22 colonic transplants. Current orientation].

Esophageal replacement using transverse colon with left colic pedicle is reviewed. 18 were done for caustic stenosis with one death, 4 for atresia without mortality. For caustic stenosis the authors advocate early transplant since the third month if dilatations failed. Improvement of technique using staplers prevented any complication in the last 8 patients. These good results allowed the authors to switch to isoperistaltic gastric tube. In esophageal atresias, the authors did not find any indication of replacement in the 12 last years.

Management of Hirschsprung's disease. The definitive operation: which, when, why and how.

Analysing the results of 34 infants operated upon from 01.01.77 to 12.31.82, the authors advocate primary corrective surgery without colostomy in the first 3 months of life. Medical treatment is based on total parenteral nutrition (TPN) and enemas. Surgery using any of the three major techniques (Swenson, Soave and Duhamel) is performed after 1 month of oral feeding and maternal care. Results with a follow-up ranging up to 6 years showed excellent defaecation control and short duration of hospitalization.

Surgical treatment of organic pancreatic hypoglycemia in the pediatric age.

From 1974 to 1980, 6 patients were operated upon for organic hyperinsulinism. 218 patients reported in the last 5 years are studied. Age at onset of symptoms is the most important factor. In infants the diagnosis is urgent. A severe mental retardation can result from waste of time in diagnosis and treatment. Simultaneous measurement of blood glucose and insulinemia during fasting and feeding periods is sufficient. Diffuse lesions are frequent, even associated with Islet cell adenoma. Surgery based upon local portal insulinemia must be adequately large and should avoid repeated pancreatectomy. In older children the diagnosis is easy. When a localised lesion is the pathologic feature, after a complete exposure of the gland, local excision is the technique of choice.

[Effectiveness of alizapride against emesis in infants and children (author's transl)].

The effectiveness of alizapride against vomiting was studied in a group of 40 patients including neonates, infants and children. Patients were hospitalized in a surgical ward. 2 to 8 mg/kg/24 h were given orally. Vomiting ceased in 75% of the cases. Optimal results were obtained with 4 to 5.6 mg/kg/24 h. With alizapride symptoms abated in all patients. Operative conditions were improved for those who needed surgery. In addition, nausea was relieved in 100% of cases and regurgitations in 67.7%. Our study shows that alizapride is a reliable drug with a short time lag and no side-effects. We conclude that it can be used against vomiting in pediatric patients.

Primary corrective operation without decompression in infants less thn three months of age with Hirschsprung's disease.

From Jan 1, 1965 to Nov 1, 1980, 155 infants with Hirschsprung's disease have been treated. All had confirmed aganglionosis at multiple rectal biopsies. From 1965 to 1969, 54 patients underwent a Swenson's procedure at an age ranging from 12 to 18 months after a preliminary colostomy. 13 patients died between colostomy and definitive operation. From 1969 to 1977, the delay in performing corrective operation after colostomy was shortened and the definitive procedure was done between 6 and 11 months of age in 69 patients. No fatality was observed but in 9 cases a perianal dermatitis lowered the quality of functional results. From 1977 to Nov 1, 1980, progress in immediate nursing and total parenteral nutrition made colostomy unnecessary in 19 neonates and infants with severe Hirschsprung's disease. The definitive surgical procedure (mainly Swenson's) was performed before 3 months of age. The postoperative results were excellent. After more than 4 years of follow-up for the first cases, the good secondary results, the quality o defecation, and the shortening of total hospital time, allow us to advocate this method in selected cases.

Clear cell sarcoma of the kidney in children: a distinct entity.

Four cases of clear cell sarcoma of the kidney in children are reported. The specificity of the tumor is examined. The pathologic features, the clinical presentation, and the occurrence of bone metastasis are reasons to distinguish clear cell sarcoma from the Wilms' tumor group. The poor prognosis may be improved in the future by radical resection associated with polyvalent chemotherapy.
