Nodular regenerative hyperplasia of the liver: diagnosis by liver biopsy.

Specimens of liver obtained by needle biopsy from two patients with rheumatoid arthritis showed features of nodular regenerative hyperplasia. In one patient the nodularity was apparent on gross examination of the specimen. Portal hypertension was present in the other patient. The cause and pathogenesis of the disorder are poorly understood.

Microbiological and pharmacological evaluation, in mice and rats, of a new nitrofuran: 2-amino-4-methyl-6-(2-ethinyl-5-nitrofuryl)-pyrimidine (nifurpyrimidine).

2-Amino-4-methyl-6-(2-ethinyl-5-nitrofuryl)-pyrimidine (nifurpyrimidine) is a new furan derivative. Its spectrum of activity is wide and includes both gram-positive and gram-negative microorganisms and fungi; moreover, it inhibits the growth of multiple-drug resistant salmonellae. The minimal inhibitory concentrations ranged from 0.25 to 10 microgram/ml. Resistance was developed in vitro in a step-wise manner and was partially reversed after 7-10 passages in drug-free media. The pharmacological study of the drug was carried out in rats and mice and gave the following results: (1) no acute toxicity was demonstrable following oral administration; (2) no intestinal absorption occurred and virtually the whole dose of nifurpyrimidine administered by oral route was recovered in the intestinal content; (3) the presence of urine, plasma or feces did not modify the antibacterial activity of the compound.

Thallium-201 myocardial imaging in young asymptomatic subjects with "ischaemic" changes in exercise electrocardiogram.

In order to assess the significance of a positive electrocardiographic response to exercise test in 10 asymptomatic subjects with normal resting ECG, a myocardial perfusion scanning with Thallium-201 at rest and during exercise was performed. The following ECG tests were also performed on these subjects: forced hyperventilation, exercise test after nitroglycerin (TNG), exercise test after propranolol. Myocardial perfusion scans did not reveal any defect at rest nor during exercise. Hyperventilation determined abnormalities of ventricular repolarization in all subjects. TNG did not improve the response to exercise test and even decreased the ischemic threshold. After propranolol the electrocardiographic response to exercise became normal in 9 cases, while in 1 subject the ST depressions were impressively reduced. All subjects had a follow-up of 14 to 91 months. During this period none presented symptoms suggesting a coronary disease. This study suggests that combined evaluation of Thallium myocardial perfusion imaging during exercise and exercise ECG test after TNG is most useful in differentiating ischemic from nonischemic exercise ST depressions in asymptomatic population.

Hypogammaglobulinemia and nodular lymphoid hyperplasia of the gut.

A 34-year-old female had longstanding hypogammaglobulinemia associated with extensive nodular lymphoid hyperplasia (NLH) of the small bowel, colon and gallbladder. The characteristic appearance in double contrast barium studies should alert the radiologist to the possibility of hypogammaglobulinemia.

Behavioural effects of naloxone in rats.

Naloxone in rats induces a behavioural syndrome closely resembling that induced by intraliquorally injected ACTH peptides. This effect is probably due to a displacement of the ACTH peptides from other receptors (e.g. opiate receptors).

Hemolytic anemia in patients receiving sulfasalazine.

Hemolytic anemia is a well-recognized complication of sulfasalazine treatment. 17 of 40 (43%) patients with inflammatory bowel disease receiving sulfasalazine had evidence of hemolysis as detected by starch gel electrophoresis. Only 47% (8) of patients with hemolysis had Heinz body formation. The hemoglobin was significantly reduced in patients with hemolysis and 53% had a reticulocyte count of greater than 5%. A significant correlation was noted between hemolysis and serum sulfapyridine level, but no correlation was seen with serum sulfasalazine level. There was no significant difference in disease extent or activity in patients with hemolysis compared to those without hemolysis. Hemolysis is not a rare side-effect of sulfasalazine therapy. Heinz body formation is not invariably found in sulfasalazine-induced hemolysis.

Early gastric carcinoma: value of combined use of endoscopy, air contrast x-ray films, cytology, and multiple biopsy specimens.

Five cases of early gastric carcinoma were diagnosed during a one-year period at the Henderson General Hospital, Hamilton, Ontario, Canada. All five patients were symptomatic and were diagnosed by a combination of endoscopy, roentgenology, brush cytology, and biopsy specimens. No cases of early gastric carcinoma were diagnosed in our hospital in the preceding ten years. A total of 75% of the cytological preparations were positive and all five biopsy specimens showed carcinoma. The ulcerated or type III lesion was present in three cases. No lymph node metastases were present in the gastrectomy specimens. The increasing rate of detection of early gastric carcinoma is attributed to the use of upper gastrointestinal endoscopy, air-contrast barium meal examinations, cytology, and multiple biopsy specimens.

[Frequency of lung asbestos bodies in autopsy material at Udine].

Following chemical digestion of the pulmonary tissue, asbestos bodies were sought in 50 random necropsies carried out at Udine. The age of the subjects examined, 24 men and 26 women, was between 23 and 90, and 43 cases were older than 50. A positive finding was observed in 44 cases (88%).

Comparative tests with formolized and irradiated vaccines against leptospirosis.

The effectiveness of gamma-irradiated antileptospirosis vaccines and of vaccines killed with formol was compared in guineapigs. Irradiated vaccines proved no more effective than formolized vaccines in preventing either death or persistent kidney infection. Anti-pomona vaccines gave little protection against serotype icterohaemorrhagiae, and there was no significant difference between irradiated and formolized preparations. Vaccine prepared from a virulent strain showed little advantage over vaccine prepared from weak strains.
