Comparative bioavailability of three brands of ampicillin.

The relative biological availability (bioavailability) of three brands of ampicillin trihydrate capsules marketed in Canada was studied by means of a crossover experimental design and was evaluated by analysis of variance. After administration of a single oral dose (500 mg.) to volunteer subjects, the mean peak ampicillin serum concentration and the area under the time ampicillin serum concentration curve (AUC) were higher for one of the products than for the other two. A second study utilizing the same experimental design revealed no differences in these parameters when different production lots of the more available product were compared. Product bioavailability was not related to the in vitro dissolution time of the capsules. The experimental design and methods of statistical analysis are discussed.

A computer-assisted two-dimensional gel electrophoresis approach for studying the variations in protein expression related to an induced functional repression of NFkappaB in lymphoblastoid cell lines.

Strategies are needed for conclusive interpretation of two-dimensional gel electrophoresis (2-D PAGE) maps in order to identify pertinent differences in protein expression during regulation of the transcription of discrete sets of genes. The model used in this study was a human lymphoblastoid cell line in which a functional repression of the transcription factors NFkappaB was obtained by induction of overexpression of IkappaBalpha, a physiological inhibitor of NFkappaB. The analytical methodology used relies on the comparison of two sets of 2-D PAGE maps for detecting differences in protein expression between samples overexpressing or not overexpressing IkappaBalpha. The analysis was based on a combination of an automatic computerized analysis, constituting an actual aid for deciding, and of an interactive visual validation, corresponding to the interpretation of computer propositions. This strategy is proposed as a rapid way to detect potential variations in protein expression applicable to any biological model. In this study, correspondence analysis data made it possible to discrimate between the samples overexpressing or not overexpressing IkappaBalpha, and pointed out some of the potential meaningful spots characterizing the samples in which NFkappaB was active. Then, after visual validation of the computer data, 53 polypeptides were considered to be different in the two classes of gels. Five polypeptides were specifically found in both samples overexpressing IkappaBalpha. The overexpression of IkappaB also induced a lower expression of 11 polypeptides. Finally, 15 polypeptides were only expressed in samples in which IkappaBalpha was not overexpressed and, consequently, in which NFkappaB factors were active. Thus, these polypeptides are candidates for further analysis as putative target gene products of NFkappaB.

Carbohydrate determinants involved in both the binding and action of insulin in rat adipocytes.

The insulin receptor apparent affinity was markedly decreased in fat cells treated with lectins specific either for D-galactose (Ricinus communis agglutinin I, RCAI), D-mannose (concanavalin A, Con A, Lens culinaris agglutinin, LCA) or N-acetyl-D-glucosamine (wheat germ agglutinin, WGA), as indicated by a rightward shift of the binding competition curves and almost lineared Scatchard plots. Limulus polyphemus agglutinin (LPA), specific for sialic acid, was ineffective. All lectins enhanced 2-deoxy-D-glucose uptake with relative bioactivities (maximal lectin effect/maximal insulin effect) of 68-86%. Insulin and lectin stimulatory effects were antagonized by specific carbohydrates used as competitors and inhibited by cytochalasin B (70 microM). Maximal effects of insulin and lectins were not additive and were completely abolished in neuraminidase-treated fat cells. Lectins did not affect insulin degradation. These data show that sialylated glycosidic moieties containing D-galactose, D-mannose and N-acetyl-D-glucosamine units are involved in both processes of insulin 'high affinity' binding and activation of glucose transport but are not implicated in hormone degradation. They suggest that N-linked carbohydrate chains of the complex type may be essential for functional insulin receptor and post-receptor systems.

Utilization of autogenous corticocancellous bone graft from the distal tibia for reconstructive surgery of the foot and ankle.

Procurement of corticocancellous autogenous bone graft from any donor site is not without potential complications. Increased postoperative morbidity, fracture at the donor site, an additional surgical procedure, increased operative time, excessive blood loss, hematoma, and increased relative cost are all factors that must be considered, regardless of donor site. The authors have been using the distal tibia as a source of corticocancellous bone since 1988. A review of 16 patients with distal tibial bone grafts at our institution has demonstrated this area to be readily available and effective with limited morbidity. The distal tibial metaphyseal area has been found to be an excellent source of corticocancellous bone for grafting in reconstructive foot and ankle surgery.

Lateral ankle instability and the Brostrom-Gould procedure.

Lateral ankle instability can present as either acute or chronic. Proper identification of the etiology is necessary to determine the proper treatment plan. The Brostrom-Gould procedure, when properly used, can provide adequate correction of the unstable ankle. A review of the procedure, its indications, and the results of 44 patients who underwent the procedure will be discussed.
