Lymphoma with bilateral cavernous sinus involvement in early childhood.

A 4-year-old girl developed complete ophthalmoplegia with intact pupillary responses. Computed tomography and magnetic resonance imaging demonstrated a mass invading the cavernous and sphenoid sinuses and posterior ethmoidal cells. Biopsy revealed non-Hodgkin lymphoma. This patient is the youngest reported with malignant lymphoma of the cavernous sinus and the second reported with bilateral cavernous sinus involvement.

Acellular pertussis diphtheria-tetanus-pertussis vaccine containing separately purified pertussis toxoid, filamentous haemagglutinin and 69 kDa outer membrane protein as a booster in children.

In two double-blind, randomized, comparative studies involving a total of 218 children, an acellular pertussis (DTPa) vaccine containing diphtheria and tetanus toxoids and pertussis components filamentous haemagglutinin (FHA), pertussis toxoid (PT), and 69 kDa outer membrane protein (69 kDa OMP) was administered as a booster to 17-month-old and 5-year-old children with a history of routine whole-cell diphtheria-tetanus-pertussis (DTPw) vaccination. The control groups in these studies received DTPw vaccine. Among 17-month-old toddlers, significantly lower proportions of DTPa vaccine recipients had local pain (7.3%), redness (14.5%) and swelling (9.1%) than DTPw vaccine recipients (23.6%, 30.9% and 23.6%, respectively). A trend toward fewer local reactions was also seen in 5-year-old children vaccinated with DTPa in private practice and public clinics although differences were not statistically significant. Fever (rectal temperature > or = 38 degrees C) was reported more frequently for DTPw vaccine recipients in both age groups. While no differences existed between groups in terms of geometric mean antibody titres (GMTs) prior to booster vaccination, anti-PT antibody GMTs were higher among DTPa vaccine recipients than among DTPw vaccine recipients after booster vaccination. The difference was statistically significant in 5-year-old subjects. Furthermore, significantly higher anti-FHA and anti-69 kDa OMP GMTs were seen in DTPa vaccine recipients in both age groups. In pre-vaccination seropositive subjects and in pre-vaccination seronegative subjects the rate of immune response to pertussis antigens was higher for DTPa than for DTPw vaccine recipients with the exception of the rate of response induced to 69 kDa OMP in 5-year-old children.(ABSTRACT TRUNCATED AT 250 WORDS)

Take of rhesus-human reassortant tetravalent rotavirus vaccine in breast-fed infants.

Rhesus-human reassortant tetravalent rotavirus vaccine at a titer of 4 x 10(4) plaque forming units was evaluated for immunogenicity in 194 6-8-week-old breast-fed Turkish infants. The vaccine was administered orally as a single dose following either a meal of breast milk or 30 ml of sodium bicarbonate-buffered soy milk formula. Four-fold or greater responses in rotavirus IgA ELISA antibody were detected in 62% and 65% of the infants in the two groups, respectively (p = 0.62). In a smaller comparison group of non-breast-fed infants, an IgA response was detected in 7 of 11 (64%) cases. In all vaccinees, a serological response was detected in 72% of the initially seronegative and 47% of the initially seropositive infants (p = 0.001). We conclude that the take of rhesus-human reassortant tetravalent rotavirus vaccine in breast-fed infants is not compromised by breast feeding before vaccination. However, a higher titered preparation of the same vaccine may be required to improve overall immunogenicity in young infants, particularly in those with pre-vaccination rotavirus antibody.

Transient protein S deficiency with deep venous thrombosis during Salmonella typhimurium infection.

A patient with deep venous thrombosis and low protein S activity during the course of Salmonella typhimurium infection is presented. Although protein S deficiency has been reported in patients with disseminated intravascular coagulation, it was not present in this patient and his protein S activity was normal after the findings of infection and deep venous thrombosis disappeared.

Cytomegalovirus disease in a renal transplant recipient manifesting with pericarditis.

A 12-year-old uraemic patient who had received a renal allograft from a donor of unknown cytomegalovirus (CMV) serology was evaluated for subfebrile fever and pericardial effusion. Viral DNA was detected with PCR. The patient responded very well to Ganciclovir therapy. This patient highlights the importance of donor serology and the need for careful evaluation of even minor signs and symptoms of CMV disease.

[Fatal varicella pneumonia unresponsive to acyclovir therapy in a child with a malignancy].

Acyclovir has become the drug of choice for prevention of visceral dissemination of Varicella-zoster virus infections in immunocompromised individuals. This article describes a 6-year-old girl taking cytotoxic therapy and radiotherapy for treatment of Hodgkin lymphoma who developed cutaneous varicella infection. Despite the early administration of acyclovir a fatal varicella pneumonia occurred and she died on the 4th day of hospitalization. Since the resistance is inducible, the increase of unresponsiveness to acyclovir in immunocompromised hosts with varicella infection is a potential risk that can cause to increase in fatalities in these patients.

Two trials of an acellular DTP vaccine in comparison with a whole-cell DTP vaccine in infants: evaluation of two PT doses and two vaccination schedules.

The present trials being carried out in Switzerland and Turkey, the reactogenicity and immunogenicity of acellular DTP vaccines containing either 25 micrograms or 8 micrograms of PT and 25 micrograms of FHA were compared with those of a conventional whole-cell DTP vaccine during a primary vaccination course following either a 0-1-2 schedule starting at three months of age (Switzerland) or a 0-2-4 schedule starting at two months of age (Turkey). The whole-cell vaccine was associated with significantly more local reactions than the acellular vaccines; general reactions were also more frequent among whole-cell recipients. The 25 micrograms PT dose vaccine was no more reactogenic than the 8 micrograms PT dose vaccine. There was no evidence of increases in frequencies of local or general reactions from one acellular vaccine dose to the next. The 25 micrograms PT dose acellular DTP vaccine resulted in immune responses to all four antigens (PT, FHA, diphtheria and tetanus toxoids) at least equivalent to those observed with whole-cell vaccination. Significantly lower anti-PT antibody levels were seen with the 8 micrograms PT dose vaccine.

[Efficacy of intravenous ceftazidime in childhood infections].

Ceftazidime, a beta-lactamase resistant cephalosporin, was administered intravenously to 33 patients with various infections. The age range was between 3 days and 15 years and substantial underlying disease was present in 90% of the patients and 72% had malignancy. Seventeen (51.5%) were in critical condition and twenty had granulocytopenia. Clinical and microbiological results were evaluated and overall clinical response was 88%. No major adverse effects were encountered. Ceftazidime was an effective and safe therapy for serious infections and neutropenic patients.

[Convulsions in childhood Shigella gastroenteritis. An evaluation of risk factors].

Fifty-five patients with the diagnosis of shigella gastroenteritis were studied, 29 (52.7%) of whom had convulsions. Various symptoms and signs were compared between patients with and without convulsions to define risk factors for the development of seizures. To have a younger age and high body temperature were important predisposing factors. Hyponatremia and type of shigella organism were not contributing factors in the development of seizures.

High levels of lactate, pyruvate, and alanine in anemic children.

Lactate, pyruvate, and alanine levels were investigated in patients with iron deficiency anemia and other types of anemia. The study was carried out on 36 children between 2 and 16 years of age. The study group was divided into three groups, each comprised of 12 children. It was shown that the anemic children have lower PO2, higher PCO2 and higher levels of lactate, pyruvate, and alanine than the control group (p less than 0.05). There was a negative linear correlation between lactate levels and hemoglobin values (r = -0.6213; p less than 0.05), but no correlation between hemoglobin and the alanine and pyruvate levels. Levels of lactate, pyruvate and alanine were similar in the iron deficiency and the other anemia groups. In conclusion, iron has no direct effect on the levels of lactate, pyruvate, and alanine, but the hypoxemia caused by anemia could be responsible for the higher levels of the lactate, pyruvate, and alanine. In cases with high levels of lactate, pyruvate, and alanine, anemia has to be considered in the differential diagnosis of hyperlactatemia.
